This is a disease arising from the dysregulation of Qi movement in the five Zang organs. It is a pathology originating internally.
The Liver governs Wind. In excess conditions, symptoms include staring eyes, yawning, loud crying, neck stiffness, and sudden oppression. In deficiency conditions, symptoms include teeth grinding and yawning. When Qi is warm, the pathology generates internally; when Qi is hot, it generates externally. Here, "Qi" refers to the breath from the mouth.
For excess conditions, use Xie Qing Wan (Drain the Green Pill) or Danggui Longhui Wan (Angelica Sinensis and Gentian Root Pill) to drain the pathogen. For deficiency conditions, use Dihuang Wan (Rehmannia Pill) to tonify.
Xie Qing Wan: Treats acute convulsive spasms due to wind-stroke (Jing Feng), primarily addressing Liver Heat.
Ingredients: Qianghuo (Notopterygii Rhizoma et Radix), Fangfeng (Saposhnikoviae Radix), Danggui (Angelicae Sinensis Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), Shanzhiren (Gardeniae Semen), Longdancao (Gentianae Radix), Dahuang (Rhei Radix et Rhizoma) (soaked in wine and stir-fried on paper, equal parts). Grind into fine powder, form pills with honey the size of Euryale Seed (Qianshi). Take half to one pill per dose, decocted with Zhuye (Bamleaf Twig) and dissolved sugar for administration.
Danggui Longhui Wan: Treats Yin deficiency of the Kidney and Liver, with accumulation of Wind-Heat causing palpitations, convulsions, and restlessness.
Ingredients: Danggui (Angelicae Sinensis Radix, 1 liang), Longdancao (Gentianae Radix, 1 liang), Huanglian (Coptidis Rhizoma, 5 qian), Huangbai (Phellodendri Cortex, 1 liang), Dahuang (Rhei Radix et Rhizoma, 5 qian), Shanzhizi (Gardeniae Fructus, 1 liang), Lushui (Aloe, 5 qian), Qingdai (Indigo Naturalis, 5 qian), Muxiang (Aucklandiae Radix, 1 qian), Shexiang (Moschus, 5 fen), Huangqin (Scutellariae Radix, 1 liang). Grind into extremely fine powder, form pills with honey the size of Sesame Seed. Take 5 to 15 pills per dose, administered with Zhuye (Bamleaf Twig) decoction.
Dihuang Wan: Treats congenital insufficiency in infants, resulting in Kidney weakness leading to mutism, delayed closure of fontanelles (Jie Lu), inability to walk at an older age. Also treats Liver Gan (malnutrition syndrome) with white membrane obscuring the pupil, hematuria, aphonia, emaciation, sores, and scabies.
Ingredients: Shudihuang (Prepared Rehmanniae Radix, 8 liangs, dried and powdered), Shanzhuyu (Corni Fructus, dried, 4 liangs), Shanyao (Dioscoreae Rhizoma, 4 liangs), Bai Fuling (Poria, 3 liangs), Mudanpi (Moutan Cortex, 2 liangs), Zexie (Alisma Rhizoma, 2 liangs). Grind into fine powder, form pills with honey the size of Euryale Seed. Infants aged one year take one pill; children over two years increase to three pills, taken on an empty stomach with warm water.
(Various convulsions and spasms caused by Wind, accompanied by body traction and deviation, are diseases of the Liver. Xie Qing Wan should be used as the primary treatment.)
An infant of seven months old experienced intermittent convulsions, deep sleep without waking, no crying or nursing, insensitivity to pinching or pricking, no sneezing upon nasal irritation, and inability to accept medication. I stated: This is True Convulsion (Zhen Chu), a critical sign indicating incurability.
If concurrent Heart symptoms are present, fever accompanies convulsions. I stated: If the Liver has Wind, the eyes blink repeatedly but do not convulse; if the Heart has Heat, convulsions occur. If the Liver has Heat, the eyes stare straight but do not convulse; if the Heart has Heat, convulsions occur. Treatment requires draining the Liver with Xie Qing Wan and draining the Heart with Daochi San. (Formulas see below under Liver and Heart sections).
If concurrent Spleen symptoms are present, mild cases involve lethargy and loss of appetite. One must observe the stool. If constipated, apply Mi Dao Fa (Honey Suppository method); strictly avoid purgatives. Purging may cause Spleen deficiency, worsening the condition into a severe illness. If stools are moist/loose, Hu Po Bao Long Wan (Amber Dragon-Holding Pill) is indicated.
If concurrent Lung symptoms are present, rapid breathing, chest oppression, and phlegm obstruction occur. Pathogens must be cleared via urination and defecation. If wheezing is audible, shoulders rise, chest elevates, and rattling sounds are heard in the throat, it is incurable. Qing Ning San (Clearing Peace Powder) is indicated.
Qing Ning San: Treats Heat from fright originating in the Heart and Lung, requiring clearance through urine.
Ingredients: Sangbaipi (Mori Cortex, stir-fried with honey water), Chi Fuling (Red Poria), Cheqianzi (Plantaginis Semen), Tiantingli (Descurainiae Semen seu Lepidii Semen, stir-fried), Shanzhiren (Gardeniae Semen, equal parts), Zhigancao (Prepared Licorice Root, half amount). Grind into fine powder. Take half a qian per dose, decocted with Shengjiang (Fresh Ginger) and Dazao (Jujube). If Liver Heat causes difficulty in both urination and defecation, add Wei Dahuang (Stir-fried Rhubarb) to promote downward movement.
An infant suffering from phlegm obstruction developed convulsions and rapid breathing. I administered Mengshi Guntan Wan (Pyrite Rolling Phlegm Pill), crushed and mixed with Sangbaipi (Morus Bark) decoction. The wheezing ceased, phlegm descended, and convulsions stopped.
If concurrent Kidney symptoms are present, sudden aphonia and rigid limbs occur. This should be treated according to Wind patterns. Mild cases are treated with Dihuang Wan. Severe cases result in permanent disability and are incurable.
