All cold pathogens causing contraction and pulling of tendons belong to the Kidney. The Kidney belongs to the Shaoyin (Lesser Yin) channel. Shaoyin is the Supreme Yin. "Supreme" means extreme. The Shaoyin channel is where the pulse qi is prosperous in winter. It resides in the North and corresponds to Water. Its nature is Cold, it governs storage and return, and it governs thoroughness and secrecy. When Cold pathogen invades the body causing tendon contraction, pulling, and spasm, this is the function of Cold. The fluids discharged from above and below in its pathology are clear and cold like water. When Clear Yang is affected by Cold, blood coagulates forming masses (Jia); when the Taiyin (Spleen) channel is affected by Cold qi gathering, it forms hernias (Shan). These are all manifestations of urgent pulses indicating Cold. Urgency primarily indicates pain, hence the sensation of urgent pain. Furthermore, the *Neijing* states: frequent pulses indicate Heat, slow pulses indicate Cold. Diseases of the Yang channels are mostly Heat, diseases of the Yin channels are mostly Cold. The pulse should be slow and moderate. If Cold toxins are internally constrained, a flooding and rapid pulse indicates Heat; this is the pulse associated with the Heart. Cold Qi generates clarity and coldness, joining with Dampness, thus causing diarrhea with clear and white stools. This is because when the intestines and stomach are Cold, transformation and transportation are abnormal; when Hot, there is congestion and obstruction. When Cold prevails, Fire declines; when Fire declines, Metal prospers. Vomiting and dysentery become fishy and foul. Fishiness is the odor of Metal and Lung. Heat favors sour taste, Cold produces a fishy/watery taste. Just as cold limbs, hard and full abdomen, difficulty bending and stretching, stiffness and shivering, these are mainly governed by Yin Water (Cold Water). Difficulty in extending and curling the body, diagnosing the pulse as deep, hidden, and slow, is the sign of the disease. If the body is cool, not thirsty but afraid of cold, feet are cold while lying down, aversion to human voices, unwillingness to speak, these are all Yin syndromes. When Yin and Yang are balanced, there is harmony; when biased, there is disease. For example, if Yang Qi suddenly fails and Yin Qi alone prevails, it becomes a Cold syndrome; if Yin Qi suddenly fails and Yang Qi alone prevails, it becomes a Heat syndrome. The classics state: Excess Yang and Deficient Yin treated with sweating will lead to death; Excess Yin and Deficient Yang treated with purging will lead to death. If there is Yang Excess with external Heat and Yin Deficiency with internal Cold, or Yin Excess with internal Heat and Yang Deficiency with external Cold. For Yang Excess attacking Yang, Liang Ge San (Cool the Diaphragm Powder) and Cheng Qi Tang (Order the Qi Decoction) should be used. For Yin Excess attacking Yin, Baizhu San (Atractylodes Macrocephala Powder) and Sini Tang (Recover the Reversed Decoction) should be used. Cold is the severe, condensing, killing, and fierce energy. Humans take Kidney Qi as the root; only the Kidney can receive Cold pathogens, and only Cold pathogens can injure the Kidney. Once Kidney Qi is deficient, Cold pathogens attack alternately, causing acute pain and spasms, trembling and stiffness, coma and cold extremities, locked jaw and loss of voice, or sudden fever, red face and profuse sweating; this is Zhong Han (Mid-Cold Syndrome). No sweat, aversion to cold, headache, pale and grim face, fever with stiffness, slightly cold hands and feet; this is Shang Han (Cold Damage Syndrome). Huoluan (Colic with vomiting and diarrhea) with tendon twitching, profuse watery diarrhea, dry heaving and vomiting, retained fluid and phlegm; this is Cold pathogen entering the intestines and stomach. Leading to cough, deficiency consumption, hernias and masses, Hong Kong foot (podagra), hemorrhoids and fistulas, spermatorrhea, malaria, various pains; Cold can also cause these. Only when Kidney Qi is insufficient and protection is neglected, is it not just frost and ice that is called Cold. Sometimes in hot summer months, cooling oneself in the wind or lying on cold ground allows Cold pathogen energy to enter from the skin into the collaterals, and from the collaterals into the zang-fu organs. Thus, the aforementioned syndromes can run rampant. To warm the Kidney and dispel Cold, use Dry Ginger (Ganjiang), Aconite (Fuzi), Chuanwu, Tianxiong, etc., as assistants. Yangzheng Lingsha (Nourishing True Spirit Cinnabar) is indeed a staple in the medicine cabinet. However, when Cold damages Ying Qi (Nutritive Qi), knowing only to warm the Kidney but not to warm the Blood may not yield perfect results. Therefore, Guigui (Cinnamon Twig) and Danggui (Angelica Sinensis) are superior herbs for warming the Blood. The *Suwen* says: The three months of winter are called closure and storage. Water freezes and earth cracks; do not disturb Yang. Sleep early and rise late, waiting for sunlight. This is the meaning of avoiding Cold and seeking Warmth. Failure to regulate properly leads to being struck by Cold. Generally, the pulse of Zhong Han must be slow and tight; if accompanied by Wind, the pulse is floating, with dizziness and numbness; if combined with Dampness, the pulse is soggy, with swelling, fullness, and pain. The treatment method must absolutely not involve reckless purging or emesis; one should only use warming and dispersing methods. Otherwise, curled tongue and contracted scrotum will make it difficult to treat.
Among the six pulses, getting Xian (Wiry) and Xi (Fine) and Se (Choppy) at the lower position (Chi), pressing reveals weakness, abdominal pain occurs frequently, heart and stomach pain radiating to the testicles, or loose stools, inability to smell odors, continuous clear and turbid nasal discharge, tearing from eyes, asthma, thirst, phlegm-cough, spitting out white foam, heavy and painful lower back, occasional pain in neck, back, and chest, sensation of fire flowing in eyes, aversion to cold in mouth and nose, occasional headache and dizziness. If chills persist, or cough or vomit, or retch or hiccup, then irritability and steaming heat occur, as if sitting in a steamer. One must remove clothes and stay in a cold place, or drink cold water to relieve it. Chills injuring Zheng Qi cause shortness of breath, chest fullness and pain, with a sensation of blockage in the diaphragm and throat nearly severing. Severely, eyes stare wide, sounds are heard externally, and tears, snot, phlegm, and saliva increase greatly. After the chills pass, irritability lasts only a moment. Chills return. If the face is white and lusterless, it is blood loss. Sadness, sorrow, and unhappiness, desolate and sad mood, forgetfulness or frequent sneezing, this is Wind-Heat severely damaging Water (Kidney), and the rebound of Dry Metal (Lung). If the six pulses are fine, wiry, and choppy, empty upon pressure, this is a Great Cold syndrome also damaging Essence and Qi. Use pungent, sweet, warm, and hot, slippery and moistening agents. Greatly purge the West (Metal) and North (Water) to achieve cure.
When Great Cold remains outside, tonification is indicated; acupuncture is ineffective, moxibustion is suitable. The *Huangdi Zhenjing* says: When Great Cold remains outside, tonify it. When it enters inward, drain it from the He-Sea points. Acupuncture is ineffective, moxibustion is suitable. When both Yin and Yang are deficient, Fire naturally takes command. Limbs are cold and very cold, depression below the knees, Cold exceeds the Lingsanjili (Zusanli) point below the knee. Where there is depression, moxibustion naturally applies. Knotted collaterals are firm and tight, treated by Fire (moxibustion). Regarding "Great Cold remains outside, tonify it," "outside" refers to the exterior. Great Cold is the Cold transformation of the North. Entering humans, it is said to be outside, on the surface of the skin and hair. Thus, it is said Cold excess causes floating pulse. This is Foot-Taiyang Bladder channel Cold Qi striking its own channel. Following the principle of similar things grouping together, the vertex, nape, and forehead suffer first. Treatment is at Fengfu (GV16) point, needle retention for a long time. It refers to Hand-Taiyang Small Intestine internal Heat, which should transmit Foot-Taiyang Bladder Cold channel. This channel again receives external guest pathogens. Moreover, skin and hair are the Yang portion, greatly fearing Yin Cold. The classic says: For Cold diseases, long needle retention constitutes Heat Tonification, tonifying internal Heat, tonifying Yang, causing Yin to melt and pathogens to retreat. Retaining and tonifying uses Fire to attack it. Another explanation for Cold entering inward, moxibustion is suitable. It refers to both Left and Right Cun pulses being weak. Not mentioning Chi pulses is because it speaks of Earth's Yin and Yang pulses being prosperous. If Wiry and Tight pulses float higher, pressing reveals Flooding and Large. Pulses are prosperous below, meaning underground Yin Fire is also hidden. Moreover, Heaven and Earth Yin and Yang are already insufficient externally, so Upper and Lower Yang are all hidden. Thus, it says depressions should be moxed, precisely referring to this. So the classic says: Jue (Cold reversal) at the feet, Zong Qi does not descend, blood in the vessels coagulates and stops; without Fire regulation, it cannot be taken.
Acupuncture has five sections. Huangdi asked Qibo: I hear acupuncture has five sections, what is this? Qibo replied: Indeed there are five. First is Zhen Ai (Shaking Dust), second is Fa Meng (Opening Ignorance), third is Qu Zhua (Removing Claws), fourth is Che Yi (Removing Clothes), fifth is Huo Huo (Resolving Confusion). Huangdi said: Sir speaks of five sections, I do not yet understand their meaning. Qibo said: Zhen Ai refers to puncturing extra-heavenly Yang pathogens. Yang is formless. Extra-heavenly refers to Yin disease in Yang, so draw Yin from Yang. Treatment is at Back-Shu points. If Zang-Fu diseases have tangible signs, such as skin, flesh, sinews, bones, and blood vessel diseases. If severed internally, it is said Yin disease in Yang, pathogens are in extra-heaven. The method takes points at the Back-Shu of the five Zang. Suppose skin disease is internally severed, treatment is in extra-heaven, needling Lung Shu is correct. Suppose blood vessels are internally severed and cannot circulate, it is said this is also Yin disease in Yang, the point is Heart Shu on the back. The other three Zang tangible diseases internally severed, according to this, all treat at Zang Shu. *Huangdi Zhenjing* says: Upper Heat Lower Cold, view the deficient collaterals depressed in the meridians, take them until Qi descends then stop. This is called guiding downward. Commentary says: The Sage says Upper Heat Lower Cold, referring to Spring, Summer, Autumn, Winter. Should descend from extra-heavenly Yang into the earth. It is that among the group of Yins in heaven, Fire blazes and assists the Six Yangs, unable to decline. First, rupture blood collaterals in the Six Yangs to bleed, causing Yin Qi to descend. Although force is slight, it can drag Six Yangs downward, Yin Blood naturally descends. This can be called the image of Old Yang transforming into Yin. So the classic says: Upper Heat Lower Cold, view the deficient pulse depressed in the meridians, take it. This is called guiding downward. But saying collaterals are all major diseases, using three-edged needle to rupture blood and remove Yin Heat. Heat is in the blood collaterals of Hand-Taiyang Small Intestine having Fire-Heat pathogens, causing this Old Yang unwilling to transform into Yin and descend. So first rupture Hand-Taiyang blood heat, allowing Three Yin to use their time to descend, performing Autumn's receiving way, entering earth to store. This is draining Double Yang in heaven, unwilling to transform and perform Yin Dao below.
In the spring of Wuchen year, I responded to the imperial summons to go to the capital. Vice Director of the Secretariat, Yang Gong Qingqing, over seventy years old, suffered from facial complexion dark red as if drunk, thick sticky phlegm, occasional dizziness as if in wind and clouds. One day at the Department Hall, this syndrome suddenly came, plus unclear vision, so he returned home. I diagnosed his two Cun pulses as Flooding and Large, Chi pulses as Wiry, Fine, and powerless. This was clearly Upper Heat Lower Cold. Wanted to treat with medicine, but cold-cooling herbs were not tolerated due to age and weak Qi. Remembered former master's discussion: treating Upper Heat is like birds nesting on high peaks, shoot and take it. Used three-edged needle at anterior vertex between eyebrows, pricked rapidly twenty-plus times, released about two he of purple-black blood. At that time felt head and eyes clear and beneficial, all bitterness gone, never recurred since.
Generally, people struck by Cold like yawning, have clear nasal discharge, those with fever and harmonious color sneeze well. Observing patients before pulse diagnosis, seeing dry mouth, clear nasal discharge, good sneezing and yawning, this person is struck by Cold. Those with diarrhea are so due to interior deficiency. Wanting to sneeze but unable, this person has abdominal pain. Generally Cold pulse is Sun (Deep) and Xian (Wiry), double Xian pulse is Cold. Xian pulse shape is like a drawn bowstring, immovable upon pressure. Pulse Shu (Rapid) and Xian should be purged. Its Cold pulse double Xian and Chi (Slow), Jian (Hard) below heart. Pulse Da (Large) and Jin (Tight), Yang contains Yin, can be purged. If Right Hand Cun Kou pulse is Xian, then hypochondriac urgency and pain, person feels rough and aversion to cold. Teacher says: Chi is Cold, Se is lack of Blood. Cun Kou pulse Wei (Micro) and Zhong Jin (Medium Tight) and Se, Jin is Cold, Wei is Deficiency, Se is Blood insufficiency. Thus knowing after sweating and then purging, Daluxiu Wan (Great Dew Exposure Pill) treats hundred diseases of Cold.
Shensheng Fuqi Tang (from Jisheng Bacuifang) treats abdominal Qi taking advantage of Winter Foot-Taiyang Cold Qi, Foot-Shaoyin Kidney Water prosperity. Son makes Mother Excess. Tears from hands, runny nose, drooling, blurred vision, tinnitus and deafness, head and mouth/nose possibly aversion to Wind-Cold, preferring sun, restless sleep, often feeling phlegm blocking diaphragm/throat, loss of taste, bilateral hypochondriac contraction urgency and pain, loose teeth unable to chew, genital sweating, cold anterior yin, walking unsteadily, difficulty rising and lying, palm cold wind-bi numbness, frequent urination more by day and night, yawning, shortness of breath panting, insufficient Qi, sudden incontinence, women leukorrhea, severe pain in vaginal opening radiating to heart, blackened complexion, men testicular pain radiating to heart/abdomen, face like ochre, poor appetite, irregular bowel/bladder movements, vexation Huoluan, rebellious Qi interior urgency, pale abdominal skin, residual gas after, unable to strain abdomen, or borborygmus, knee tendon tension, shoulder/hip severe pain. Also treats biting cheek/lip/tongue, stiff tongue root etc. miracles. Avoid meat soup. Eat meat does not assist Fire pathogens in collaterals. If no above symptoms, women leukorrhea, cold below umbilicus, men cold testicles, radiating heart/abdominal pain, palms cold, bilateral Chi pulses wiry/fine, non-pulsating upon pressure, urinary incontinence or frequent yawning, stool mostly dry/hard or soft, urine color changed, shortness of breath, cold forehead nose unable to smell, red/swollen nose tip good sneezing, much sadness/sorrow, mind unhappy, forgetful angry, night sweats fear wind, slippery frequent urination later incontinence, umbilical or nasal clear discharge, endless tears, spirit not clear, should take it. Not necessary all symptoms present. Generally Kidney and Bladder channels have Cold, Yuan Qi insufficient, all should take it. Take one dose every 3-5 days during waxing/waning moon. If disease urgent, any time, treat immediately. One formula has Tao Ren, no Gancao.
Chufengshi Qianghuo Tang (from Jisheng Bacuifang) Wei (Flaccidity) is caused by Damp-Heat invading Liver/Kidney. Must remove urgently, otherwise Lower Jiao Yuan Qi exhausted, becoming soft paralysis, lower waist unable to move, vexation unbearable. If body heaviness reduces, Qi not short, urine normal, and Damp-Heat season recedes, or increased pathogenic Qi recedes, do not use Wuweizi, Zexie, Fuling, Huangbai, Zhuling, Zhimu, Cangzhu, Baizhu. Only modify based on original disease syndrome. Regular medication must use wine-processed Huangbai 2-3 fen. If seasons change, Clear-Dry Qi prevails, add Pungent-Warm draining herbs. If Dampness wins, Wind syndrome does not retreat, dizziness/numbness continues.
Qianghuo (1 liang), Fangfeng (remove radicle 1 qian), Chaihu (5 fen), Gaoben (3 fen), Duhuo (5 fen), Fuling (2 fen), Zexie [2 (1) fen], Zhuling (peel 2 fen), Huangqi (1 qian), Zhi Gancao (5 fen), Jupi (3 fen), Huangbai (3 fen), Huanglian (1 fen), Cangzhu (water washed peel weigh 1 qian), Shengma (7 fen), Chuanxiong (3 fen). Cut up. Each dose weigh 3 qian or 5 qian, two zhan water, decoct to one zhan, filter dregs, take slightly warm. Use for Deficiency/Excess. If incomplete symptoms, use modification methods according to formula.
Fuzi Lizhong Tang (from Yifang Dacheng) treats Five Zang Zhong Han Zhong Shi (Mid-Cold Mid-Damp), rebellious throat Qi weakness, also treats locked jaw, limb stiffness, aphasia. Formerly a soldier at border, heavy snow went out tent to look, suddenly fainted. Lin Ji acting as accompanying official doctor,灌ed this medicine two doses then woke. Also treats Phlegm retention in epigastrium, Cold Qi stabbing pain.
Renshen, Baizhu, Ganjiang (processed), Shu Fuzi (each 1 liang), Gancao (stir-fried 5 qian). Cut up. Each dose 3 qian, 5 slices ginger.
Siwuwu Tang treats Cold-induced dizziness/aversion to cold, facial deviation, deafness.

Yangwei Tang (from Heji Fang) treats External Cold invasion, Internal Cold/Raw food injury, aversion to cold strong fever, head/eye dizziness/pain, limb stiffness. Treats both Wind-Cold syndromes. Cover with quilt first, continuously take this to cause slight sweat in four limbs. If no sweat, help with thin porridge. If already sweating, do not take. Also requires mild-warm harmonizing, or if residual Heat, use Shen Su Yin to adjust. Also treats Diet injuring Spleen causing Phlegm-Malaria, repels mountain miasma, four-season plagues.
Cangzhu (stir-fried), Houpo (processed), Banxiaqu (each 1 liang), Renshen, Fuling, Caoguo, Huoxiang (each half liang), Juhong (7.5 qian), Gancao (half liang). Cut into pieces, make powder. Jiangfu Tang (from Zhizhi Fang) treats Zhong Han locked jaw, limb stiffness, aphasia, or sudden fainting, spitting saliva, resembling silent Wind, cold extremities, or abdominal irritability. Also treats Yin Syndrome Cold Damage, spontaneous loose stool with fever, pathogen not resolved, no exterior signs.
Ganjiang, Shu Fuzi (each 2 liang). Cut up. Each dose 4 qian, 1.5 zhan water, decoct to 0.7 zhan, filter dregs, take warm. Or worry this medicine is too drying, then sequentially take Fuzi Lizhong Tang. Jiangfu originally treats Cold Damage after purging, then re-sweating, interior/exterior both deficient, body no great Heat, daytime irritability, nighttime quiet, no vomiting/thirst, six pulses deep/hidden, all should take this. Those not knowing pulse must examine carefully. Also treats Middle Jiao Deficient-Cold long-term accumulated Phlegm/Water, heart/abdominal pain, Huoluan tendon twitching, cold extremities. Enter Liver add Mugua, enter Lung add Sangbaipi, enter Spleen add Baizhu, enter Heart add Fuling, add according to syndrome. Accompanied by Wind add Fangfeng, combined with Damp add more Baizhu, tendon/spasm add Mugua, joint pain add Guixin, Renshen, Baizhu equal parts. One formula has Gancao, use fresh ginger decoct.
Baizhu Tiaozhong Tang (from Xuanming Lun) treats Mid-Cold blooming/stuffiness urgent pain, Cold-Damp combat vomiting/diarrhea abdominal pain, upper/lower discharged fluids clear/cold, undigested grain, clear white urine not astringent, body cool not thirsty, originally/end did not see Yang Heat signs, its pulse Slow is correct. This is due to eating too many cold foods, Yin prevails Yang declines becoming Mid-Cold. Or Cold/Heat strike each other, reversing Yang Qi oppression unable to disperse, oppressive Heat internally generated becoming Heat syndrome, cannot also say it is Cold, must distinguish by pulse/syndrome. Damp-Heat vomiting/diarrhea commonly see Yang pulse, if fluid loss Qi deficiency, can also reverse see all Yin pulses. Must clarify root/branch, do not treat recklessly. Or Heat syndrome mistakenly takes this Baizhu Tiaozhong Tang, warm medicines can also develop Yang Qi, disperse/communicate and heal, no other harm. Cold/Heat long/new all suitable take okay. Or have mouth ulcers eye diseases, pregnant women etc. vomiting/diarrhea, use baked Ganjiang, Guigui not suitable take.
