Mr. Mao discusses the syndrome of pediatric convulsion with accumulation: The child suddenly presents with fever, rapid breathing, a low-pitched cry, and upon waking from sleep at night, sensation of movement within the abdomen. This syndrome arises from prolonged fright causing mental unrest, which then transmits to form this condition. Treatment involves using Niuhuang Gao (Civet and Calculus Bovis Ointment; see chapter on Diaphragm Heat) combined with Zhusha Gao (Cinnabar Ointment; see chapter on this section) for regulation, leading to recovery.
The song from *Wu Jue* regarding the convulsion with accumulation syndrome: (First use Zhen Wuxi Gao [True Rhinoceros Horn Ointment; see chapter on Acute and Chronic Convulsions], then regulate Qi, calm the Heart, and resolve phlegm.)
Pediatric convulsion with accumulation is often caused by phlegm-saliva, with accumulated heat transmitting through the Liver and Heart organs. Symptoms include qi stagnation with choking, cyanotic complexion, increased phlegm, weak Stomach Qi leading to frequent vomiting, and various types of diarrhea.
The song from *Shibi Jing* regarding the thirty-six types of internal convulsion with gradual heat syndrome:
Gradual heat is often due to accumulation in the Spleen; stool resembles white earth or grease. In the morning, internal heat causes frequent sweating, followed by irritability, thirst for water, and crying. If the condition turns deficient with intensified heat, cooling the Heart is appropriate. Observe the ears as a diagnostic sign: redness indicates Wind, while blackness follows Heat.
Due to deep-seated accumulation, abdominal heat occurs, along with heat in the palms and soles. Another convulsion manifestation includes drinking cold water, increased irritability and thirst, nighttime crying, and diarrhea resembling bean paste. If untreated, deep sleep accompanied by limb twitching ensues. Treatment requires separating fluids and grains, regulating Qi, gradually eliminating accumulation, stopping thirst, and inducing sweating for recovery. (*Fengsui Jing* contains identical song verses, noting: administer Shengyin Wan [Raw Silver Pill] for Wind, or Jinye San [Gold Leaf Powder] for Heat. Shengyin Wan formula is identical to *Yu Jue*, see Acute/Chronic Convulsions chapter; Jinye San formula is in this section.)
The forty-eight manifestations of convulsion with gradual heat in *Xiao'er Xingzheng Lun* are identical, adding: This syndrome is convulsion-accumulation combined with tidal fever; morning forehead sweating is the characteristic sign. Administer Wan pills (see Epilepsy chapter) for peace. If fever persists, mild purgation followed by Qi regulation is used.
