(Comprising twelve verses, involving eleven prescriptions)
It is most difficult to treat smallpox and chickenpox in women because the female constitution is Yin, and blood deficiency is inherent. When the rash is about to erupt, one must worry that menstruation may occur during this critical period.
The *Zheng Li Lun* (Theory of Orthodoxy) states: This condition is even more pronounced in infants and women. Since women possess a Yin constitution, their blood is often insufficient. The treatment of smallpox/chickenpox always prioritizes Qi and Blood;
if either is deficient, complications will arise. Therefore, for women over fourteen years old who contract smallpox/chickenpox, there is constant fear that menstruation will cause blood loss and Qi deficiency, often leading to the internal collapse and sinking of the toxin.
Fever accompanied by out-of-cycle menstruation indicates that Fire pathogen is forcing blood to circulate recklessly. It is imperative to urgently cool the blood and stop bleeding; do not wait until Spleen Qi is depleted, as regret will come too late.
During fever from smallpox/chickenpox, if menstruation flows irregularly, it is not the normal menstrual cycle. This is due to internal accumulation of toxic heat disturbing the Sea of Blood, forcing the vessels to bleed abnormally, causing menstruation to be untimely. One should use Xuanshen Dihuang Tang (Scrophularia and Rehmannia Decoction),
or Siwu Tang combined with Huanglian Jiedu Tang, focusing on cooling the blood, with the goal of stopping the bleeding. If bleeding persists for a long time, Spleen Qi weakness may lead to the sinking and entrapment of the toxin.
If fever coincides with the onset of menstruation, the accumulated impurities are expelled, and the toxin is lightly resolved and clearing. However, if it continues beyond four or five days, one must consider tonification; warming the meridians and tonifying Qi will naturally calm the heat.
When fever occurs and menstruation happens simultaneously, the accumulated waste is removed, and the toxin is mildly resolved; no specific treatment is needed. If it does not stop after four or five days, it is because Heat pathogen takes advantage of the emptiness of the Blood Chamber,
forcing blood to flow recklessly. One should first administer Xiaochaihu Tang with Sheng Dihuang (Minor Bupleurum Decoction plus Fresh Rehmannia) to clear heat from the Blood Chamber, followed by Shiquan Dabu Tang (Ten Significant Tonifying Decoction) to tonify Qi and Blood, ensuring the rash emerges evenly, rises easily, and crusts easily.
If menstruation stops abruptly during fever, it is due to the emptiness of the Blood Chamber, which predisposes to irritability and oppression. If delirium and unclear consciousness appear, it indicates Heat has entered the Uterus; medical intervention must not be delayed.
If menstruation stops exactly when fever begins, Chaihu Siwu Tang (Bupleurum and Four Substances Decoction) should be taken early to prevent the emptiness of the Blood Chamber, avoiding invasion by toxin pathogen which could cause other diseases. If chills and high fever set in, with unclear spirit,
delirious speech, and confused language, this is Heat entering the Blood Chamber. Use Siwu Tang combined with Daochi San (Guiding Out Redness Powder) alternately with Anshen Wan (Calming Spirit Pill).
Menstruation occurring once every three months is called Ju Jing (Resident Menses); such a long interval makes the body unable to withstand the assault of smallpox/chickenpox. One worries about stagnation in the Sea of Blood and hidden toxins in the Bao Men (Womb Gate).
Amenorrhea in women is called Ju Jing; fullness without discharge indicates pathology in the Heart and Spleen. The Classics state: Diseases of the Two Yang (Stomach and Large Intestine) originate in the Heart and Spleen, leading to amenorrhea in women. The toxin of smallpox/chickenpox originates in the Heart and is governed by the Spleen;
if the Heart and Spleen are diseased first, the Blood Chamber fails to function, and the Chong and Ren meridians accumulate filth. Once the fire of smallpox/chickenpox becomes depressed in the Mingmen (Gate of Life) and Bao Hu (Uterine House), preventing eruption, the toxin remains trapped,
leading to aberrant complications. Therefore, at the initial stage of fever, one must cleanse the stagnation using Taoren Chengqi Tang (Peach Kernel and Rhubarb Decoction), followed by regulation with Siwu Tang combined with Yunqi San (Harmonizing Qi Powder).
Prolonged metrorrhagia leads to blood depletion; discharge without fullness leaves one wondering if there is excess or deficiency. How can one endure a pandemic smallpox/chickenpox? Supporting the weak and stabilizing the precarious depends on subtle respiratory and vital adjustments.
If a woman has had continuous metrorrhagia, her Qi and Blood are already deficient. If she contracts pandemic smallpox/chickenpox, she cannot tolerate the toxin; the primary treatment must be massive tonification of Qi and Blood using Shiquan Dabu Tang (Ten Significant Tonifying Decoction). If the rash appears grayish-white, flat, and sunken,
difficult to rise and hard to crust, add 1-2 qian of Shu Fuzi (Prepared Aconite).
When the rash rises and forms pus, but menstruation suddenly starts, Qi scatters with Blood; how can it stop? If appetite is good and Stomach Qi is sufficient, there is little to worry about; but if appetite is poor, one must guard against sinking and entrapment patterns.
After the rash appears, it is best for both exterior and interior to be solid, and for food intake to increase. If, during the rising and pus-forming stage, menstruation suddenly occurs, people only fear that foul qi will attack healthy Qi alone, unaware that
the body's own blood deficiency is the issue. When blood is lost, Qi also becomes deficient, allowing toxin pathogen to sink internally. Only those with strong Yuan Qi and good appetite will avoid this change. For those with Qi deficiency and poor appetite, sinking patterns invariably occur; use Shiquan Dabu Tang (Ten Significant Tonifying Decoction). In severe deficiency cases, add Shu Fuzi (Prepared Aconite) and Lurong (Deer Antler Velvet).
Sudden aphonia during menstruation indicates toxin invasion; blood loss dries fluids, causing sudden loss of voice. Nourishing blood and unblocking the Heart restores speech; a moment of voice is worth a thousand gold.
In women with smallpox/chickenpox, if menstruation suddenly starts, sudden aphonia and inability to speak occur. The Heart governs blood, and the tongue is the sprout of the Heart; blood loss causes Heart deficiency, so the Shaoyin meridian cannot nourish the tongue,
resulting in sudden loss of voice and muteness. First, use Danggui Yangxin Tang (Angelica Tonifying Heart Decoction) to nourish Heart blood and open the Heart orifices, restoring speech naturally, then regulate with Shiquan Dabu Tang (Ten Significant Tonifying Decoction).
Rising phase accompanied by menstruation indicates aberrant symptoms; internal deficiency and sinking require early planning. Effective medicine hitting the target ensures auspicious outcomes; adverse signs and ignoring advice lead to discord.
Heavy menstruation prevents the rash from rising; if it is not plump, full, red, or vibrant, but instead flat, sunken, grayish-white, green-dry, or black-sunken, this is a sign of interior deficiency with re-entering toxin. Use Shiquan Dabu Tang (Ten Significant Tonifying Decoction) alternately with Duoming San (Life-Saving Powder). If the rash is plump, red, and bursting, or if a second wave emerges from empty pustules, it is an extremely auspicious omen; however, if accompanied by abdominal distension, wheezing, delirium, restlessness, shivering, teeth grinding, and cold extremities, death is certain.
Pregnancy with smallpox/chickenpox has authentic transmission; the priority is always fetal stabilization. Do not allow the fetus to move lightly; if the fetal root moves independently, life ends.
For pregnant women with smallpox/chickenpox, fetal stabilization is paramount; do not disturb the fetus. During initial fever, use Shen Su Yin (Ginseng and Perilla Drink) to disperse; after the rash appears, taking Antai Yin (Fetal Stabilizing Drink) is advisable. If rising and crusting are delayed, use Shiquan Dabu Tang (Ten Significant Tonifying Decoction) minus Rougui (Cinnamon Bark).
When the rash is at its peak and labor approaches, few know the rules; gently expelling lochia and tonifying Yuan Shen yields profound effects.
For pregnant women with smallpox/chickenpox at the peak stage who go into labor, the main treatment is massive tonification of Qi and Blood using Shiquan Dabu Tang (Ten Significant Tonifying Decoction). If there is slight abdominal pain, it indicates incomplete expulsion of lochia; use Shenghua Tang (Generation and Transformation Decoction) to cleanse it.
Postpartum smallpox/chickenpox: with the fetus gone, there is no need for anxiety. Rely solely on tonification for efficacy; avoid cold and anger to protect generating Qi.
For postpartum women with smallpox/chickenpox, the main treatment is massive tonification of Qi and Blood using Shiquan Dabu Tang (Ten Significant Tonifying Decoction). The Baishao (White Peony Root) should be stir-fried with good wine; do not recklessly use cold-natured herbs, as they damage the generating Qi.
