Mr. Xu discussed the case of Liang Kuanfu, whose illness was located in the lung area of the right hypochondrium. He suffered from coughing and expectorating blood, and panting with reverse flow upon movement; this is Lung Abscess (Fei Zhong, a purulent lesion of the lung). Accompanied by fever, rapid pulse (Shu Mai), and inability to eat, this indicates Fire Evil (Huo Xie) overcoming Metal (Lung), resulting in deficiency of both the Lung and Spleen. When both the Lung and Spleen are deficient and Fire Evil takes advantage of this, the condition becomes adverse. In such cases, standard tonification or purgation methods should not be used. If one tonifies Metal (Lung), there is a concern that Metal and Fire will contend, exacerbating panting and coughing; if one purges Fire, there is a concern that Fire will not retreat from its position, causing stagnation masses (Pi Ji) to worsen. It is appropriate to first use Buzhong Yiqi Tang (Tonify the Middle and Boost Qi Decoction) to support Yuan Qi (Primary Qi), adding a small amount of medication for the specific disease. It was heard that medication had been administered without effect; this must be because the disease momentum is severe and resistant, and the medicinal force has not yet reached the site. Perhaps recovery can be expected when autumn coolness arrives! Since Lung diseases dislike the Fire energy of Spring and Summer, and when Autumn and Winter arrive the Fire energy recedes, one should only adjust the dosage of Yiqi Tang according to the rising and falling, cold and heat of the four seasons, and modify it based on visible symptoms. If one feels Qi stagnation, occasionally administer Jiajian Zhizhu Wan (Modified Atractylodes and Immature Bitter Orange Pill); if there is fluid retention, occasionally administer Zhizhu Tang (Atractylodes and Immature Bitter Orange Decoction) from *Jufang*. After a few days, if the reverse Qi slightly returns, treatment methods can be applied. However, if the complexion has already turned green or red, and the pulse has become wiry (Xian Mai) or flooding (Hong Mai), it may be too late! Without observing the complexion and pulse after the illness, one cannot speculate remotely. Speaking of recurrence after recovery, one should strictly follow seasonal medication principles to support Yuan Qi, so that future recoveries do not recur. The initial infection must have been deep, and the damaged substances might not have been fully guided out at that time, remaining stagnant. Prolonged delay led to other symptoms, resulting in the current state. It is advisable to add a small amount of digestive/guiding-out medications to Yiqi Tang, so that gradual efficacy can be achieved.
Danxi stated: For mixed evil conditions (Zahe Xie), one should employ mixed treatment methods. For example, aversion to cold and fever, caused by common cold, clearly indicate external evil; the pulse is floating and rapid, and the Qi Kou pulse is tight and full, clearly indicating food injury. The patient also feels fatigued; heavy pressure on the pulse reveals a hollow sensation, accompanied by chest and diaphragm stuffiness pulling into both hypochondria. In mild cases, the pulse seems wiry, which is due to Liver evil caused by frequent anger; careful examination of the Left Chi pulse seems deep and weak, which is due to past excessive sexual activity. The treatment method should set aside the common cold aspect, observe the patient's form, color, strength, and thickness, and initially provide medications to tonify the middle, transform food, and move stagnation. Then, cool Stomach Fire, and use ginger and pepper to promote circulation. Once Middle Qi slightly recovers, injured Qi moves, fluids harmonize, and the whole body sweats, external evil resolves naturally. If one does not investigate thoroughly but merely disperses the exterior, fails to deduce concurrent evils and pulses, and fails to inquire exhaustively about the origin of the disease, while catering to the patient's temperament and stubbornly applying clever mixed treatments, one will see Primary Qi becoming deficient and Evil Qi becoming entrenched. This is all the fault of clumsy practitioners.
