[Case of Gu Shiquan] The lungs were affected by wind pathogenic factors, which lingered for a long time without resolving. In the middle of the previous month, staying up late for examinations allowed cold qi to attack again. The defensive qi (Wei Qi) was blocked by the pathogen, causing the yang qi to be constrained and uncomfortable, resulting in fever. Heat steams phlegm-dampness, disrupting the circulation of nutritive and defensive qi, leading to alternating chills and fever resembling malaria. With phlegm-dampness obstructing, turbid qi cannot descend, and clear fluids cannot ascend, depleting fluid sources, causing a peeled tongue and thirst. Phlegm-dampness steaming causes profuse sweating. The Stomach is the commander of the twelve meridians, governing the binding of sinews and bones and facilitating joint movement; thus, the *Neijing* states that treating flaccidity syndromes should focus solely on the Yangming. Now, with damp-phlegm accumulation and obstruction, the Yangming channel fails to nourish sinews and bones, causing sudden flaccidity and stiffness in both legs. These are the conditions before the asthma attacks. Currently, coughing has stopped, but severe asthma prevents lying down, with cold sweat pouring out and cold limbs. This indicates Lung qi only exhales but cannot grasp, presenting a critical deficiency. However, the deficiency is caused by asthma, so what causes the asthma? The Lung governs the descent of qi to the right; the Stomach resides below the Lung. The area between the Lung and Stomach has long been occupied by phlegm-dampness. Once the pathway for the Lung's right descent is blocked, violent asthma ensues, yet the sputum expelled is less than usual. Belching relieves the asthma slightly, indicating Stomach excess. Danxi said, "Excess qi becomes fire." Ascending qi-fire transforms turbid pathogens into dryness, causing white decay in the mouth. The pulse lacks spirit, showing signs of collapse. Regarding treatment, Li Shicai stated: If disease arises from deficiency, treat the deficiency to resolve the disease; if deficiency arises from disease, treat the disease to preserve the deficiency. These fleeting thoughts are offered for discussion.
Prescription: Chuan Guizhi (Cinnamon Twig) 5 fen, Dan Ganjiang (Dried Ginger) 5 fen, Wei Shigao (Calcined Gypsum) 7 qian, Guang Xingren (Apricot Seed) 4 qian, Sheng Yiren (Coix Seed) 5 qian, Dongguazi (Winter Melon Seed) 5 qian, Zhiqiao (Bitter Orange Fruit) 1 qian, Qing Luguan (Reed Rhizome) 1 liang.
[Right Kidney Case] Kidney deficiency fails to store and collect. Every winter, when storage is required, phlegm-asthma erupts. Last winter was extremely warm, failing to store; spring earth qi then vented, losing root qi control, causing wheezing unable to sit or lie. Heixi Dan attracts kidney yang, which is appropriate, but chronic kidney yin deficiency cannot handle warming tonification, causing floating deficient yang, manifesting as head sweating, cracked lips, and malar flush. Six pulses are fine and choppy, coating pale yellow, heart of tongue hairy and rough. Qi fails to grasp, risking sweat collapse. Prescribe tonifying kidney yin to grasp kidney qi. Efficacy is uncertain.
Prescription: Sheng Shu Di Tan (Charred Rehmannia Root) 3 qian each, Niuxi (Achyranthes Root) 4 qian, Yun Fuling (Poria) 3 qian, Danpi (Moutan Bark) 2 qian, Cishi (Magnetite) 5 qian, Zi Kou Hage (Clam Shell) 5 qian, Da Maidong (Ophiopogon Tuber) 3 qian, Huai Shanyao (Chinese Yam) 3 qian, Kanqi (Umbilical Cord) 1 strip, Qiushi (Human Urine Salt) 2 qian, Wuweizi (Schisandra Berry) 3 fen.
[Left Kidney Case] Kidney is originally empty, closure not secure. Winter qi does not gather, dryness attacks externally, causing dry cough without sputum. Last winter, yang qi rose, turning cough into asthma, only upon exertion, resolving quickly. At Spring Equinox, yang vents excessively, kidney qi cannot store, asthma worsens significantly. Deafness and distension, throat obstruction, difficult urination/defecation, thirst and dry throat, emaciation. Occasional gray-black sputum. Pulse fine lacking spirit, tongue red with white dry hairy coating. Chong vessel carries dragon-fire upward, blocking Lung qi descending to Kidney, and Kidney qi failing to inhale Lung qi downward—this is kidney deficiency. Fear of yang leakage and sweating. Tentatively prescribe communicating Lung and Kidney, using pills to descend, avoiding cloying herbs.
Prescription: Cishi (Magnetite) 5 qian, Dan Qiushi (Human Urine Salt) 2 qian, Tian Maidong & Mai Dong (Ophiopogon Tubers) 2 qian each, Zi Hage (Clam Shell) 7 qian, Fuling (Poria) 3 qian, Huai Niuxi (Achyranthes Root) 3 qian, Cheqianzi (Plantago Seed) 3 qian, Fen Danpi (Moutan Bark) 3 qian, Fei Zhimu (Anemarrhena Rhizome) 1.5 qian, Duqi Wan (Duqi Pill) 5 qian taken twice.
Second Visit: Communicating Lung and Kidney improved hearing and urination, asthma paused. Still thirsty/dry throat, immobile, worse movement. Pulse fine-rapid lacking spirit, Right Chi particularly fine-choppy. Root emptiness, failure to grasp evident. Previous prescription expanded.
Prescription: Shu Di Tan (Charred Rehmannia) 4 qian, Sheng Baishao (White Peony Root) 1.5 qian, Fen Danpi (Moutan Bark) 2 qian, Cishi (Magnetite) 3 qian, Fuling (Poria) 3 qian, Tianhuafen (Trichosanthes Root) 3 qian, Yurou Tan (Cornus Fruit Char) 1 qian, Fei Zhimu (Anemarrhena Rhizome) 2 qian, Zi Hage (Clam Shell) 6 qian, Niuxi (Achyranthes Root) 3 qian, Tian Maidong & Mai Dong (Ophiopogon Tubers) 2 qian each, Zhi Sangpi (Mulberry Bark) 3 qian, Hutun Wuweizi (Schisandra Berry) 3 fen.
[Chen Case] Lungs combine with skin/hair; pores open, allowing wind entry. Sealing must be tight. Who seals? The Kidney. Classics say: "Kidney is the foundation of sealing and storing, residence of essence." Essence stores inside, exterior qi seals outside. Kidney emptiness leads to autumn/winter cough/asthma due to lack of storage. Slight contact causes wind-cold, exterior insecurity. Lung above governs exhalation, Kidney below governs inhalation. Kidney deficiency fails to inhale Lung qi, causing shortness of breath on movement. Chronic phlegm-fluid carried by Chong qi upward causes itching/cough/hemoptysis. Both Kidney Yin and Yang deficiency cause failure to store. Left pulse string-large-rapid indicates Kidney Yin deficiency, not Yang deficiency with cold-fluid. Sputum is sticky, not water-fluid. Fluid turns to phlegm via heat. Jingde's Xiao Qinglong Tang and Zhenwu Tang treat fluid-retention. Fluid differs from phlegm-turbidity. Lower deficiency requires immediate action. Use Zuogui Yin method.
Prescription: Zi Kou Hage (Clam Shell) 4 qian, Sheng Di Tan (Charred Rehmannia) 4 qian, Huai Shanyao (Chinese Yam) 3 qian, Chang Niuxi (Achyranthes Root) 3 qian, Yurou (Cornus Fruit) 2 qian, Bai Fuling (Poria) 3 qian, Cheqianzi (Plantago Seed) 2 qian.
[Gu Child Case] Cold enters Lung; slight exposure triggers external cold contacting latent cold, causing frequent asthma/cough/hemoptysis. Asthma damages lung collaterals, distinct from vomiting blood. Latent cold deep, lung heat unresolved. Lung is delicate; excessive acrid-warm may damage it. Prescribe acrid-warm combined with cold, heavy dose light administration.
Prescription: Mahuang (Ephedra Stem) 3 fen, Chuan Guizhi (Cinnamon Twig) 3 fen, Shigao (Gypsum) 1.5 qian, Sheng Shu Gancao (Licorice Root) 2 fen each, Bai Fuling (Poria) 3 qian, Dan Ganjiang (Dried Ginger) 2 fen, Guang Xingren (Apricot Seed) 3 qian, Dongguazi (Winter Melon Seed) 3 qian.
[Some Patient Case] Phlegm-asthma triggered by labor and cold. Wheezing sounds, chest discomfort precedes illness. Phlegm obstruction and qi sinking chronic. Tongue white coating, pulse deep-stringy. Follow Xiao Qinglong modification. Follow Jingde's intent, free Lung qi to allow phlegm descent.
Prescription: Mahuang (Ephedra Stem) 3 fen, Du Su Zi (Perilla Fruit) 2 qian, Qianhu (Peucedanum Root) 1.5 qian, Bai Jiezi (Mustard Seed) 3 fen, Nan Shashen (Adenophora Root) 3 qian, Sheng Gancao (Licorice Root) 2 fen, Xuanfuhua (Inula Flower) 1 qian, Guizhi (Cinnamon Twig) 2 fen, Wei Shengjiang (Ginger) 1 slice, Gualou Ren (Trichosanthes Seed) 2 qian, Baishao (White Peony Root) 1.5 qian, Juhong (Citrus Peel) 6 fen, Pipaye (Loquat Leaf) 2 leaves.
[Right Liver Case] Yin deficiency, wood stagnation, Chong qi carries phlegm-water up. Left lower abdomen burning, qi rises straight up, headache/red face, throat obstruction (Shaoyang meridian passes throat, Gallbladder is outer mansion of Liver). Yang qi ascends, damaging yang collaterals, causing hemoptysis and fatigue. Frequent hemoptysis reduces asthma frequency as stagnant yang vents through blood. Pulse soft-stringy, coating white, body red. Liver-Kidney Yin deficiency is source, Chong-Yang ascending is transmission site. Phlegm-fluid formulas do not fit. Must nourish water/liver, grasp Kidney Yin. Water rising stops, phlegm becomes fluid. Use Zuogui Yin modified.
Prescription: Da Shengdi (Rehmannia Root) 4 qian, Shan Yurou (Cornus Fruit) 2 qian, Huai Niuxi (Achyranthes Root) 3 qian, Bai Fuling (Poria) 3 qian, Gaidai San (Clam-Cinnabar Powder) 5 qian, Maidong (Ophiopogon Tuber) 3 qian, Chao Hei Danggui (Angelica Sinensis) 1.5 qian, Cheqianzi (Plantago Seed) 2 qian, Xian Qiushi (Human Urine Salt) 6 fen, Sheng Baishao (White Peony Root) 2 qian, Nüzhenzi (Ligustrum Fruit) 3 qian, Danpi Tan (Moutan Bark Char) 1.5 qian.
[Yan Case] Acrid-warm combined with cold reduced asthma significantly. Cold-heat obstructing Lung. Adjust effective formula.

Third Visit: Yu's method worked initially, then asthma worsened. Fluctuating asthma, seminal emission for a month. Pulse deep-stringy, Right weak. Upper deficiency, lower excess. Use Lei Shaoyi's method.
Prescription: Zhi Banxia (Pinellia Tuber) 1.5 qian, Shu Di Tan (Charred Rehmannia) 4 qian, Du Su Zi (Perilla Fruit) 3 qian, Cheqianzi (Plantago Seed) 2 qian, Shang Chunghou (Magnolia Bark) 7 fen, Qianhu (Peucedanum Root) 1 qian, Bai Fuling (Poria) 3 qian, Niuxi Tan (Achyranthes Root Char) 3 qian, Zi Kou Hage (Clam Shell) 5 qian, Juhong (Citrus Peel) 1 qian.

Prescription: Zhi Banxia (Pinellia Tuber) 1.5 qian, Su Zi (Perilla Fruit) 3 qian, Bai Fuling (Poria) 3 qian, Fen Qianhu (Peucedanum Root) 1 qian, Juhong (Citrus Peel) 1 qian, Cheqianzi (Plantago Seed) 2 qian, Xuanfuhua (Inula Flower) 2 qian, Guang Xingren (Apricot Seed) 3 qian, Huai Niuxi (Achyranthes Root) 3 qian, Duqi Wan (Duqi Pill) 5 qian.
Sixth Visit: Cold aversion gone, phlegm-asthma absent. Upper excess, lower deficiency confirmed. Treat upper and lower separately.
Prescription: Zhi Banxia (Pinellia Tuber) 1.5 qian, Fuling (Poria) 3 qian, Cheqianzi (Plantago Seed) 3 qian, Huai Niuxi (Achyranthes Root) 3 qian, Qizi (Goji Berry) 3 qian, Su Zi (Perilla Fruit) 3 qian, Juhong (Citrus Peel) 1 qian, Zi Hage (Clam Shell) 6 qian, Huai Shanyao (Chinese Yam) 3 qian, Yurou (Cornus Fruit) 2 qian, Pipaye (Loquat Leaf) 4 leaves, Duqi Wan (Duqi Pill) 6 qian.
Seventh Visit: Kidney Yin grasped, but Yang rises causing head distension/insomnia. Excess Yang, insufficient Yin. Expand previous method.
Prescription: Shengdi (Rehmannia Root) 4 qian, Shanyao (Chinese Yam) 3 qian, Niuxi (Achyranthes Root) 3 qian, Sheng Baishao (White Peony Root) 2 qian, Yun Fuling (Poria) 2 qian, Yurou (Cornus Fruit) 2 qian, Cheqianzi (Plantago Seed) 2 qian, Sheng Muli (Oyster Shell) 5 qian, Yejiaoteng (Polygonum Vine) 5 qian, Longgu (Fossilized Bone) 3 qian, Duqi Wan (Duqi Pill) 5 qian.
Supplementary Formula: Phlegm-fluid in Lung/Stomach, Kidney empty. Contact triggers internal fluid, causing asthma/cough. Coughing reverses Kidney qi upward. At critical stage, nourishing Kidney/grasping works instantly. Deficiency with slight excess. Focus on root.
Prescription: Da Shudi (Rehmannia Root) 12 liang, Huai Niuxi (Achyranthes Root) 1.5 liang, Buguzhi (Psoralea Fruit) 2 liang, Bai Fuling (Poria) 3 liang, Shang Mianqi (Astragalus Root) 3 liang, Gan Qizi (Goji Berry) 3 liang, Zhi Banxia (Pinellia Tuber) 1.5 liang, Bajirou (Morinda Root) 2 liang, Hang Baishao (White Peony Root) 1.5 liang, Yurou (Cornus Fruit) 1.5 liang, Zhi Shouwu (Polygonum Root) 4 liang, Cheqianzi (Plantago Seed) 1 liang, Yuzhu (Atractylodes Rhizome) 2 liang, Tusizi (Cuscuta Seed) 2 liang, Shanyao (Chinese Yam) 3 liang, Chen Guangpi (Tangerine Peel) 1 liang, Hutaorou (Walnut Kernel) 3 liang, Kui Dangshen (Codonopsis Root) 3 liang, Zi Kou Hage (Clam Shell) 5 liang, Qianshi (Lotus Seed) 3 liang, Zhihei Cao (Licorice Root) 5 qian, Tong Shayuan (Astroseed) 3 liang.
Decoct three times, reduce to paste. Add Guiban Jiao (Tortoise Shell Glue) 1 liang, Lu Jiao Jiao (Deer Antler Glue) 2 liang, Ejiao (Donkey-Hide Glue) 2 liang. Take 7-8 qian with hot water.
[Guo Case] Asthma: Lung is excess, Kidney is deficiency (regarding Qi, not just phlegm). Now phlegm fills bowl, hoarse voice, back cold, mouth sticky no thirst. Right pulse fine-weak-stagnant, Left string-large. Weak Qi generates phlegm. Liver-Kidney deficiency, Wood lacks nourishment. Awakening of Yang causes Qi to rise, old fluid/phlegm reverse up, blocking Lung descent. Fear of upper excess/lower deficiency, Yin-Yang separation. Phlegm/saliva are Yin; drinkers use warm harmonizing herbs. But turbid Yin overflowing contradicts malar flush/appetite. Lung wants warmth, Kidney wants coolness. Prescribe acrid-warm combined with cold.
Prescription: Chuan Guizhi (Cinnamon Twig) 4 fen, Bai Fuling (Poria) 3 qian, Dan Ganjiang (Dried Ginger) 4 fen, Hai Ge Fen (Clam Powder) 5 qian, Wei Shigao (Gypsum) 3 qian, Chao Maidong (Ophiopogon Tuber) 2 qian, Bei Shashen (Glehnia Root) 5 qian, Xingren Ni (Apricot Paste) 3 qian, Wuweizi (Schisandra Berry) 6 grains, Erquan Jiao (Collagen Gelatin) 1 qian.
