Xieqing Wan (Drain the Liver Pill): This is the principal formula for treating Liver channel diseases. In pediatric practice, formulas used to arrest wind and stop convulsions often utilize mineral substances (Jinshi) and aromatic opening-orifice agents like Borneol (Nao) and Musk (She), which are of no benefit and may cause harm. Only this formula clears Heart fire, calms the Liver, disperses wind, and cools blood. Whenever a child has persistent fever that is about to develop into wind-convulsion (Feng Chu, i.e., convulsions due to liver wind), or has already developed such convulsions, administering this pill yields immediate and effective results. Although this is an ancient formula, people do not know how to use it. When I was traveling in Tanzhou in the past, my teacher instructed me on its application, and only then did I learn to use it. I reject all other pediatric formulas such as Baolong Wan, Baoming Dan, Zhibao Dan, Xin'an Jin Yao, and Suhe Xiang Wan. This formula alone is the premier miraculous prescription for arresting wind and stopping convulsions in pediatrics.
Chuan Qiang Huo (Notopterygii Rhizoma et Radix), Zheng Chuan Xiong (Ligustici Chuanxiong Rhizoma), Hei Shan Zhi (Gardeniae Fructus), Long Dan Cao (Gentianae Radix), Quan Dang Gui (Angelicae Sinensis Radix), Bei Fang Feng (Saposhnikoviae Radix) (each 1 liang), Jin Zhuang Huang (Rhei Radix et Rhizoma) (5 qian).
Mix the above herbs together, dry them with heat, grind into a fine powder, bind with refined honey to form pills, coat with Qing Dai (Indigo Naturalis), size of soybeans. Take 1–2 pills per dose, dissolve and take with clear tea.
Dao Chi San (Direct the Red Powder): Treats Heat in the Heart channel; effective for all cases of irritability, crying, and restlessness.
Huai Sheng Di (Rehmanniae Radix Preparata) (2 qian), Chuan Mu Tong (Akebiae Caulis) (1.5 qian), Ku Huang Qin (Scutellariae Radix) (1 qian), Sheng Fen Cao (Glycyrrhizae Radix et Rhizoma Preparatum) (0.5 qian), Dan Zhu Ye (Lophatheri Herba) (14 slices), Deng Xin (Junci Medulla) (10 stems).
Decoct in water and take hot.
Pediatric nocturnal crying presents several syndromes: there is Cold in the Viscera, Heat in the Heart, Disquieted Spirit (Shen Bu An), and Stubborn Crying (Ao Ku). The cold and heat patterns differ significantly within these, requiring careful differentiation.
In cases of Visceral Cold, Yin predominates at night; when night falls, extreme Yin causes restlessness, severe cold leads to abdominal pain, crying stops upon abdominal pressure but resumes when the hand is removed. External signs include a cyan complexion, cold hands and feet, refusal to suckle milk, and ceaseless nocturnal crying. Treat with Jiawei Danggui San (Modified Angelica Decoction).
In cases of Heart Heat causing irritable crying, the face is red, the tongue is crimson, or the tongue coating is white and rough. Crying subsides slightly in the absence of light but intensifies upon exposure to light. Treat with Dao Chi San added with Mai Dong (Ophiopogonis Radix) and Deng Xin (Junci Medulla); if severe, add Chuan Lian (Coptidis Rhizoma) and Dan Cao (Gentianae Radix).
In cases of crying due to Disquieted Spirit, the child startles and palpitates during sleep, cries loudly while clinging to the mother, and has a purplish-black complexion, indicating deficiency of the Spirit and fright. Treat with Anshen Wan (Calming the Spirit Pill) to stabilize the mind. Nocturnal crying following vomiting/diarrhea or after severe illness is also due to insufficiency of Heart Blood, treated similarly.
Nocturnal crying that ceases immediately upon seeing a lamp is merely a habituated response to lighting, classified as Stubborn Crying (Ao Ku), and is not actually a disease. Do not light lamps at night; allow the child to cry freely, and they will settle naturally within two or three nights.
