Black discoloration in the center of the forehead, accompanied by redness on both cheekbones, indicates death from war within sixty days. If redness appears at the "Nian Shang" (area above the nose root), it indicates death within thirty days. If redness appears at the "Ming Men" (in physiognomy, the area before the ears is Ming Men, and the space between the eyebrows is Ming Gong), it indicates death in a market or marketplace within one hundred days. (Recorded in *Qianjin Yi Fang*.)
When a woman is about to give birth or during difficult labor, to determine life or death and auspiciousness or inauspiciousness, observe if the mother's face, lips, and tongue are cyanotic, with excessive salivation that cannot be stopped; in such cases, both mother and child will die. If the lips are cyanotic but the tongue is red, the mother dies and the child lives. If the face and lips are red as usual, but only the tongue is cyanotic, the child dies and the mother lives. (From Chao Yuanfang.)
The tongue is the sprout of the Heart. Red or purple color indicates Heart Heat; darkened black indicates extreme Heart Fire (though Cold-Dampness is also possible); pale white indicates Deficiency.
The tip of the nose and the gum margins are the orifices of the Spleen. Red and dry nose tip indicates Spleen Heat. Dull yellow indicates Spleen Failure. Red and swollen gums indicate Heat. Ulceration indicates Fire toxin in the Stomach and Spleen.
The lips are the orifices of the Stomach and Spleen. Red and purple indicates Heat; pale white indicates Deficiency; jet black indicates impending exhaustion of Stomach and Spleen Qi. Deviation of the mouth corner to the right indicates Liver Wind; deviation to the left indicates Phlegm of the Spleen.
The nostrils are the orifices of the Lung. Dryness indicates Heat. Clear runny nose indicates Cold.
The ears and teeth are the orifices of the Kidney. Tinnitus indicates Disharmony of Qi; teeth growing as large as soybeans (abnormality) indicates exhaustion of Kidney Qi.
The eyes are the orifices of the Liver. A fierce gaze with flexible eye movement indicates Wind. Fixed staring without eye movement indicates impending exhaustion of Liver Qi. Subdividing the parts of the eye, they correspond to the five Zang organs. The black pupil belongs to the Liver; pure yellow color is an ominous sign. The white sclera belongs to the Lung; blue-green color indicates Liver Wind insulting the Lung; pale yellow indicates Spleen stagnation; old yellow indicates Lung affected by Damp-Heat, resulting in Jaundice. The pupil belongs to the Kidney; lack of luster combined with yellowing indicates Kidney Qi Deficiency. The outer canthus (large angle) belongs to the Large Intestine; ulceration indicates Lung Wind. The inner canthus (small angle) belongs to the Small Intestine; ulceration indicates Heart Heat. The upper eyelid belongs to the Spleen; swelling indicates Spleen injury. The lower eyelid belongs to the Stomach; bluish color indicates Stomach Cold. Upper and lower eyelids not closing tightly, leaving a slit, indicates extreme Deficiency of Stomach and Spleen.
There are five areas on the face, each belonging to a Zang organ. The forehead belongs to the Heart, corresponding to Li Fire. The left cheek belongs to the Liver, corresponding to Zhen Wood. The right cheek belongs to the Lung, corresponding to Dui Metal. The area above and below the lips belongs to the Kidney, corresponding to Kan Water. The five Zang organs are Interior. The six Fu organs are Exterior. The Small Intestine is the exterior counterpart of the Heart; short, yellow, painful, and difficult urination indicates Heart Heat; clear, long, and free urination indicates Deficiency. The Stomach is the exterior counterpart of the Spleen; red lips with vomiting indicate Stomach Heat; pale white lips with vomiting indicate Stomach Deficiency; normal lip color with vomiting should be treated as Stomach Injury. The Large Intestine is the exterior counterpart of the Lung; constipation indicates Lung Fire. If there is no Lung Heat but stool is retained, it must be Blood Dryness; purgatives should not be used. Rectal prolapse indicates Lung Deficiency. The Gallbladder is the exterior counterpart of the Liver; bitter taste indicates Liver Excess; being startled by sound indicates Liver Deficiency (also due to Phlegm in the Pericardium; every time sound is heard, limbs convulse). The Bladder is the exterior counterpart of the Kidney, located to the right of the Qi Sea below the umbilicus; it has a name but no form; tendon swelling and pain indicate Cold Qi of Kidney Water invading the Bladder.
There are five colors of the face: 1. Red indicates disease in the Heart; red face indicates Heat. 2. Cyan indicates disease in the Liver; cyan face indicates Pain. 3. Yellow indicates disease in the Spleen; yellow face indicates Spleen injury. 4. White indicates disease in the Lung; white face indicates Cold. 5. Black indicates disease in the Kidney; black face without luster indicates failure of Kidney Qi. Observing the color, if it differs from usual, and the color of the orifices matches the facial color, then the Deficiency or Excess of the Zang-Fu organs will be accurately diagnosed. (The term "orifices/sprouts" refers to the meaning of gates, wells, and stoves in *Qian Jin Fang*, not merely for distinguishing Zang organ locations.)
Umbilical Tetanus (Qi Feng) is caused by Wind-Cold entering through the umbilicus, occurring within seven days after birth. (Some cases are congenital, where Life Fire is incomplete, and Cold rushes up from below the umbilicus, hence all are called Umbilical Tetanus.) When Wind attaches to Wood (Liver), it causes noise; since the eyes are the orifices of the Liver, yellow color first appears at the corners of the eyes. Liver Evil overacting on the Spleen affects the nose tip, which is the orifice of the Spleen, so yellow appears on the nose. From Spleen affecting the Kidney, the lips turn yellow and the mouth puckers. A stiff tongue indicates Kidney Evil invading the Heart. In the early stage, sucking milk will be slightly looser than before. Sudden yellow color near the eyebrows at the eye corners requires urgent treatment. If the yellow reaches the nose, it is still easy to treat; if it reaches the Ren Zhong and Cheng Jiang points, treatment is more difficult. If the lips and mouth are already tightly bound and the tongue is rigid, treatment is futile. (*Ji Cheng* discusses that children with strong constitution often show yellow at the eye corners and nose tip, but this author verified it personally and it is not false talk. Those with strong constitution have a red face, and their yellow is deep and hidden, showing red through the yellow. Umbilical Tetanus presents with a premature aging/dying complexion, and the yellow is floating and stagnant. From Xia Yuzhu's *You Ke Tie Jing*.)
Blue veins appearing straight or horizontal above the Shan Gen (root of the nose) all indicate Liver Heat. Red veins appearing straight or diagonal all indicate Heart Heat. Yellow veins appearing at the Shan Gen, or yellow skin color, regardless of orientation, all indicate Spleen-Stomach disorders, manifesting as vomiting or diarrhea, abdominal pain, or loss of appetite. (From Chen Yuan Gong's method.)
Protrusion of the fontanelle indicates Wind-Heat and Damp-Heat in the Liver, Kidney, Lung, and Stomach. Depression indicates Congenital Insufficiency or excessive diarrhea/loose stools leading to Spleen-Kidney Deficiency and Cold Qi weakness. Rapid jumping or irregular discontinuation indicates Qi Collapse. (Added content.)
