The *Baojian* (Comprehensive Mirror) prescribes moxibustion on Yougong (Infant Palace) for three moxa cones, and additionally moxibustion one fen behind the middle fingernail. The *Wanquan Fang* (Ten Thousand Prescriptions) treats infantile nocturnal crying characterized by crying after lights are lit until dawn; it prescribes moxibustion on Zhongchong (Zhongchong, PC9), located one fen behind the middle fingernail, with one moxa cone sized like a wheat grain.
[Xue Ji] The classics state: "The tongue is the mechanism of voice, and the throat is the gate of voice." Sudden aphonia in infants is caused by cold pathogenic factors invading the epiglottis, causing the epiglottis to fail to open and close properly, leading to uncontrolled closure of the glottis, thus resulting in aphonia. If the voice in the throat remains normal but the tongue cannot move to articulate speech, this is termed "tongue aphonia" (She Yin). This is caused by wind-cold pathogens invading the collaterals of the Spleen or affecting the Lianquan (Lianquan, REN23) acupoint under the tongue. Since the tongue is the sprout of the Heart, and the Heart produces speech, wind pathogens blocking the meridians cause the tongue to be immobile. If the root of the tongue cannot move to articulate speech, accompanied by hoarseness in the throat, this is termed "throat aphonia" (Hou Yin). This is also caused by wind-cold invasion obstructing the airway, preventing sound emission and resulting in a voiceless throat. However, aphonia can also result from wind-phlegm obstruction, Qi deficiency of the Heart Meridian, wind invading the Spleen's collaterals, wind-phlegm stagnating in the Spleen's collaterals, Spleen Qi deficiency, or failure of clear Yang to ascend from the Stomach. Generally, this condition may also stem from congenital insufficiency of Kidney Qi inherited from the father, causing inability to speak; or from the mother's emotional fire (Five Zang passions) passing to the infant, steaming and closing the clear pathways; or from the infant's illness depleting body fluids, drying up the epiglottis; or from insufficient Kidney Qi with deficient fire flaming upward damaging the Lung; or from convulsive wind or stroke. For cases with residual heat and fluid depletion, use Qiwu Baizhu San (Seven-Ingredient Atractylodes Macrocephala Powder). For cases with failure of clear Yang to ascend, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). For cases of congenital Kidney deficiency and deficient fire damaging the Lung, use Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill). If symptoms include coughing with the head tilted back, emaciation of limbs, prominent whites of the eyes (leukocoria), or combined with open fontanelles (Jie Lu), yawning, and teeth grinding, all belong to Kidney deficiency. Without Dihuang Wan (Rehmannia Pill), recovery is impossible.
