(General Summary: Prolapse of the anus that persists for a long time is difficult to retract; if complicated by wind invasion, the condition is worrying. Moreover, since the Spleen and Stomach are already weak, careful differentiation of cold and heat patterns makes it easier to cure.)
The Lung and Large Intestine have an exterior-interior relationship. The anus is the "Gate of the Corporeal Soul" (Po Men), which is the excretory opening of the Large Intestine.
Chao Yuan-fang stated: Excess-heat leads to constipation, while deficiency-cold leads to anal prolapse. Some cases arise from prolonged dysentery, characterized by tenesmus (urgent need to defecate with heavy sensation in the rectum), where straining causes the anus to open and be exposed to external wind. Others result from latent summerheat causing diarrhea, leading to intestinal laxity and incontinence. Some individuals have congenital weakness and are prone to cold invasion, also causing deficiency-depletion of the Large Intestine. All diarrheal diseases in children are caused by summerheat-dampness and wind-heat taking advantage of Spleen-Stomach deficiency. Wind belongs to Wood; when Liver-Wood is excessive, it overacts on Earth. Earth governs the Spleen and Stomach; when deficient, it is controlled. Dampness tends to damage the Spleen; due to deficiency, dampness invades, impairing the ability to separate the clear from the turbid. Water and food mix together, resulting in profuse watery diarrhea (Dong Xie). After prolonged Dong Xie, the Large Intestine becomes deficient. The Large Intestine corresponds to Hand-Yangming Dry Metal. When Earth is deficient, it cannot generate Metal; once Metal Qi is deficient, the pathway of transportation and transformation also becomes weak. If additionally attacked by wind-cold, the anus may prolapse and fail to retract. The treatment method should tonify the Spleen and warm the Stomach. Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) should be used with doubled amounts of Mu Xiang (Aucklandiae Radix), Bai Shao (Paeoniae Radix Alba), Yu Ke (Pericarpium Papaveris), Sha Ren (Amomi Fructus), and Di Yu (Sanguisorbae Radix). Externally, Fu Long Gan (Caudex Dragoni) can be applied, and Bi Ma Gao (Ricinus Paste) can be pasted onto the anterior fontanelle to guide the qi upward, allowing spontaneous retraction. Once fully retracted, wash off the paste with water. If there is accumulation of damp-heat in the Large Intestine without prior purgation, this disease may also occur. Shao Yao Tang (Peony Decoction) with a small amount of Da Huang (Rhei Radix et Rhizoma) added can purge the accumulated qi. Once dysentery stops and the anus no longer prolapses, Pan Long San (Coiling Dragon Powder) can be applied externally. Generally, hot fingers and toes indicate heat patterns, while cold fingers and toes indicate deficiency patterns; this is another diagnostic criterion.
For chronic dysentery that does not cease, burn walnut shells to ash while preserving their medicinal properties, grind into powder, and administer with wine according to age. Alternatively, use Hawthorn (Shan Zha) powder mixed with white sugar for red hawthorns or brown sugar for white hawthorns, which is also effective.
Generally, treating dysentery: within one or two days, when vital energy (Yuan Qi) is not yet depleted, treatment should focus on unblocking accumulations, employing the method of "treating obstruction with unblocking agents" (Tong Yin Tong Yong). After three or four days, do not use unblocking methods, as vital energy may be depleted; instead, prioritize clearing heat and detoxifying, and regulating the Spleen and Stomach.
