痘疹毒从何起？母胎火毒流传。生来秽物下喉咽，藏在命门里面。一旦天行时气，感令相火熬煎。毒从骨
髓见皮间，彼此一般传染。
五脏各有形证，认时须要分明。往来寒
热睡脾经，呵欠顿闷肝证。咳嗽喷嚏受肺，面红惊悸属心。惟肾清静忌邪侵，手足耳尻俱冷。
五脏各有一证，其间治法难同。肝为水肺泡为脓，大小疮形异种。脾证发为疹子，心经现作斑红。肾为黑陷
病多凶，纵有灵丹何用！
痘疹要知顺逆，天时人事相随。大端阳火是根基，若遇阴寒不喜。春夏发而多吉，秋冬逆以何疑。如逢稠密
必凶危，稀少方为平易。
治法而今不定，清凉温补分明。各持一见论纷纭，自曰予为神圣。解毒喜行凉泻，补中爱使辛温。不明时令
与元神，枉自捕风捉影。
假令天时暄热，辛温助为灾殃，严凝凉解雪加霜，病者如何抵挡？壮实再行温补，虚 又使寒凉。虚虚实实
伐元阳，无异隔靴爬痒。
看取时行疫疠，天时热气炎炎。精神肥健食能兼，解毒清凉甚便。若是风寒太甚，虚羸吐泻连绵。此宜温补
法为先，又在医人活变。
痘疹要知轻重，吉凶顺逆精通。毒轻疮少顺家风，汤药不宜妄用。疮密毒重为逆，皮肤寸寸成脓。此般形证
例多凶，仔细扶持休纵。
轻者三四次出，头面胸背稀疏。小便清利大便稠，饮食如常充足。重者遍身齐出，状如麻子麦麸。咽疼泄泻
闷悠悠，饮食不思可恶。
多有先轻后重，只因触冒风寒。房劳不避秽腥干，食饮偏于冷暖。闲杂人带秽厌，诸般禽兽盘桓。庸工术浅
误汤丸，反使痘疮变换。
重者变轻何以？常常和暖衣衾。房中谨密少人行，饮食随时添进。未见误投汤药，不曾妄啖酸腥。此为人事
夺天灵，安可归于有命！
要识痘疮死证，无过五证分明。紫黑喘渴闷何宁，痒塌切牙寒噤。灰白顶陷腹胀，皮嫩易破成坑。泄泻气促
见鬼神，声哑头面足冷。

致倾亡，枉使魂灵飘荡。
大抵痘疮未出，先须升葛参苏。如斯不出汗忙疏，红点见时药住。大便若还秘结，轻轻四顺相符。假饶自利
伐无辜，定与阎罗掌簿。
调痘无过二法，补中解毒兼行。补中参术草 苓，枳实山楂有应。解毒芩连栀柏，连翘枳实防荆。芎归养血
妙如神，加减消详前定。
血气要分虚实，但于疮色推求。紫肿红 血实由，四物内加解毒。灰白中陷气弱，四君子是良谟。略加解毒
药相符，补气实脾无误。
但是痘疮初出，如逢热盛昏醒。解毒发散药先行，莫待临渴掘井。桔梗升麻干葛，连翘甘草黄芩。牛蒡栀子
木通荆，蝉蜕防风作引。
若是如常潮热，只消干葛升麻。芩连甘草赤芍加，牛蒡连翘无价。或用参苏饮子，青皮木香内加。煎来一服
胜灵砂，痘见表疏才罢。
初热多生搐搦，急将导赤疏通。木通甘草与防风，生地黄连同用。再加辰砂调服，须臾救护朦胧。此方端的
有神功，管取行之必
中。
壮热不曾出现，大便秘结难通。癫狂唇裂眼珠红，此证凶险堪恸。急与芩连栀柏，大黄酒炒疏中。连翘牛蒡
与木通，贯众射干俱用。
自此出而稠密，认他虚实调医。虚家泄泻色如灰，大补十全堪取。若是肿 红绽，芩连栀柏芎归。翘蒡升
葛枯甘奇，此个真机妙秘。
灰白不能起发，又加泄泻频频。温中妙药不宜停，急急扶危济困。当归黄芩芍药，甘草干姜人参。木香栀
子及青陈，官桂丁香灵应。
毒甚常生咽痛，可怜饮食难尝。射干甘桔最相当，连翘升麻牛蒡。若是痘堆颈项，此名锁膈凄惶。一朝破
烂命将亡，变作喑哑水疮。
起发状如蚕壳，干枯不见水浆。此名血竭毒归藏，不治必然命丧。当归地黄养血，参 甘草温良。连翘牛
蒡与木香，桔梗青皮发旺。
起发常将捻视，切防黑陷来攻。若然黑陷现其中，药点许多妙用。豌豆七粒烧过，乱发火锻和同。珍珠水
浸胭脂红，针破搽之自肿。
药点反加黑陷，丧门吊客匆匆。百祥牛李与宣风，总是脱空卖弄。不如人猫鸡犬，四般举 和同。木香汤
引妙无穷，妙法半文不用。
起发初生瘙痒，比于痒塌争差。伤寒身痒表留邪，痘亦同兹休讶。治用疏风凉血，荆防翘蒡芎麻。地黄归
葛效堪夸，竹叶木通无价。
大抵痘宜胀痛，最嫌虚痒相干。只因饮食湿邪攒，心火克而闷乱。外宜茵陈艾炬，内宜参术汤丸。若还止
痒证方安，抓破面皮凶断。
何为正面怕痒？面含五脏精华。假如破损实堪嗟，气散魄飞魂罢。尤忌先伤正额，心
经火带虚邪。几番试验不争差，寄语明人体察。
相火位居正额，出现胖壮休先。果然额上痘蝉联，大抵凶而不远。喜是两颐口鼻，始终都在其间，任教稠
密势缠绵，到底终无倾险。
起发成浆欲靥，忽然泄泻来攻。此时脾胃不宜空，变出百端可恸。多是内伤饮食，只求药有神功。若还消
肿泄淋浓，父母北邙泣送。
先用人参白术，黄 炙草煨姜。茯苓山药及木香，大剂煎来温养。不效次求豆蔻，木香芡实良方。三番只
有异功良，此实尽头酌量。
记取成浆欲靥，最防厌秽腥膻。大黄苍术共烧烟，可解一切秽厌。内服调元饮子，黄 炙草人参。当归苍
术酒芩连，犯着荆防即变。
到得成浆痘熟，根据时都要成痂。若还腐烂臭腥加，此是表虚堪讶。急进参 归术，荆防苍葛升麻。连翘牛
蒡密蒙花，休得弄真成假。
若是痘疮熟烂，皮破脓血淋漓。内服归术与参 ，牛蒡连翘官桂。外用多年败草，晒干研细成灰。铺开床
席任施为，最解火邪毒瓦斯。
痘熟不能收效，反行破烂成疮。一时焦痛甚难当，请问如何发放？但取甘草滑石，辰州豆粉清凉。蜜调涂
上便安康，此法不留书上。
有等痘疮纯正，缘何日久难收？请君仔细问根由，不可临时差谬。或者曾伤冷水，或因秘结热留。两般治
法各推求，不枉青囊异授。
果是曾伤冷水，湿伤脾胃皆虚。脾应肌肉主中区，无怪血脓流注。可用参 苍白，青陈甘桔无拘。丁香官
桂有方书，救里收表妙处。
如是大便秘结，三朝一七未通。此为热
气内蒸烘，因此毒难开纵。内服归黄麻子，大黄略入相攻。再行胆导妙无穷，管取成痂去壅。
收后许多余证，医家须要分明。毒留肝脏目生疔，翳障瞳人隐隐。毒入肺脾痈肿，责归手足阳明。内伤外
感要调停，免致多生怪病。
两目忽然肿痛，痘家毒入肝经。轻为浮翳掩瞳睛，重则终身废病。去翳菊花蝉蜕，蒙花蒺藜谷精。各为细末
共和匀，汤煮猪肝作引。
痈毒发于肢节，常常脓血不干，不知调理早求安，废疾终身为患。内服千金托里，外涂太乙金丹，排脓长
肉未为难，任是千金不换。
痘后不宜澡浴，痘疤皮嫩易伤。不知禁忌受寒凉，遍体热生痛僵。此因伤寒劳复，不宜官桂麻黄。只须九
味羌活汤，次以补中调养。
痘后或伤饮食，致令腹痛非轻。不宜转下损脾经，消导方为对证。白术人参枳实，黄芩大麦青陈。山楂白茯
与砂仁，积化痛疼俱定。
痘烂不齐收靥，正面灌痛流脓。急防两目毒来攻，解毒清凉好用。酒炒芩连栀柏，连翘蝉蜕木通。升麻牛蒡
苦参同，细研酒丸酒送。
大凡痘疮一证，名为百岁圣疮。如龙蜕骨换心肠，又似蝉蜕壳样。出现光壮收靥，落痂颜色相当。饮食寒暑
顺阴阳，自此精神长旺。

诊治痘疹要知道顺证与逆证，这与天时人事密切相关。大体上以阳火作为根基，如果遇到阴寒之气则不喜。春夏季节发病多为吉兆，秋冬季节发病则逆证无疑。如果痘疮稠密必然凶危，稀少才算是平易之症。
目前的治疗方法并不固定，清凉与温补界限分明。各家各执己见争论纷纷，自认为自己是神圣医家。解毒喜欢使用凉泻之法，补益中气喜爱使用辛温之品。如果不明白时令气候与人体元神元气，只是盲目捕风捉影，徒劳无功。
假使天时温暖炎热，使用辛温药物会助长灾殃，在严寒凝滞时使用凉解药物则是雪上加霜，病人如何抵挡？体质壮实者再行温补，虚弱者又使用寒凉药物。虚虚实实，损伤元阳，无异于隔靴搔痒，无济于事。
观察时行疫病，天时热气炎炎。精神肥健、饮食正常者，使用解毒清凉法很方便。如果是风寒太甚，身体虚弱羸瘦、呕吐腹泻连绵不断，此时适宜以温补法为先，关键在于医生的灵活变通。
诊治痘疹要知道轻重，精通吉凶顺逆。毒轻疮少属于顺证常规，汤药不宜妄用。疮密毒重属于逆证，皮肤寸寸化脓。这种形证大多凶险，必须仔细扶持护理，不可放纵。
轻症痘疮三四次发出，头面胸背分布稀疏。小便清利、大便干稠，饮食如常且充足。重症痘疮遍身齐出，状如麻子麦麸。咽喉疼痛、泄泻、胸闷悠悠，饮食不思令人厌恶。
多有先轻后重的情况，只因触犯冒受风寒。房事不避讳，接触秽腥食物，饮食偏于过冷或过热。闲杂人员携带秽浊厌气，各种禽兽徘徊附近。庸医医术浅薄误投汤丸，反而使痘疮发生变异。
重症如何转为轻症？常常保持衣被温暖。房室严密，减少人员走动，饮食随时增添。未曾误投汤药，不曾乱吃酸腥食物。这是通过人事努力来夺取天地的灵气，怎么可以归咎于命运！
要识别痘疮的死证，没有比五种死证更明确的。紫黑色、气喘口渴、闷乱不安、瘙痒塌陷、切牙寒战。灰白色、顶陷、腹胀，皮肤嫩薄易破成坑。泄泻、气促、出现幻觉见鬼神，声音嘶哑、头面足部冰冷。
既然认识了这五种死证，那么吉利的病症又是怎样的呢？疮头饱满形成脓窠，任凭推磨也不破裂。四周根盘红活，安眠静卧神态平和。光泽壮实，收靥顺利不拖延，保证不会留下疥疾等后遗症，无需服药即可痊愈。
黑陷干枯是肾气败绝，切牙寒战是肝气受伤。失声喘气是肺气摧残，泄泻腹胀是脾气虚弱。瘙痒塌陷、闷乱是心神死亡，狂言乱语、见鬼神是亡魂之象。皮肤嫩薄易破是阳气虚无，便血阴崩是阴血崩溃的模样。
痘疮首尾不可轻易发汗或攻下。发汗时腠理开张，风寒容易侵入透发斑疮，导致收靥不齐、火旺。误用攻下必犯脾胃，无事自取内伤。导致泄泻、黑陷而倾覆死亡，枉使灵魂飘荡。
大体上痘疮未发出之前，先用升麻葛根参苏饮。如果这样仍不出汗，急忙疏解，见到红点时就要停药。如果大便秘结，轻轻使用四顺清凉饮相符。假使自行腹泻，还要攻伐无辜正气，必定招致阎罗王掌簿勾魂。
调理痘疮不过两种方法，补益中气与解毒兼行。补中气用参、术、草、苓，枳实、山楂有效。解毒用芩、连、栀、柏，连翘、枳实、防风、荆芥。川芎、当归养血妙如神，加减详细预先定好。
气血要分虚实，只从疮色推断。紫肿红色由血实引起，在四物汤内加解毒药。灰白中陷由气弱引起，四君子汤是良策。略加解毒药相符，补气实脾无误。
凡是痘疮初出，如果遇到热盛昏沉清醒交替。先使用解毒发散药，不要等到临渴掘井。桔梗、升麻、干葛，连翘、甘草、黄芩。牛蒡、栀子、木通、荆芥，蝉蜕、防风作为引经药。
如果是正常的潮热，只需干葛、升麻。加入芩、连、甘草、赤芍，牛蒡、连翘价值连城。或者使用参苏饮，加入青皮、木香。煎服一剂胜过灵砂，痘疮透发表散才停止。
初热期多发生抽搐，急忙使用导赤散疏通。木通、甘草与防风，生地黄同用。再加辰砂调服，片刻间救护朦胧意识。此方确实有神功，保证施行必中。

From where does the toxin of smallpox and chickenpox originate? It stems from the fire-toxin transmitted within the mother's womb. After birth, impure substances are swallowed and hidden within the Mingmen (Gate of Life). Once exposed to seasonal epidemic qi, it induces the ministerial fire to simmer and roast. The toxin emerges from the bone marrow to the skin surface, transmitting from person to person.
Each of the five zang organs has its own morphological signs and symptoms; recognition must be clear. Alternating chills and fever with drowsiness indicate Splei meridian involvement; yawning and sudden chest oppression indicate Liver conditions. Coughing and sneezing indicate Lung pathogen invasion; facial redness and palpitations belong to the Heart. Only the Kidney remains pure and tranquil, avoiding evil invasion, manifesting as coldness in the hands, feet, ears, and sacrum.
Each of the five zang organs has a primary syndrome, making treatment methods difficult to unify. The Liver is associated with water flooding affecting the Lungs, and blistering leading to pus; the forms of major and minor sores vary. Spleen syndromes manifest as rashes, while Heart meridian syndromes present as red macules. Kidney syndromes presenting as black depression are mostly ominous; even with miraculous elixirs, what use are they?
One must understand the favorable and unfavorable courses of smallpox and chickenpox, which follow the timing of heaven and human affairs. Generally, yang-fire serves as the foundation; yin-cold is unwelcome. Eruptions in spring and summer are mostly auspicious, while those in autumn and winter are undoubtedly unfavorable. Dense eruptions inevitably signal danger, whereas sparse ones are considered mild.
Current treatment methods are not fixed; the distinction between cooling and warming tonification is clear. Practitioners argue各执己见, claiming themselves as divine healers. Detoxification favors cooling purgation, while tonifying the center loves using acrid-warm agents. Without understanding seasonal timing and the primordial spirit (Yuan Shen), one merely grasps at shadows, working in vain.
If the weather is warm and hot, using acrid-warm agents causes disaster; using cooling resolvents in severe cold adds frost to snow, how can the patient withstand it? Tonifying warmth for the robust and using cold-coolants for the deficient leads to "deficiency creating deficiency," damaging the primordial yang, akin to scratching an itch through a boot—ineffective.
Observe seasonal epidemics: when heavenly heat is blazing, if spirit is robust, appetite good, detoxification and cooling are convenient. If wind-cold is excessive, with emaciation, vomiting, and continuous diarrhea, warming and tonification should be prioritized, relying on the physician's flexibility.
One must know the severity of smallpox/chickenpox and master favorable/unfavorable outcomes. Light toxin with few sores follows the normal course; decoctions should not be used recklessly. Dense sores with heavy toxin are unfavorable, with skin turning to pus inch by inch. Such manifestations are mostly ominous; careful nursing is essential, do not neglect.
Mild cases erupt three or four times, sparse on the head, face, chest, and back. Urine is clear, stool is firm, diet is normal and sufficient. Severe cases erupt all over the body, resembling hemp seeds or wheat bran. Throat pain, diarrhea, and oppressive discomfort make loss of appetite loathsome.
Many cases start mild then become severe due to exposure to wind-cold. Sexual indiscretion, contact with foul/fishy substances, and dietary extremes of cold/heat. Strangers carrying foul odors, and various animals lingering nearby. Quack doctors with shallow skills misprescribe pills/decoctions, causing the sores to transform abnormally.
How to turn severe cases mild? Keep clothing and bedding warm. Keep the room private with few visitors, advance diet appropriately. Avoid misprescribed medicines and reckless consumption of sour/foul foods. This is human effort seizing heavenly vitality; how can it be attributed to fate?
To recognize death signs of smallpox/chickenpox, nothing is clearer than five signs. Purple-black color, panting/thirst/oppression, itching/collapse, teeth-chattering/chills. Gray-white color, depressed crown, abdominal distension, tender skin breaking into pits. Diarrhea, shortness of breath, seeing ghosts/spirits, hoarse voice, cold head/face/feet.
Having recognized these five death signs, what are the auspicious diseases? Sores are full and form pus nests, unbreakable even by grinding. Roots around the perimeter are red and vibrant; sleep is peaceful, demeanor is calm. Luster is robust, crusting proceeds without delay; guaranteed no sequelae like scabies, healing without medicine.
Black depression and dryness indicate Kidney failure; teeth-chattering and shivering indicate Liver injury. Loss of voice and panting indicate Lung damage; diarrhea and abdominal distension indicate Spleen deficiency. Itching/collapse and confusion indicate Heart death; delirium and seeing ghosts indicate soul departure. Tender skin easily broken indicates Yang emptiness; bloody stool and yin collapse indicate Yin blood depletion.
Sweating and purging are contraindicated at the beginning and end. Sweating opens pores, allowing wind-cold to penetrate eroded sores, leading to uneven crusting and excess fire. Mistaken purging harms the Spleen/Stomach, causing internal injury unnecessarily. Diarrhea and black depression lead to collapse and death, wasting the soul.
Generally, before eruption, use Shengma Gegen Rensu Decoction. If still no sweat, urgently resolve; stop medication once red spots appear. If constipation, gently use Sishun Qingliang Decoction. If spontaneous diarrhea, attacking the innocent qi will surely invite death.
Regulating smallpox/chickenpox involves two methods: tonifying the center and detoxifying simultaneously. Tonifying center uses Ginseng, Atractylodes, Licorice, Poria; Zhi Shi and Hawthorn are effective. Detoxification uses Scutellaria, Coptis, Gardenia, Phellodendron; Forsythia, Zhi Shi, Saposhnikovia, and Schizonepeta. Chuanxiong and Angelica nourish blood miraculously; adjustments are predetermined.
Differentiate Qi and Blood deficiency/excess by observing sore color. Purple/swollen/red indicates Blood excess; add detoxifiers to Siwu Tang. Gray-white/depressed indicates Qi weakness; Sijunzi Tang is the strategy. Add slight detoxifiers to match; tonifying Qi and strengthening Spleen is error-free.
When sores first emerge, if heat is excessive with coma/waking alternation, use detoxifying/dispersing herbs first, do not wait until thirsty to dig a well. Thereafter, sores erupt densely; distinguish deficiency/excess for treatment. Deficient patients with diarrhea and gray color: Shiquan Dabu Decoction is suitable. If swollen/red/blooming: Scutellaria, Coptis, Gardenia, Phellodendron, Chuanxiong, Angelica. Forsythia, Burdock, Cimicifuga, Kudzu, dried Licorice are wondrous; this is the true secret.
Gray-white failing to rise, plus frequent diarrhea. Warming middle herbs should not stop; urgently support crisis. Angelica, Scutellaria, Peony, Licorice, Ginger, Ginseng. Costus, Gardenia, Green Tangerine peel, Chenpi, Cinnamon twig, Clove are effective.
Severe toxin often causes throat pain, making eating difficult. Belamcanda and Licorice root are most appropriate; Forsythia, Cimicifuga, Burdock. If sores cluster on the neck, named "Locking Chest" (Suoge), tragic. Once ruptured, life ends; transforms into aphonia and water blisters.

Treat with dispersing wind/cooling blood: Schizonepeta, Saposhnikovia, Forsythia, Burdock, Chuanxiong, Ephedra. Rehmannia, Angelica, Kudzu efficacy is praiseworthy; Bamboo leaf, Akebia are priceless.
Generally, sores should be painful/swollen; void deficiency-itching. Due to dietary damp-toxin accumulation, Heart fire overcomes causing confusion. Exteriorly use Mugwort moxa; interiorly use Ginseng/Atractylodes decoctions/pills. If itching stops, condition is safe; scratching face skin is ominous.
Why fear itching on the face? Face contains essence of five zang. If damaged, truly lamentable; Qi scatters, Soul departs. Especially avoid injuring the forehead; Heart fire carries deficient evil. Tested repeatedly without error; advice to wise physicians to observe.
Ministerial Fire resides on the forehead; if puffy/robust appear first. If forehead sores connect continuously, mostly ominous soon. Favorable if cheeks/nose/mouth involved throughout; regardless of density/prolongation, ultimately no danger.
Rising to filling/crusting, suddenly attacked by diarrhea. Spleen/Stomach should not be empty here; myriad changes cause grief. Mostly internal injury from diet; seek divine drug efficacy. If swelling reduces but diarrhea thickens, parents weep at burial mound.
First use Renshen Baizhu San, Astragalus, honey-fried Licorice, baked Ginger. Poria, Yam, Costus; large dose decocted for warming/nourishing. If ineffective, next seek Cardamom, Costus, Euryale seed; good formula. Three iterations only Yigong San is good; this is the final measure.
Remember filling/crusting stage; most fear foul/fishy smells. Rhubarb and Atractylodes burned as smoke, resolving all foul odors. Interiorly use Tiaoyuan Yin: Astragalus, honey-fried Licorice, Ginseng. Angelica, Atractylodes, wine-fried Scutellaria/Coptis; encountering Schizonepeta/Saposhnikovia causes transformation.
When filling matures, according to time, all should crust. If rotting/stench increases, this is exterior deficiency, surprising. Urgently advance Ginseng, Astragalus, Angelica, Atractylodes, Schizonepeta, Saposhnikovia, Atractylodes, Kudzu, Cimicifuga. Forsythia, Burdock, Primula flower; do not confuse truth with falsehood.
If sores mature/rot, skin breaks, pus/blood drips. Interiorly use Angelica, Atractylodes, Ginseng, Astragalus, Burdock, Forsythia, Cinnamon twig. Exteriorly use old decayed grass, sun-dried, ground to ash. Spread on bed mat, apply freely; best resolves fire-toxin vapor.
Mature sores failing to heal, instead rotting into ulcers. Sudden burning pain hard to bear; ask how to treat? Take Licorice, Talcum, Chenzhou bean powder for cooling. Honey-mixed application brings health; this method is not recorded in books.
Some pure sores, why long duration in healing? Ask carefully for root cause, do not err temporarily. Perhaps injured by cold water, or constipation retaining heat. Two treatments sought separately, not wasting unique transmission.
If indeed injured by cold water, dampness injures Spleen/Stomach, both deficient. Spleen governs muscles/central region; no wonder blood/pus flows. Use Ginseng, Astragalus, White/Yellow Atractylodes, Green/Chen Tangerine peel, Licorice, Platycodon without restriction. Clove, Cinnamon twig have prescriptions; saving interior/exterior is wondrous.
If severe constipation, three days to seven days unpassed. This is internal heat steaming; thus toxin cannot open/release. Interiorly use Angelica, Rhubarb, Hemp seed; slight Rhubarb attack. Again use gallbladder suppository; wondrous effect, guaranteed crusting and removing obstruction.
Many residual symptoms after recovery; physicians must distinguish clearly. Toxin remaining in Liver causes eye carbuncles, nebula obscuring pupil. Toxin entering Lung/Spleen causes abscesses, responsibility lies in Hand/Foot Yangming meridians. Balance internal injury/external contraction to avoid strange diseases.

Sores rotting unevenly, crusting; face pours pain/pus. Urgently prevent toxin attacking eyes; detoxification/cooling useful. Wine-fried Scutellaria/Coptis/Gardenia/Phellodendron, Forsythia, Cicada slough, Akebia. Cimicifuga, Burdock, Sophora root together; finely ground, wine pills, wine delivery.
