The manifestations of Ji Jing (Acute Convulsion), also known as Zhen Chu (True Spasm), include tightness of the jaw, high fever with surging saliva, upward staring or strabismus, opisthotonos, convulsions and tremors, frequent blinking and traction of the lips, mouth, eyebrows, and eyes. The breath from the mouth is cold, the face is red, the lips are crimson, urine and stool are yellow-red, and the pulse is floating, rapid, full, and tight. This indicates internal excess heat combined with external wind pathogen invasion. Heat accumulates in the Heart causing fright, and wind arises in the Liver triggering spasms. The Liver wind and Heart fire contend between the two viscera, leading to chaotic blood flow and rebellious Qi, excessive phlegm-fluids, stagnation of all meridians, blockage of orifices, and flourishing wind qi that has no outlet for discharge, thus resulting in violent and fierce attacks. There are cases presenting with convulsions, opisthotonos, and strabismus, but without tightness of the jaw, and without phlegm-fluids in the mouth but with hot breath; these cannot be directly diagnosed as Jing Feng (Wind-Fright). It is likely Wind-Cold, Cold Damage, food retention, fright retention, or rash/pox conditions. This refers to Qian Yi's concept of "Fake Spasms," and treatment should be based on the specific syndrome. Furthermore, during acute convulsions, one must not forcibly grasp the child's limbs, but only support them; otherwise, wind-phlegm may rebel into the meridians, leading to contracture of the limbs and permanent disability. [Qian] stated: Ji Jing occurs after hearing a loud noise or experiencing great fright, followed by convulsions, returning to normal after the attack. This is a condition of Yin deficiency (Yang excess) and requires purgation, treated with Li Jing Wan (Drain Fright Pill). Ji Jing fundamentally stems from heat generated in the Heart, manifesting as body heat, red face, thirst, hot breath, and yellow-red urine and stool. Severe cases involve convulsions, as extreme heat generates wind, which belongs to the Liver. This is Yang excess and Yin deficiency, so Li Jing Wan is used to clear phlegm-heat. Do not use Croton seed (Ba Dou) or warm purgatives strongly, lest the post-convulsion deficient heat fail to resolve. When pediatric phlegm-heat lodges in the Heart and Stomach, hearing an extraordinary loud noise triggers fright and convulsions; if heat is extreme, spontaneous convulsions occur even without noise or fright. [Yan] stated: Ji Jing involves internal heat generating wind, possibly triggered by fright, with eyes rolling up, tidal phlegm, and convulsions. The body and breath are hot, but the child returns to clarity after the attack subsides or wakes up. When the convulsive trend diminishes, administer one or two doses of heart-calming and heat-clearing medicines such as She Xiang Wan (Musk Pill), Zhen Xin Wan (Heart-Calming Pill), Bao Long Wan (Embrace Dragon Pill), Chen Sha Wan (Cinnabar Pill), or Zi Xue (Purple Snow). After the fright stabilizes, use medicines like Li Jing Wan, Ruan Jin Dan (Soft Golden Pill), or Tao Zhi Wan (Peach Branch Pill) to purge phlegm-heat. Once phlegm-heat is purged and the Heart Spirit calms, recovery ensues. Jie Gu stated: Ji Jing is a Yang syndrome involving the Fu organs, with phlegm-heat lodging in the Heart and Lungs, indicating flourishing Shaoyang Ministerial Fire. As the Classic states: "Heat generates wind." Attacks triggered by loud noises relate to the East Zhen Trigram activating with Fire energy. Fire does not move spontaneously; flames move with wind. Treat with Li Jing Wan, Dao Chi San (Guide Out Red Powder), Xie Qing Wan (Drain Green Pill), or Di Huang Wan (Rehmannia Pill). After convulsions cease, take An Shen Wan (Spirit-Calming Pill). [Li Gao] warned against Fang Feng Wan (Saposhnikovia Pill) for treating fright, as Fang Feng is acrid and warm, potentially fatal. Acrid dispersants and warm agents belong to Fire, strengthening the Mother to overfill the Child, leading to death. If fright causes green diarrhea, first calm the Liver with Cinnabar, avoiding cold medicines. Strictly forbid Liang Jing Wan (Cool Fright Pill), as Flourishing Wind-Wood attacks the Spleen-Stomach; one must first strengthen Earth before draining the Child (Liver). [Zhu Danxi] stated: For Ji Jing Feng with fever, locked jaw, latent heat in palms/soles, and phlegm-cough/wheezing, use emesis. Severe cases use Gua Di San (Melon Peduncle Powder); mild cases use Sophora root and Adzuki bean powder mixed with sour vinegar juice. After stabilization, take Tong Sheng San (Universal Benefit Powder) honey pills. Occasionally use Sang Niu (Mulberry Insect), dried and powdered, mixed for calming wind. Also use Northern Mint Leaf and Halloysitum Rubrum each 1 liang, Qing Dai (Indigo Naturalis), Bai Jiang Can (Bombyx Batryticatus), and Zhu Sha (Cinnabar) each 1 qian, Quan Xie (Scorpion) 2 pieces, Zhu Ya Zao Jiao (Gleditsia Sinensis Fruit) and Huai Jiao (Sophora Angustifolia Fruit) each 5 fen. Powdered and administered with Deng Xin Cao Decoction (Juncus Effusus Decoction) mixed with milk. For opisthotonos and fixed gaze due to fright, use Nan Xing (Arisaema Cum Bile) and Ban Xia (Pinellia Rhizoma) with Ginger Juice and Bamboo Sap, plus moxa at Yintang. Initial fright uses Fang Feng Dao Chi San: Sheng Gan Di Huang (Rehmannia Root), Chuan Mu Tong (Akebia Caulis), Fang Feng (Saposhnikovia Radix), and Gan Cao (Licorice Root) in equal parts, decocted with 3 qian of Zhu Ye (Bamboo Leaf). Use Ning Shen Gao (Spirit-Calming Paste): Mai Men Dong (Ophiopogon Tuber) dehearted, Tian Zhu Huang (Bambusa Textilis Calamus), Fu Shen (Poria with Hostwood), and Zhu Sha each 1 liang, She Xiang (Musk) 1 qian. Pulverized finely, mixed with refined honey into small cakes. Dissolve one cake in Mint Decoction before sleep. Old physicians say pediatric convulsions are mostly heat syndromes; using Bai Fu Zi (Typhonii Rhizoma), Quan Xie, Jiang Can, or Chuan Wu (Aconitum Carmichaelii Radix Preparata) initially creates bad outcomes. Later pediatricians used Dao Chi San added with Di Huang and Fang Feng for three doses to drain Heart channel pathogenic heat, stopping convulsions, followed by Ning Shen Gao for miraculous effect. [Zeng] noted that discussions on Ji Jing were absent in ancient texts, referred to only as Yang Xian (Yang Epilepsy). It generally results from lack of care, exposure to drafts or heat, excessive spicy food by day, heavy bedding by night, causing stagnant evil heat accumulating in the Heart and transmitting to the Liver, compounded by fright from objects, falls, shouts, thunder, drums, or animal sounds. Before onset, sleep is restless, crying or laughing in sleep, grinding teeth, biting nipples, sweating on nose/forehead, shortness of breath with phlegm wheezing, sudden collapse, fixed upward gaze, tight jaw, locked mouth, and limb convulsions due to extreme heat, accompanied by red face and rapid pulse. Heart heat and Liver wind constitute Yang within Yang. Wind moves, Fire flares with wind. Liver stores Hun (Ethereal Soul), Heart stores Shen (Spirit); heat agitates them, causing fright. Heart governs Shen and is not easily touched; fright causes heat, extreme heat generates wind, forming convulsions named Ji Jing. Treatment: First use Wu Ling San (Five-Ingredient Poria Pill) added with Huang Qin (Scutellaria Root) and Gan Cao, decocted, or Bai Jie San (Hundred-Release Powder) to release the exterior. Then use Mu Tong San (Akebia Powder) to unblock Heart Qi, and San Jie San (Three-Release Powder) to soothe the Liver channel, calm the soul, and reduce heat, using Niu Bang Tang (Arctium Decoction) or Fang Feng Tang. After removal of fright-wind, mild cases use Ban Xia Wan (Pinellia Pill), severe cases use Shui Jing Dan (Crystal Pill) to remove phlegm and prevent dementia. Avoid strong cold medicines; removing heat may invite cold. "Excess brings harm, control follows." In emergencies with simultaneous fright and wind, use Wu Ling San added with Zhu Sha powder, mixed with Mint Decoction. Wu Ling San contains Ze Xie (Alisma Rhizome) to guide urination; Heart and Small Intestine are interior-exterior pairs. Unblocking the Small Intestine allows Heart Qi to flow, reducing fright. Gui Zhi (Cinnamon Twig) dries Wood, suppressing Liver Qi and stopping wind. Zhu Sha calms the soul. After clearing heat and cooling Heart/Liver, use gentle Tang San (Powder) for regulation. Hot agents are difficult to use; physicians must examine carefully. I lament that physicians often confuse the root and branch of convulsions, indiscriminately using metallic, mineral, camphor, musk, centipede, silkworm, snake, scorpion, and strong cold wind-searching agents, depleting True Qi and worsening the condition, often leading to death. They do not realize fright originates in the Heart, wind in the Liver, and convulsions begin with Qi. Chen Shi's theory on Qi accumulation causing convulsions is most logical. I treat this with Kuan Qi Yin (Qi-Widening Decoction), primarily using Zhi Ke (Aurantii Fructus) and Zhi Shi (Aurantii Fructus Immaturus). Peaceful Qi ensures health; stagnation causes disease. Children's frequent crying causes Qi accumulation; inability to ascend/descend or vent externally leads to convulsions over time. Good physicians examine the source and treat accordingly. Differentiate Yin/Yang and Deficiency/Excess. Yang Excess: Wu He Tang (Five-Harmony Decoction) and San Jie San. Yin Deficiency: Gu Zhen Tang (Solidify Truth Decoction) and Kuan Qi Yin. Sudden onset without differentiation: Wu Ling San, Kuan Qi Yin, and slightly Kuan Re Yin (Heat-Widening Drink) mixed, administered with Ginger Juice and boiling water. Treating convulsions values widening Qi; smooth Qi stops convulsions naturally. To prevent palpitations, Kidney Water must be peaceful. Dai Shi said: "Treating fright is不如 tonifying the Kidney." Heart is Fire, dry nature; Liver Wind makes it flare. Tonifying Kidney raises Water and lowers Fire, preventing evil heat invasion. Even with Liver Wind, no fright occurs. Administer Bu Shen Di Huang Wan (Kidney-Tonifying Rehmannia Pill) or Hu Po Bao Long Wan (Amber Embrace Dragon Pill) at Shen Hour (3-5 PM). Water is born in Shen; medicine assists Kidney Water to calm Heart Fire. [Lian Fang] treatment emphasizes order and weight: after opening orifices, stop wind and stabilize convulsions; once stable, purge phlegm-heat. If pathology is phlegm-heat without fright, only clear heat and transform phlegm; do not recklessly use fright medicines containing cold agents that might draw phlegm-heat into meridians. If pathology is heat, do not treat phlegm; release the exterior. If pathology is fright, do not treat wind; since fright comes from phlegm-heat, clear heat and transform phlegm to stop fright. If pathology is phlegm, do not immediately treat fright; urgently clear heat and transform phlegm. If pathology is wind, do not immediately treat convulsions; since wind arises from fright, benefit fright and transform phlegm to disperse wind. If convulsions exist, stop wind and scatter fright. If treating fright fails to resolve phlegm/heat, how can fright stop? Transforming phlegm without clearing heat fails to disperse wind; how can phlegm leave? Thus, "non-treatment is treatment." When Ji Jing initially transmits and wind-convulsions stabilize, purge phlegm-heat, then urgently harmonize Stomach and calm Heart. If phlegm-heat is minimal after stabilization, use light medicines to eliminate phlegm and heat. Although Ji Jing requires purgation, do not overuse cold agents or mercury, calomel, Croton, Mirabilite too rapidly. These are不得已 (last resorts). Stop when disease is gone. Misuse leads to Man Jing (Chronic Convulsion). Purging phlegm-heat need not be rapid;斟酌 (moderate) use of Da Huang (Rhubarb Root) suffices. The source of Ji Jing lies in dispelling Liver wind and descending Heart fire, a crucial point in *You You Shu*. [Xue Ji] described Ji Jing symptoms: tight jaw, high fever, surging saliva, staring, opisthotonos, convulsions, hot breath, red cheeks/lips, floating-full-rapid pulse. This is Liver Blood deficiency with Fire stirring Wind. Wind scatters Yin Blood, intensifying Yin Fire; Fire damages Lung Metal, allowing Liver Wood to flourish. Nourish Liver Blood, nourish Spleen Qi. If repeatedly using wind-dispelling/phlegm-transforming/fire-draining/acrid-dispersing agents, recognize Spleen deficiency and Blood damage, urgently tonify Spleen Earth. If Wind-Fire contend with convulsions, oblique eyes, stiff sinews, and abundant phlegm, use Si Wu Tang (Four Substances Decoction) to generate Liver Blood, add Gou Teng Uncariae Ramulus cum Uncis and Shan Zhi Ren (Gardeniae Semen) to clear Liver Fire, Si Jun Zi Tang (Four Gentlemen Decoction) to tonify Spleen, and Liu Wei Wan (Six-Ingredient Pill) to nourish Kidney. If Lung Metal overacts on Wood with yawning, use Xie Bai San (Drain White Powder) to drain Lung evil, Di Huang Wan to benefit Liver Blood. If evil enters Liver, use Chai Hu Qing Gan San (Bupleurum Liver-Clearing Powder) added with Long Dan Cao (Gentiana Root). If evil enters Heart, use Zhi Zi Qing Gan San (Gardenia Liver-Clearing Powder) added with fried Huang Lian (Coptis Rhizoma). If evil enters Kidney, use Liu Wei Di Huang Wan. If evil enters Lung, use Di Gu Pi San (Lycium Bark Powder). If evil enters Spleen, use Liu Jun Zi Tang added with Chai Hu (Bupleuri Radix) and Shan Zhi. If not nourishing Liver Blood or Spleen Qi, persistently using wind/phlegm medicines increases Spleen deficiency and Blood damage, prolonging evil, inevitably transmitting to Man Jing. Initially with phlegm-heat but no fright, first release exterior, dispel Liver wind, descend Heart fire.
