为痰。又以柔药滋其阴，酸寒收涩，痰湿之气，尽行郁遏。以致痰带腥秽，色尽黄稠，黄为土色，是湿痰也
今内热咳嗽，痰仍腥秽。脉数濡弦，左部虚弦，舌苔薄白而滑。此气阴两亏，而湿热逗留之象，从实变虚，
从假变真，殊难措手。前人谓因虚致病者，补其虚而病自除，因病致虚者，去其病而阴自复。八年之病，
虽有成例可遵，恐鞭长之莫及耳。拟导其湿热下行，而不涉戕伐，俾得熏蒸之焰息，即所以保其阴气之消耗也。

陈氏（左侧脉象）：肝郁气滞，病症从左胁疼痛开始，加上火灸导致络热动血，多次进服阴柔药物，阴分固然赖以逐渐恢复，然而湿热由此而生，发为浊症。湿热逗留，风邪外触，于是导致咳嗽。先前用燥药伤了气，导致气虚不能鼓舞运行，饮食全部化为痰。后又用柔药滋养其阴，酸寒收涩，使痰湿之气完全郁遏。以致痰带腥秽，颜色全黄稠，黄色属土色，这是湿痰。如今内热咳嗽，痰仍腥秽。脉数濡弦，左部虚弦，舌苔薄白而滑。这是气阴两亏，而湿热逗留的现象，从实证变为虚证，从假象变为真象，很难着手治疗。前人说因虚致病者，补其虚而病自除；因病致虚者，去其病而阴自复。八年的病，虽然有成例可遵循，恐怕力所不及。拟引导其湿热下行，而不伤害正气，使得熏蒸的火焰熄灭，这就是保护其阴气不被消耗的方法。

Clearing and resolving has not worked; tentatively attempt nourishing water (Yin) to control sunlight (deficient fire), hoping for nature's help.

Certain Patient: Cough lighter by day, heavier at night, sputum yellow, thick and dense, stool hard with black color. Pulse rapid, tongue peeled. This is nothing but Yin deficiency with fire flaming up, damaging Metal (Lung) and Water (Kidney). However, since the sputum is thick and yellow, there must be damp-heat lingering in the Stomach. From clearing and nourishing, also guide damp-heat downward.

Patient Chen (Left pulse): Liver Qi stagnation, disease started from pain in the left hypochondrium. Added moxibustion caused collateral heat and bleeding. Repeatedly taking yin-nourishing medicines allowed Yin to gradually recover, yet damp-heat arose from this, manifesting as turbid symptoms. Damp-heat lingers, external wind touches, leading to cough. Previously, drying medicines injured Qi, causing Qi deficiency unable to promote circulation, so all food transformed into phlegm. Then soft/yin-nourishing medicines were used; sour-cold astringents completely suppressed the Qi of phlegm-dampness. Thus sputum became fishy and foul, entirely yellow and thick. Yellow belongs to Earth, indicating Phlegm-Dampness. Now internal heat causes cough, sputum remains fishy and foul. Pulse rapid, soggy, and wiry; left position deficient and wiry; tongue coating thin white and slippery. This is dual deficiency of Qi and Yin, with lingering damp-heat, changing from excess to deficiency, from false to true, very difficult to handle. Predecessors said: if disease arises from deficiency, tonify deficiency and disease removes itself; if deficiency arises from disease, remove disease and Yin recovers itself. Eight years of illness, although precedents exist, fear strength is insufficient. Plan to guide damp-heat downward without damaging healthy Qi, extinguishing the steaming flame, thereby preserving Yin consumption.
