逐恶露为要,月外各归经络,气血两分既定而犯此症,是气血虚极。其左手脉不足,补血倍于补气;

Cicada Slough (Chantui), Snake Slough (Shetui), and Tortoise Shell (Guijia) (each 3 qian). Dry them on a new tile, grind into powder, and take with good wine; the fetus will be delivered immediately.
For a fetus dead in the uterus, roast bull dung into powder, mix with half a cup of vinegar, wrap in green cloth, and apply heat to the umbilicus; the fetus will be expelled immediately.
Maternal syncope due to blood loss occurs in both full-term delivery (Dachan) and miscarriage (Xiaochan). The treatment primarily focuses on greatly tonifying Qi and Blood. However, the etiologies differ: miscarriage is often caused by deficiency of both Qi and Blood, whereas full-term delivery syncope results from residual blood (Baixue, stasis) flowing into the Liver Meridian, causing visual darkness and vertigo. Treatment should prioritize breaking stasis (Huoxue Huayu). First, place a basin of fierce fire near the patient's nose and mouth, pour a cup of good vinegar into the fire to generate vapor for inhalation, which stabilizes the Spirit and restores consciousness. Subsequently, treat according to specific symptoms. For syncope due to excessive bleeding, use Ginseng Decoction (Renshen Tang); for syncope due to excessive mental or physical exertion, use Fangfeng Tongsheng San combined with Siwu Tang.
Postpartum syncope is caused by residual blood rushing to the Liver.
Hawthorn Fruit (Shanzha) (3 qian), Safflower (Honghua) (2 qian), Peach Kernel (Taoren), Official Cinnamon Bark (Guangui), Cyperus Rhizome (Xiangfu), Galangal Root (Liangjiang), Curcuma Zedoaria Rhizome (Ezhu) (each 1 qian).
Headache, fever, dry mouth, abdominal fullness, and sweating after childbirth are caused by excessive food intake. Treatment prioritizes resolving food stagnation and dispelling wind, curable within a few days:
Hawthorn Fruit (Shanzha) (3 qian), Curcuma Zedoaria Rhizome (Ezhu), Gastrodia Tuber (Tianma), Platycodon Root (Jiegeng), Tangerine Peel (Chenpi), Malt (Maiya), Safflower (Honghua) (each 1 qian), Charred Dried Ginger (Hei Ganjiang) (8 fen).
Alternating chills and fever after childbirth occur because blood entering the Heart and Lungs causes heat, while blood entering the Spleen and Stomach causes cold, leading to alternating sensations. Treating this as malaria is incorrect.
Burreed Tuber (Sanleng), Curcuma Zedoaria Rhizome (Ezhu), Safflower (Honghua), Peach Kernel (Taoren) (each 2 qian), Official Cinnamon Bark (Guangui), Charred Dried Ginger (Hei Ganjiang) (each 1 qian), Hawthorn Fruit (Shanzha) (3 qian).
Edema of the four limbs after childbirth is caused by residual blood flooding the five Zang organs and transmitting to the limbs, failing to be transformed, or by dietary injury to the Spleen. Treating it as edema is incorrect. Use Wuling San plus Burreed Tuber (Sanleng) and Curcuma Zedoaria Rhizome (Ezhu) each 2 qian, Hawthorn Fruit (Shanzha) 3 qian; if within one month postpartum, add Peach Kernel (Taoren), Safflower (Honghua), and Charred Ginger (Heijiang), without excessive dosage.
Postpartum mania, seeing spirits, and incoherent speech are caused by residual blood and hot blood rushing to the Heart, resembling external pathogen invasion; do not treat as Wind Pathogen.
Hawthorn Fruit (Shanzha), Official Cinnamon Bark (Guangui) (each 3 qian), Costus Root (Muxiang), Cyperus Rhizome (Xiangfu), Charred Ginger (Heijiang) (each 1 qian), Safflower (Honghua), Peach Kernel (Taoren) (each 2 qian).
Persistent diarrhea after childbirth is often due to accidentally consuming raw, cold, or hard foods, which clash with foul blood, flow into the Large Intestine, and fail to digest.
Burreed Tuber (Sanleng), Curcuma Zedoaria Rhizome (Ezhu), Safflower (Honghua), Peach Kernel (Taoren), Charred Dried Ginger (Heiganjiang), Hawthorn Fruit (Shanzha), added to Wuling San.
Postpartum diarrhea varies. Some cases result from Qi and Blood deficiency after difficult labor, leading to chronic cold from prolonged diarrhea; others result from Cold Pathogen invasion, leading to chronic deficiency. The treatment methods are identical.
Wuling San plus Burreed Tuber (Sanleng), Curcuma Zedoaria Rhizome (Ezhu), Hawthorn Fruit (Shanzha).
Some cases result from stagnant blood flowing into the Large Intestine.
Charred Ginger (Heijiang), Official Cinnamon Bark (Guangui), Hawthorn Fruit (Shanzha), Safflower (Honghua), Peach Kernel (Taoren).
For the above two conditions, if diarrhea is prolonged and belongs to deficiency-cold, treat similarly. Physicians must be meticulous; an error may result in fatality. Postpartum diarrhea with undigested food and incontinence indicates extreme deficiency-cold. Regardless of duration, the treatment is the same.
Ginseng (5 fen), Atractylodes Macrocephala Rhizome (Baizhu), Poria (Fuling), Cinnamon Bark (Rougui), Charred Ginger (Heijiang) (each 2 qian), Polyporus (Zuling), Alisma Rhizome (Zexie) (each 1 qian).
Postpartum diarrhea with abdominal pain and urgent, obstructive defecation is also due to stagnant blood entering the Large Intestine. If chronic, treat as deficiency-cold.
Charred Ginger (Heijiang), Official Cinnamon Bark (Guangui), Safflower (Honghua) (each 2 qian), Burreed Tuber (Sanleng), Curcuma Zedoaria Rhizome (Ezhu) (each 1 qian), Peach Kernel (Taoren).
General joint pain after childbirth is due to open bone joints allowing Wind-Cold invasion, causing residual blood to stagnate in the meridians.
Safflower (Honghua), Peach Kernel (Taoren) (each 2 qian), Angelica Sinensis Root Tail (Guiwei), Charred Dried Ginger (Heiganjiang), Official Cinnamon Bark (Guangui), Lindera Root (Wuyao), Gentian Root (Qinjiao) (each 1 qian).
Hematuria with chicken-liver-colored urine after childbirth is due to improper postpartum care and anger injuring the Liver and Heart meridians, causing blood to flow into the Small Intestine. If it flows into the Large Intestine, obstruction occurs.
Safflower (Honghua), Peach Kernel (Taoren), Moutan Cortex (Danpi) (each 2 qian), Costus Root (Muxiang), Platycodon Root (Jiegeng), Akebia Stem (Mutong), Cyperus Rhizome (Xiangfu) (each 1 qian).
Postpartum chest distension, Qi stagnation, and irregular vomiting are due to excessive food accumulation in the Stomach, knotting with Phlegm. Symptoms include panting, limb temperature fluctuations, dry mouth, chest oppression, and restless sleep. For recent onset:
Hawthorn Fruit (Shanzha) (3 qian), Lindera Root (Wuyao) (2 qian), Amomum Fruit (Sharen), Malt (Maiya), Fritillaria Bulb (Chuanbeimu), Tangerine Peel (Chenpi), Cyperus Rhizome (Xiangfu), Charred Dried Ginger (Heiganjiang) (each 1 qian).
For distant/chronic onset:
Burreed Tuber (Sanleng), Curcuma Zedoaria Rhizome (Ezhu), Peucedanum Root (Qianhu), Areca Seed (Binglang), Bupleurum Root (Chaihu) (each 1 qian), Amomum Fruit (Sharen) (2 qian).
Cicada-like sounds in the throat after childbirth are caused by anger, residual blood rushing to the Heart, transferring to the Lung Meridian, where residual blood and Qi knot into a mass in the throat. This condition is rarely curable (one in ten survives).
Cinnamon Bark (Rougui), Safflower (Honghua), Peach Kernel (Taoren), Galangal Root (Liangjiang) (each 2 qian), Akebia Stem (Mutong), Lindera Root (Wuyao), Tangerine Peel (Chenpi) (each 1 qian).
Postpartum diarrhea with chest fullness and irregularity is due to food stagnation.
Burreed Tuber (Sanleng), Curcuma Zedoaria Rhizome (Ezhu) (1 qian), Hawthorn Fruit (Shanzha), Cardamom (Kesharen), Charred Dried Ginger (Heiganjiang), Official Cinnamon Bark (Guangui) (each 2 qian).
If chronic, treat as deficiency-cold. Presence of fecal residue is treatable; "leaking roof black water" stool indicates certain death.
Diarrhea is the most feared complication postpartum; caution is essential.
Yellow complexion, dry tongue, epistaxis, and generalized macules after childbirth indicate residual blood entering the Zang-Fu organs and skin, with heat knotting and stagnation. This is fatal (none survive).
Safflower (Honghua), Peach Kernel (Taoren), Official Cinnamon Bark (Guangui), Moutan Cortex (Danpi) (each 2 qian), Curcuma Zedoaria Rhizome (Ezhu), Akebia Stem (Mutong), Notopterygium Rhizome (Qianghuo), Saposhnikovia Root (Fangfeng), Schizonepeta Spike (Jingjie) (each 1 qian).
Postpartum fever, headache, and inability to extend/flex limbs are due to careless exposure to Wind Pathogen during delivery. Do not treat as Stroke; avoid dispersing exterior agents.
Gastrodia Tuber (Tianma), Gentian Root (Qinjiao), Fritillaria Bulb (Chuanbei), Saposhnikovia Root (Fangfeng), Dried Ginger (Ganjiang), Official Cinnamon Bark (Guangui), Perilla Stem (Sugeng).
Postpartum fever, headache, and unbearable stabbing abdominal pain are due to Cold Pathogen invasion causing residual blood stagnation and knotting. Treat regardless of duration.
Hawthorn Fruit (Shanzha) (3 qian), Dried Ginger (Ganjiang), Cinnamon Bark (Rougui), Lindera Root (Wuyao), Evodia Fruit (Wuyu), Cyperus Rhizome (Xiangfu), Corydalis Rhizome (Yanyan/Huyan), Safflower (Honghua), Peach Kernel (Taoren) (each 2 qian), Perilla Stem (Sugeng) (1 qian).
Postpartum mutism and running wildly.
Acorus Rhizome (Shichangpu), Ophiopogon Tuber (Maidong), Saposhnikovia Root (Fangfeng), Gastrodia Tuber (Tianma), Ligusticum Rhizome (Chuanxiong), Fritillaria Bulb (Chuanbei), Cinnabar (Chensha), Cyperus Rhizome (Xiangfu).
One month postpartum, fever/chills, anorexia, and abdominal pain are due to Qi and Blood deficiency.
Angelica Sinensis Root (Danggui), Peony Root (Shaoyao), Ligusticum Rhizome (Chuanxiong), Ginseng (Renshen), Atractylodes Macrocephala Rhizome (Baizhu), Poria (Fuling), Cyperus Rhizome (Xiangfu).
One month postpartum, panting, phlegm reversal, and sallow complexion are due to dietary injury to the Stomach and phlegm accumulation.
Burreed Tuber (Sanleng), Curcuma Zedoaria Rhizome (Ezhu), Magnolia Bark (N Houpo), Official Cinnamon Bark (Guangui), Charred Dried Ginger (Heiganjiang), Fritillaria Bulb (Chuanbei), Tangerine Peel (Chenpi), Platycodon Root (Jiegeng), Cyperus Rhizome (Xiangfu).
One month postpartum, uneven chills and fever are due to Qi and Blood deficiency.
Licorice Root (Gancao), Peony Root (Shaoyao), Bupleurum Root (Chaihu), Atractylodes Macrocephala Rhizome (Baizhu), Angelica Sinensis Root (Danggui), Poria (Fuling), Tangerine Peel (Chenpi), Ligusticum Rhizome (Chuanxiong), Cyperus Rhizome (Xiangfu).
One month postpartum, persistent spontaneous sweating is due to Qi and Blood deficiency combined with Wind Pathogen.
Atractylodes Macrocephala Rhizome (Baizhu), Poria (Fuling) (each 2 qian), Astragalus Root (Huangqi), Cyperus Rhizome (Xiangfu), Ophiopogon Tuber (Maidong), Saposhnikovia Root (Fangfeng) (each 1 qian), Official Cinnamon Bark (Guangui) (5 fen).
One month postpartum, aphonia, persistent vomiting, and inability to ingest food.
Tangerine Peel (Chenpi), Atractylodes Macrocephala Rhizome (Baizhu), Poria (Fuling), Cyperus Rhizome (Xiangfu), Dried Ginger (Ganjiang), Peucedanum Root (Qianhu), Astragalus Root (Huangqi).
Most postpartum syndromes are caused by Wind-Cold invasion and residual blood pathology. Dried Ginger (Ganjiang), Safflower (Honghua), Peach Kernel (Taoren), Lindera Root (Wuyao), Cinnamon Bark (Rougui), Hawthorn Fruit (Shanzha), and Cyperus Rhizome (Xiangfu) are indispensable.
The key to treating postpartum conditions lies in Dried Ginger. Use heavy dosage for high fever, light for mild fever (1 qian light, 3 qian heavy). It must be stir-fried with wine until black; uncharred ginger is contraindicated as it may generate fire.
One month postpartum, when Qi and Blood have settled and returned to the meridians, even if symptoms persist, greatly tonify Qi and Blood and invigorate Blood. Although many texts list Peony Root (Shaoyao) as strictly contraindicated postpartum, it is essential for resolving conditions occurring after one month. This is noted to counter skepticism.
Facial deviation, opisthotonos, and limb stiffness within one month postpartum are caused by Wind Pathogen invasion during delivery. For those with fever/headache:
Gastrodia Tuber (Tianma), Fritillaria Bulb (Beimu), Gentian Root (Qinjiao), Angelica Sinensis Root Body (Guishen), Official Cinnamon Bark (Guangui), Perilla Stem (Sugeng), Dried Ginger (Ganjiang).
Invasion after one month should not be treated as Stroke. It is due to severe Qi and Blood deficiency; tonify Qi and Blood, then resolve Phlegm. For postpartum fever/chills, expelling lochia (Elu) is priority in the first month; after one month, when Qi and Blood settle in meridians, symptoms indicate extreme deficiency. If Left Pulse is deficient, double Blood tonification over Qi tonification; if Right Pulse is deficient, double Qi tonification over Blood tonification. Do not use exterior-dispersing agents. If caused by irregular diet, in the first month add Hawthorn (Shanzha), Malt (Maiya), Amomum (Sharen), Fritillaria (Beimu), Tangerine Peel (Chenpi) to lochia-expelling formulas; after one month, regulate Qi and tonify Blood, adding Hawthorn (Shanzha) and Malt (Maiya) for digestion and Phlegm resolution. Exterior-dispersing agents are strictly contraindicated.
Facial deviation, limb numbness, and hemiplegia after childbirth are due to carelessness, drinking raw water, or Wind Pathogen invasion. Onset months later should not be mistaken for Wind Pathogen or Ghost Arrow syndromes; avoid acupuncture and exterior-dispersing agents. Formula:
Safflower (Honghua), Trichosanthes Root (Huafen) (each 2 qian), Saposhnikovia Root (Fangfeng), Schizonepeta Spike (Jingjie), Notopterygium Rhizome (Duohuo), Angelica Sinensis Root (Danggui), Gentian Root (Qinjiao), Notopterygium Rhizome (Qianghuo), Achyranthes Root (Niuxi), Atractylodes Macrocephala Rhizome (Baizhu), Codonopsis Root (Shashen), Poria (Fuling), Ligusticum Rhizome (Chuanxiong) (each 2 qian), Costus Root (Muxiang) (8 fen).
