Mao Xiansheng records a condition in newborn infants called "Zhong Husun Jin" (Mid-Monkey Lockjaw): the body has a slight fever, hands are clenched and pressed against the chest, lips are puckered and cannot open, shoulders are hunched, and the body posture resembles an active monkey. This condition arises because during the sixth month of pregnancy, the mother saw someone playing with a monkey, opened her mouth to inhale its breath, and after birth, the infant was struck by wind-pathogens, triggering this condition. If not treated promptly, it will result in death.
The *Experience Formulas* treat pediatric "Husun Jin".
Burn cat feces to produce smoke, and fumigate the infant to resolve the condition.
According to *Chao Shi Bing Yuan* on the "White Tiger Condition" in children: The Kan Yu calendar's You Nian Tu mentions the White Tiger deity: when the Tai Sui is in Mao, the White Tiger is in Yin; deducing from this, one can know the location of this deity. If a child's dwelling offends this deity, illness ensues. The symptoms include fever, occasional crying and calling, occasional coldness of the body, fingers flexing and extending as if counting, resembling wind-phlegm epilepsy, but without convulsions of the limbs.
Zhang Huan's *Shun Zheng Ji* prescribes Xiang San (Fragrant Powder) to treat the White Tiger disease.
Ingredients: Jiangzhen Xiang (Aquilariae Lignum Resinatum), Chen Xiang (Aucklandiae Radix), Tan Xiang (Santalum Album Lignum), Ruxiang (Olibanum, ground), Anshixiang (Styrax), Ren Shen (Ginseng Radix et Rhizoma, remove rhizome, each 1 liang), Fushen (Poria Cum Radice Pinelliae), Suanzaoren (Ziziphi Spinosae Semen, each half liang).
Crush and sieve the above ingredients into fine powder. Take 1 qian per dose, add to 0.8 zhan of water, add a small amount of Shexiang (Moschus), decoct until 0.5 zhan remains, strain, and take warm. Retain the dregs and burn them before sleep.
(Thesis on Epilepsy Conditions) Twelve categories in total.
*Qianjin Fang* states: The reason children suffer from epilepsy (Xian Bing) and tetanus/spasm (Jing Bing) is due to imbalance of visceral qi. Those who have epilepsy immediately upon birth have five viscera that fail to converge, blood and qi that fail to gather, five meridians that do not flow, and skeletal weakness with incomplete development; they often do not survive to adulthood. Those who develop epilepsy between forty days past one month up to one year of age also suffer from improper nursing and rearing, disharmony of blood and qi, and invasion of wind pathogens. (*Ying'er Fang* states: For those developing epilepsy between thirty days past one month up to one year of age, it is due to non-flowing five meridians, skeletal weakness, improper nursing, or wind pathogen invasion.) If the onset involves fever, spasms, and startled crying before the epilepsy attack, and the pulse is floating, it is Yang Epilepsy. The pathology is in the Six Fu organs, superficial in the skin and muscles, and is relatively easy to treat. If the onset involves coldness, no startling spasms, no crying, and the pulse is deep, it is Yin Epilepsy. The pathology is in the Five Zang organs, deep in the bone marrow, and is extremely difficult to treat. If the body is soft during attacks and consciousness returns intermittently, it is called Epilepsy. If the body is rigid and arched like a bow, and consciousness does not return, it is called Tetanus/Spasm. For all cases of arching, if an adult's hand fits sideways under the spine, or three fingers fit for a child, it is incurable. Floating and deep pulses are used to distinguish whether the pathology is in Yin/Yang or Exterior/Interior. Within these, there are further variations such as large/small, slippery/choppy, deficient/excess, and slow/rapid signs; treatment should be based on the pulse morphology. *Shennong Bencao Jing* states there are 120 types of pediatric startled epilepsy. Any symptom profile slightly different from normal constitutes an epileptic sign. At birth, if blood vessels are not converged and viscera are unformed, slight improper care leads to illness. Children who frequently fail to mature properly may fall ill after meridian transformation periods; among residual signs, wind-stroke is the most sudden. If a child's limbs show poor movement, startled spasms, and slight abnormal breathing, epilepsy is impending. If the transformation period passes without resolution, Longdan Tang (Gentian Decoction) is suitable (see All Epilepsy Section).
Pediatric epilepsy is divided into three types: Wind Epilepsy, Startled Epilepsy, and Food Epilepsy. Wind and Startled epilepsies occur frequently; only one or two out of ten cases are Food Epilepsy. Cases starting with chills followed by fever are always Food Epilepsy. Startled Epilepsy should be treated with acupuncture and moxibustion according to diagrams. Wind Epilepsy should be treated with Zhu Xin Tang (Pig Heart Decoction). Food Epilepsy requires purgation to cure; Zi Wan (Purple Pill) is preferred (see Transformation Section).
Wind Epilepsy occurs because clothing is too warm causing sweating, allowing wind pathogens to enter. In Wind Epilepsy, initial symptoms include fingers flexing as if counting before onset. In Startled Epilepsy, onset follows fright and loud crying. Mild Startled Epilepsy may stop spontaneously if held tightly and protected from further fright. If a child refuses nursing, vomits, develops heat, and then has an epileptic seizure, it is Food Epilepsy; early purgation leads to recovery. Siwei Zi Wan Zhupi Yin (Four-Ingredient Purple Pill for Expelling Accumulations) is best, removing disease rapidly without damaging healthy qi. Chi Wan (Red Pill) is inferior but used for severe cases. (Note: No original formula for Chi Wan exists; it is suspected to be related to the Eight-Ingredient Zi Shuang Wan containing Cinnabar, Croton Seed, and Kansui.)
For infants unable to nurse, Zi Wan should be used to purge. Newborns have vigorous vital qi; even mild stagnation requires purgation to prevent damage and ensure deep healing. Delayed purgation leads to severe, hard-to-treat disease. Siwei Zi Wan is optimal as it removes disease without harming the patient. If Siwei Zi Wan fails to induce purgation, use Chi Wan, doubling the dose if necessary. If residual heat remains after purgation, administer Longdan Tang moderately and apply Chiga (Red Ointment). Wind Epilepsy also requires purgation, delivered via Zhu Xin Tang. Startled Epilepsy requires only acupuncture/moxibustion and application of Sheng Gao (Living Ointment); heavy purgation is contraindicated because it causes internal deficiency and worsens the unstable heart qi. Severe Startled Epilepsy is particularly difficult to treat. Therefore, caregivers must protect children from loud noises, handle them gently to avoid fear, and block ears during thunderstorms while making soft noises to distract them. Mild startling nourishes blood vessels; excessive startling requires moxibustion on the Jing Mai (Startle Meridian). Moxibustion at 50-60 days may worsen fright; it is better after 100 days. Fever, refusal to nurse, restlessness, and frequent startling indicate early epilepsy; Zi Wan cures it. For minor startling during sleep, administer Zi Wan once a month to reduce excess qi and prevent epilepsy. After Li Xia (Start of Summer), avoid reckless moxibustion. Use Rechu Tang Bath (Heat-Removing Decoction Bath) and Rechu San Powder (Heat-Removing Powder), apply Rechu Chiga, and rub ointment on the umbilicus. Keep the child cool, allow free access to fresh water. Thin clothing leads to undigested milk, sour-smelling stools, indicating early Pi Ji (Accumulation); treat mildly with Zi Wan. Start with small doses to maintain loose stools, avoiding heavy purgation. Reduce dosage as stools normalize. Winter illnesses are easier to manage than summer ones. Post-purgation abdominal fullness requires temporary reduction of nursing. Nursing amounts should increase gradually with age; decrease indicates internal disharmony. Withhold solid food, give only milk, and medicate lightly for 5-10 days depending on severity. Refusal of solids in favor of milk indicates significant accumulation requiring purgation; failure to purge leads to alternating chills/fever, vomiting-induced epilepsy, or dysentery. Early treatment of mild cases prevents deterioration. Yellow, foul-smelling stool indicates latent heat; use Longdan Tang mildly. White, sour stool indicates retained cold; use Zi Wan. Adjust dosage based on severity and restrict nursing to restore stomach qi. Unrestricted nursing leads to recurrence and gastric damage from repeated purgation. Large pulses in children with accumulations predict epilepsy (Food Epilepsy); purgation cures it. Monitor pulses in the palm and three fingers; early purgation prevents pulse manifestation and difficulty in treatment. Abdominal disease causes chills/fever, moving blood vessels, unsettling the heart, leading to fright and rapid epilepsy onset.
*Ying Ru Shu* discusses pediatric startled crying and limb spasms during sleep with unresolved transformation; caution against acupuncture or scratching as it moves hundred meridians and causes epilepsy through fright. Only Yin Epilepsy, Lockjaw, and Spasm may be treated with acupuncture/scratching.
*Ying Tong Bao Jian* discusses conditions transforming into epilepsy. Minor illnesses should be treated early to avoid fatal outcomes. Various chronic untreated diseases transform into epilepsy: prolonged high fever, cold with fright, phlegm-fluid retention, continuous fright, persistent diarrhea, chronic cough, night crying, Ke Wu (Guest Malevolence) attacks, cholera/vomiting-diarrhea, nursing issues, and umbilical ulcers with discharge. All these can transform into epilepsy.
*Hui Yan Guan Zheng* states: Untreated fright becomes wind, untreated wind becomes epilepsy, untreated epilepsy becomes spasm; once spasm sets in, medication is difficult. Epilepsy has subtypes: Jing Feng Xian (Startled-Wind Epilepsy) triggered by much fright; Ru Feng Xian (Milk-Wind Epilepsy) triggered by milk rushing to lungs, causing immediate vomiting; Gan Feng Xian (Liver-Wind Epilepsy) common in all seasons, presenting with chills, fever, and lockjaw; Fei Feng Xian (Lung-Wind Epilepsy) common in winter, presenting with lockjaw and black face. Generally, epilepsy does not kill directly but evil qi resides in the heart; heavy emetic/purgative therapy and regular Zhen Xin Tang (Calm Heart Decoction) are needed. If one side of limbs moves during onset, emetics are contraindicated. Numbness, inability to raise arms, unclear speech, and black teeth indicate wind-phlegm and evil qi blocking orifices.
