The *Neijing* (Classic of Internal Medicine), in the chapter "Zhi Zhen Yao Da Lun" (Great Treatise on the Most Essential Principles), lists pathogenesis mechanisms; those mentioning "Fire" number five, and those mentioning "Heat" number four, seemingly all referring to Fire pathogens. However, the severity and urgency of various diseases recorded in different chapters vary significantly, with diseases arising from deficiency being the most critical. The rationale behind this is not difficult to understand.
(It is said that this prescription originated as a fire-clearing agent from Tai Cang Gong, used by Cui Shi to treat Commander Liu Hu, and was also claimed to be his own creation.) It treats all types of Fire-Heat conditions, characterized by exterior heat excess, mania, restlessness, and vexation. (When pathogenic factors enter the Yang aspect, mania occurs; when the Heart is disturbed by Heat, vexation arises; restlessness is an intensification of vexation.) Dry mouth and throat. (Excessive Fire consumes body fluids.) High fever and dry retching. (Rebellious upward movement of Heat toxins.) Incoherent speech and insomnia. (Heat confounds the Spirit. Minister Cui stated: Dry feces in the Stomach can cause incoherent speech.) This can also lead to incoherent speech. If incoherent speech accompanies constipation, Chengqi Tang (Gut-Purging Decoction) is appropriate; if it occurs despite normal bowel movements, Huanglian Jiedu Tang (Coptis Decoction for Resolving Toxicity) is appropriate.) Hematemesis (vomiting blood) and epistaxis (nosebleed). Epistaxis occurs when sweating should have been induced but was not, causing accumulated Heat to force blood upward.) Heat rash (erythema) due to extreme heat. (Heat toxins entering the Stomach.)
Huanglian (Rhizoma Coptidis), Huangqin (Radix Scutellariae), Huangbai (Cortex Phellodendri), Zhizi (Fructus Gardeniae), in equal proportions.
Accumulation of Heat in the Triple Burner causes Evil Fire to act recklessly. Therefore, Huangqin is used to clear Lung Fire in the Upper Burner, and Huanglian to clear Spleen Fire in the Middle Burner. (Wang Haizang stated: Huanglian... Bladder... slight heat... three times... damp-heat... made into water pills, named Sanhuang Jinhua Wan (Three-Yellow Flower Pill). It treats various internal and external heat syndromes, night sweats, teeth grinding, somniloquy, and palpitations with fright. Hematemesis, epistaxis, stranguria, and constipation, consumptive cough, and bone-steaming fever. Adding Dahuang (Radix et Rhizoma Rhei) creates... porridge pills... fire generates... qi... ( ...sweat ...Yang ...Lian ...Dan ...receive ...then blood returns to its vessels and settles naturally. Kou Zongshi stated: Using bitter substances to drain heat simultaneously nourishes the Heart with bitterness, achieving two goals with one action. Wu Hegao stated: Treating disease requires seeking its root. When Yang toxicity causes bleeding from upper orifices, Heat is the root, and blood loss is the branch. Removing the Heat allows the blood to return to its vessels without specific hemostatic treatment. Li Shicai stated: The ancients' use of Dahuang to treat hematemesis due to consumption is profound. Because if turbid Yin does not descend, clear Yang cannot ascend. If stasis blood is not expelled, new blood cannot generate. Yang Renzhai stated: Blood flows chaotically when exposed to Heat, so cooling herbs are commonly used for hemostasis. However, there are cases of Qi deficiency with Cold, or deficient and scattered Ying-Qi, where blood also moves erratically, known as 'when Yang is deficient, Yin must leak.' The method should be to warm the Middle to allow blood to return to its vessels naturally. Lizhong Tang (Regulate the Middle Decoction) with Muxiang (Radix Aucklandiae) or Qi Qi Tang (Seven-Air Decoction) with Chuanxiong (Rhizoma Chuanxiong), or Gancao Ganjiang Tang (Licorice and Dried Ginger Decoction) are very effective.) Additionally, Dahuang soaked in wine, steamed and sun-dried nine times, formed into honey pills, is named Sanhuang Wan (Three-Yellow Pill). It treats accumulation of Heat in the Triple Burner, swelling and pain of the head and neck, red eyes, oral ulcers, irritability in the chest and diaphragm, constipation, scanty and painful urination, as well as Xiao Ke (Wasting-Thirst syndrome) and emaciation. (Xiao Ke and emaciation result from blazing Fire drying up Kidney Water.)
