子以泻得之为重;女子以吐得之为重。古人有云∶病家怕惊不怕泻,医家怕泻不怕惊。如泻久不止,且先治泻;

[Song] Pediatric convulsion diseases have various origins; some arise from food stagnation combined with fright, others from phlegm accumulation. Consequently, sudden spasms and twitching occur, with teeth grinding, eyes rolling upward, and tendon cramping in the feet. Food stagnation requires digestion and guidance (to resolve), while phlegm must be resolved to clear the airways; clearing Heart Fire allows fright to settle naturally. If fright-convulsion involves Heat accumulation in the Heart, wind-phlegm attacks originate from the Liver. In an instant, urgent spasms resemble wind and fire; this involves four syndromes: Fright, Wind, Heat, and Phlegm. Vomiting and diarrhea cause Spleen-Stomach deficiency; half-open eyes, drowsiness, and whole-body coldness indicate a slow progression. Slow convulsion is a transmission to Yin syndromes; if emergency treatment is delayed, it may be difficult to save. Extreme deficiency affecting the Spleen is critical; Spleen Wind can lead to death within moments. Sudden fright-crying due to external shock (Ke Wu) presents with convulsions and eye deviation. Internal coiling (Nei Diao) is caused by excessive internal Cold; infants also suffer from Umbilical Wind and Epilepsy. These fright conditions vary in depth; do not treat them vaguely as trivial matters.
[Discussion] Children's Yuan Qi (Original Qi) is not yet full, and their Shen (Spirit) is not yet settled, making them prone to fright and palpitations, leading to various illnesses. This may result from overeating causing heat obstruction; encountering fright after eating; phlegm gathering due to empty dwelling environments; or internal heat generating wind. Thus, there are four types of convulsions: Fright Convulsion, Wind Convulsion, Food Convulsion, and Phlegm Convulsion. One should examine the cause and treat accordingly. If there is Excess Heat coupled with external Wind pathogen, the Heart receives heat causing fright, and Liver Wind generates convulsions. With abundant phlegm-saliva, wind and fire act together, blocking all vessels and orifices. When wind-heat proliferates without outlet, it erupts violently as Acute Fright Convulsion (Ji Jing Feng). Symptoms include tight jaw, tidal flow of hot saliva, staring eyes, opisthotonos, twitching, head shaking, mouth and eye deviation, hot breath, red lips, constipation, dark urine, and a floating, rapid, surging, or tight pulse. Alternatively, after vomiting, diarrhea, cold damage, dysentery, hematemesis, or hematochezia, insufficient Yuan Qi generates wind and fright, named Chronic Fright Convulsion (Man Jing Feng). Symptoms include whole-body coldness, cold breath from mouth and nose, or slight body heat without coldness, fluttering limbs, drowsiness with exposed eyes; this indicates absence of Yang, requiring urgent warming and tonification, which can be treated according to symptoms. If the face is green, forehead sweats, tongue is short, head droops, eyes are closed in sleep, frequent head shaking and vomiting, locked jaw with teeth grinding, slight limb twitching that cannot retract, cold body and extremities, and a faint, deep pulse, this is the sign of Slow Spleen Wind (Man Pi Feng). Caused by vomiting/diarrhea damaging the Spleen, the disease has progressed extremely, ultimately affecting the Spleen. If one tries to expel wind, there is no wind to expel; if one treats fright, there is no fright to treat. Only phlegm-saliva in the Spleen and alternating deficiency-heat remain, which is very critical. Large doses of Ginger and Aconite may save one or two out of ten. Tian Diao (Heavenly Suspension/Fright) is a Yang syndrome, characterized by high fever, fright convulsions, rolling eyes, hand and foot cramps, crying or laughing, irregular emotions, and severe cases showing blue nails resembling possession. This is generally due to the nursing mother consuming excessive alcohol and rich food, allowing toxic vapors to enter the milk, causing Heat in the Heart and Lungs, phlegm stagnation, and Qi blockage, compounded by external Wind. Treatment should focus on resolving Wind-Heat. Nei Diao (Internal Coiling) is a Yin syndrome, presenting with abdominal pain, frequent crying, black lips, swollen testicles, stooping, and opisthotonos, with red veins and blood spots in the eyes. It arises from Qi stagnation combined with fright, or from fright/wind in the womb. Initially, internal organs spasm with extreme pain and screaming, or vomiting/diarrhea with leg curling and pain tolerance. After the Nei Diao episode passes, external convulsions return; internal and external attacks make it difficult to manage, requiring separate treatments for both aspects. Night crying is due to Cold in the Zang organs; Yin is dominant at night, causing Cold movement, which leads to extreme Yin agitation, Cold-induced pain, hence night crying. There are also cases of Heart Heat causing烦躁 crying, accompanied by red face, white tongue, and dark/scanty urine; these must be differentiated by Cold and Heat. Ke Wu (Guest Shock) occurs because children's Shen Qi is tender and weak; external guests, animals, or strange objects touch and offend them. Symptoms include fright crying, yellow-white foam from the mouth, changed complexion, panting, abdominal pain, and convulsions resembling epilepsy but without upward eye deviation. The pulse is wiry, rapid, and tense. Inspect the mouth for a uvula swelling or small nodules on either side, and prick them with a bamboo needle. Treatment should prioritize dispelling evil Qi, scattering fright, and calming the Heart; delaying makes it harder to treat. Epilepsy has five types and is a severe pediatric condition. Young children have unconsolidated Blood-Qi and ungathered sinews/bones, susceptible to Wind pathogen injury, fright/shock, irregular feeding, retained phlegm, and mass formation. Symptoms include depressed Spirit, staring eyes, facial traction, locked jaw with salivation, abdominal distension, limb jerking, speaking or silence, appearing dead or alive, opisthotonos, or stiff spine, but soft limbs, waking intermittently after attacks—this is Epilepsy. The manifestations of the Five Zang are listed below. Jing Syndrome (Spasm) refers to rigidity. Jing has Yin and Yang syndromes, named Gang (Rigid) and Rou (Flexible), characterized by whole-body stiffness and unconsciousness lasting all day. Epilepsy has Wind, Fright, and Food syndromes; investigate causes and observe symptoms to prescribe correctly. Ignoring names and treating blindly is like asking for directions at sea.
Acute Fright Convulsion
Discussions on Acute Fright Convulsion were not recorded in ancient texts, only referred to as Yang Epilepsy. Generally, it results from lack of care, exposure to wind, proximity to heat, excessive spicy food during the day, or thick bedding at night. Depressed and steaming Evil Heat accumulates in the Heart, transmits to the Liver, and is triggered by fright from people/objects, falls, shouting, thunder, drums, music, barking dogs, etc. Before onset, sleep is restless, crying in sleep, grinding teeth, biting nipple, sweating on nose/forehead, rapid breathing with phlegm wheezing. Suddenly becoming comatose, staring straight ahead, tight jaw, locked mouth, limb convulsions—this is due to extreme Heat, especially with red face and rapid pulse. Heart has Heat, Liver has Wind; both are Yang within Yang. Heart is Fire, Liver is Wind. Wind and Fire are Yang entities. Wind governs movement; Fire with Wind creates flames—this is the creation of the Five Elements. Two Yangs drumming together, Wind and Fire fighting, Liver stores Hun (Ethereal Soul), Heart stores Shen (Spirit); Heat makes Hun and Shen move easily, causing Fright. Heart governs Spirit and is not directly touched by fright; upon fright, it generates Heat, Heat extreme generates Wind, causing convulsions, named Acute Fright Convulsion. Use Xie Qing Wan to drain Liver, Dao Chi San to drain Heart; Zhu Po Wan and Niu Huang Wan are essential medicines. For Fright Convulsion with blocked orifices and unconsciousness, use Tong Ding San. For Fright Convulsion with tidal hot saliva and tight jaw, use Meng Shi Gun Tan Wan. For Fright Heat, use Liang Jing Wan. For Wind excess causing convulsions, use Tian Ma Fang Feng Wan. If a child has only Phlegm-Heat without Fright Convulsion, only clear Heat and transform Phlegm; do not recklessly administer Fright Convulsion medicines. Since meridians are originally healthy, attacking with Cold-Cooling drugs forces Phlegm-Heat inward, creating Wind-Epilepsy. Acute Fright Convulsion with three or two恶叫 (evil screams) indicates Heart exhaustion, difficult to treat. Limb flaccidity in Acute Fright Convulsion is untreatable. Nasal bleeding in Acute Fright Convulsion indicates Heat dispersing, thus easy to treat. Oral bleeding indicates reckless blood movement, difficult to treat. Constipation in Fright Convulsion is easy to treat; enuresis/incontinence is difficult. Black vertical lines on eyebrows in Acute Fright Convulsion indicate death.

Chronic Fright Convulsion & Slow Spleen Wind
Chronic Fright Convulsion has various causes: using Cooling medicines for Acute Fright Convulsion excessively damaging Spleen-Stomach, allowing Wind pathogen to invade; continuing vomiting/diarrhea; sudden vomiting/diarrhea due to Qi deficiency; progression of Cold Damage over time; chronic cough; or aggressive purging of accumulation. Only vomiting/diarrhea, accumulation, and dysentery turning into Deficiency change rapidly. Any vomiting/diarrhea immediately becomes Chronic Fright Convulsion, requiring warming the Middle and promoting interior function. If convulsions are urgent, it is early transmission of Chronic Fright Convulsion, still having Yang signs; do not mistake it for Acute Fright Convulsion and use Cooling medicines. People say slow convulsions are Chronic Fright Convulsion, which is incorrect; adhering to this often misidentifies Slow Spleen Wind as Chronic Fright Convulsion. For Chronic Fright Convulsion, Diarrhea is heavier for males; Vomiting is heavier for females. Ancients said: Families fear Fright more than Diarrhea; Doctors fear Diarrhea more than Fright. If diarrhea persists, treat diarrhea first; treating Wind will worsen Fright Convulsion. Only nourish Spleen, calm Stomach, generate fluids; Da Yang Pi Wan, Shen Ling Bai Zhu San, Da Xing Pi San, Wu Xu Si Jun Zi Tang can be taken anytime. If due to other syndromes, treat according to root cause. Chronic Fright Convulsion with pulling on one side of mouth, eyes, hands, feet is difficult to treat. Lips should be red in Chronic Fright Convulsion; if fontanelle swells, death ensues. Zhang Zhongjing said: Yin syndromes should not sweat. Yin syndromes have no sweat; sweating means loss of Yang. Thus, Chronic Fright Convulsion with profuse sweating is fatal.
Transmission from Chronic Fright Convulsion to Slow Spleen Wind changes rapidly and is extremely deficient. Treatment must be gradual and mild; there is no principle of quick recovery. The main method is generating Stomach Qi and restoring Yang; do not overdose. Zhong Yang has Huang Tu Tang, using Earth to overcome Water, balancing Wood so Wind stops naturally, based on Spleen Earth. If eyes are half-open/half-closed and extremities are not cold, the syndrome is still Chronic Fright Convulsion; do not use Yang-restoring methods. If already in Slow Spleen Wind but Yang is not yet collapsed, do not immediately use Sulfur or Aconite formulas. As extremities warm, regulate with Xing Pi San etc. Medicines for Slow Spleen Wind are used only when necessary; it is critical like a lamp running out of oil, gradually dimming. If one Zang organ fails in Slow Spleen Wind, medication is contraindicated.

Ke Wu implies being offended/touched. Children's Blood-Qi is timid, Spirit is vague; suddenly encountering strangers or cattle/horses causes Fright. Appearance resembles Epilepsy but eyes do not roll up; pulse is wiry, urgent, and rapid. If struck by Evil Guest Shock, urgently make Vinegar Charcoal or fumigate with Jiang Xiang and Zao Jiao. Administer Su He Xiang Wan with Ginger decoction. Crush Dou Chi into pills sized like bullet seeds, rub on five centers (palms, soles, chest, back, abdomen) five to six times.
Su He Xiang Wan (Formula see Vol 2 Stroke section.)
None
Xiao Er Tian Diao is named for upward eye gaze. Symptoms: unsettled Spirit, whole-body high fever, limb convulsions, palpitations, rolling eyes, Phlegm-Heat obstruction. This condition is due to nursing mother's irregular diet, excessive alcohol/meat, toxic vapors entering milk, causing unresolved stagnation, Heat in Heart/Lungs, Heat stagnation, compounded by external Wind. Moreover, children's convulsions always involve upward eyes, truly a Yang Convulsion sign. Treatment should remove Phlegm, resolve Wind-Heat, use Gou Teng Yin. If Yu Kou (Fish Mouth), Ya Sheng (Crow Sound), dull eyes, or black nails appear, death ensues.
Gou Teng Yin

Jing Syndrome (Spasm)
Signs result from initial Wind injury followed by Cold-Dampness, representing extreme Deficiency generating Heat, and extreme Heat generating Wind. Wind-Cold with fever, headache, sweating, vomiting. Unclear physicians induce sweating again, inevitably causing Jing. Excessive sweating in Cold-Damp also causes Jing. Signs include neck/back stiffness, opisthotonos, head shaking, convulsions, locked jaw with fever, abdominal pain, unconsciousness; appearance is terrifying, affecting Foot Taiyang Meridian. Gang Jing (Rigid Spasm) has no sweat; Rou Jing (Flexible Spasm) has sweat. Red face, red eyes, tight teeth, open hands, abundant phlegm, dizziness, thirst, dark/scanty urine. Delirium preceding onset indicates Gang Jing; loose stool, no speech/thirst. Cold extremities preceding onset indicates Rou Jing. Modify Xiao Xu Ming Tang for both: remove Fu Zi, add Ma Huang for Gang Jing; use Fu Zi, remove Ma Huang for Rou Jing. For free stool and cold extremities, assist with Shu Fu Zi. Jing is hardest to cure; save one in ten; after three days, difficult to treat.
Xiao Xu Ming Tang (Formula see Vol 2 Stroke section.)
Epilepsy Syndrome
Generally, Epilepsy has many forms, falling into five categories: Red face, staring eyes, protruding tongue, vexation = Heart Epilepsy; Green face/lips, upward eyes, limb clenching/reversal = Liver Epilepsy; Yellow face, abdominal fullness, spontaneous diarrhea, limb flaccidity = Spleen Epilepsy; Pale face like dry bone, whites of eyes looking backward, jumping fright, foaming mouth = Lung Epilepsy; Dark/dull face, trembling eyes staring at people, clear foam, motionless like corpse = Kidney Epilepsy. Although each Zang Epilepsy has its master, they essentially involve Wind, Fright, and Food, and do not exceed Yin and Yang syndromes. Yang Epilepsy: Fever, floating pulse, dark urine, constipation, hot breath; disease in Fu organs, easy to treat. Yin Epilepsy: Cold body, deep pulse, cold breath; disease in Zang organs, difficult to treat. Treat Wind Epilepsy by first dispersing Wind. Treat Fright Epilepsy by first relieving Fright. Treat Food Epilepsy by first digesting/guiding, then using Ding Xian agents.
