Wang of Handong's *Jiabao*: When a child has fever occurring twice a day, in the morning and evening, it is called "Jing Re" (Fright Fever), commonly known as "Chao Re" (Tidal Fever).
Wang of Handong's *Jiabao* on Chao Re Syndrome: Pediatric tidal fever is generally caused by excessive congestion of Qi and Blood, where heat generated by the Five Zang organs steams and diffuses outward, causing fever. It is largely similar to "Wen Zhuang" (Warmth with Steaming Heat), but may also be accompanied by latent heat or residual cold. If accompanied by latent heat, the stool is yellow and foul-smelling; if accompanied by residual cold, the stool is white and sour-smelling. This is due to disharmony of the Zang-Fu organs, where both cold and hot Qi are excessive, leading to accumulation in the intestines and stomach, which constitutes the pattern of Wen Zhuang. For cases where heat is not severe, it is appropriate to administer Maidang Tang (Wheat Decoction) three or two doses to resolve it (formula found in the section on Cold Damage with Macules), followed by Jinlian San (Golden Lotus Powder) three or two doses to reduce fever (formula found in the section on Cold Damage with Fright). Additionally, Qibao Qingqing Dan (Seven Treasures Light Green Pill) three or two doses should be taken (formula found in the section on Simple Cold Damage). If the fever does not subside after following this sequence and instead intensifies, with frequent fright during sleep, Tie Xian Gao (Iron Saliva Ointment) three or two doses should be administered (this formula is found in this section). If the fever persists despite using this ointment, it indicates the presence of Fright Accumulation or Food Accumulation, which requires purgation. Following the aforementioned methods, Shuijing Dan (Water Essence Pill) one dose should be used to promote evacuation (formula found in the section on Cold Damage with Food Stagnation). By dawn, the discharge of five-colored accumulated toxins, appearing as sticky, slippery substances resembling fish saliva, confirms efficacy. It is then appropriate to use Renshen San (Ginseng Powder) (formula found in the section on Cold Damage with Food Stagnation) and regulate with Guanyin San (Avalokitesvara Powder) (formula found in the section on Disharmony of Stomach Qi). If residual heat remains, Qingqing Dan (Light Green Pill) should be used to reduce fever.
Qian Yi's Inquiry on Tidal Fever: The son of Xu from the capital, aged three, suffered from tidal fever. Every day at dusk, he experienced convulsions, slight body heat, slightly deviated eyes with eyelids retracted, cold limbs, and wheezing, with slightly yellow stool. Qian Yi treated him together with Physician Li. Qian Yi asked Physician Li, "What causes the convulsions?" Li replied, "It is Wind." "Why is the body heat only mild?" Li said, "It is due to the condition of the limbs." "Why are the eyes deviated and eyelids retracted?" Li said, "The eyes deviate during convulsions." "Why are the limbs cold?" Li said, "Cold extremities necessarily indicate internal heat." "Why is there wheezing?" Li said, "It is due to severe convulsions." "How should it be treated?" Li said, "Instill Tianjing Wan (Sneeze-Startle Pill) into the nose, and the convulsions will surely stop." Qian Yi asked again, "Since it is stated to be Wind disease with Warmth and Steaming Heat, convulsions drawing the eyes to deviate and retract, internal heat with cold limbs, and wheezing due to severe convulsions, what medicine treats all these?" Li said, "All are treated with this same medicine." Qian Yi said, "Incorrect. Convulsions indicate Liver Excess, hence the convulsions. Slight body heat at dusk is due to Lung Tidal Fever taking effect; the Lung governs body warmth, and heat here indicates Lung Deficiency. Therefore, the deviated and retracted eyes result from Liver Wood overcoming Lung Metal. Cold limbs indicate Spleen Deficiency. If the Lung is severely deficient, its mother organ, the Spleen, is also weak; Wood Qi attacks the Spleen, causing cold limbs. Treatment should first employ Yihuang San (Yellow Protecting Powder) (formula found in the section on Disharmony of Stomach Qi) and Ejiao San (Donkey-Hide Gelatin Powder) (formula found in the section on Wheezing and Cough in the Upper Jiao). After the Spleen Deficiency symptoms recede, treat with Xieqing Wan (Draining Green Pill) (formula found in the section on Fright Heat), Daochi San (Guiding Red Out Powder) (formula found in the section on Excess Heat), and Liangjing Wan (Cooling Fright Pill) (formula found in the section on All Kinds of Fright). Nine days later, the child recovered."
