Regarding jaundice in cold damage, it is generally caused by the interaction of dampness and heat, leading to what was historically called "Dan" (jaundice). "Dan" means yellow; it is a condition of excess Yang without Yin. Jaundice in cold damage indicates severe illness. Dampness and heat, when severe, cause jaundice. If internal heat is already intense and further aggravated by fire therapy, jaundice also results. Wind-pathogen combined with fire-heat, two Yangs scorching each other, inevitably causes jaundice. In Yangming syndrome, if fire therapy is applied causing sweating only on the forehead and urinary difficulty, jaundice will definitely occur. This is due to internal heat exacerbated by fire therapy. In Yangming syndrome, if there is no sweating, urinary difficulty, and mental restlessness (Ao Nao), jaundice will definitely occur. This is caused by excessive heat in Yangming.
After sweating treatment in cold damage, if the body and eyes turn yellow, it is because cold-dampness remains unresolved internally. Purging should not be used; instead, treatment should focus on resolving cold-dampness. This type of jaundice is caused by cold-dampness. Both dampness and heat can cause yellowing; can one distinguish them? Their root causes differ, and their colors are distinct. Jaundice from dampness appears like smoked yellow, dull and dark. Jaundice from excessive heat appears bright orange-yellow; severe cases may stain clothing, resembling the yellow of cypress bark. Thus, dampness and heat are clearly different. Generally, jaundice belongs to the Taiyin channel, which is the spleen meridian. The spleen governs yellow, corresponding to the color of earth. When the spleen channel is steamed by damp-heat, the yellow color manifests externally. The classics state: "In cold damage, if the pulse is floating and slow, and the hands and feet are warm, the disease is related to Taiyin, and Taiyin disease typically presents with jaundice." Heat must be internally excessive. If there is spontaneous sweating and free urination, jaundice will not occur. It must be sweating only on the head, no sweating on the body, stopping at the neck, urinary difficulty, thirst for drinks—this is stagnant heat in the interior, leading to jaundice.
Jaundice is characterized by excessive heat, but treatments vary. In cold damage lasting eight or nine days, if the body is orange-yellow, with urinary difficulty and lower abdominal fullness, Yinchin Hao Tang (Artemisiae Scopariae Decoction) is indicated to drain and purge heat. If the body is yellow with fever, Zhizi Bopi Tang (Gardeniae Corticis Phellodendri Decoction) is indicated to dissipate heat. If stagnant heat is in the interior causing jaundice, Mahuang Lianqiao Chixiaodou Tang (Ephedra Forsythiae Red Bean Decoction) is indicated to dissipate heat. These methods of draining and dissipating differ but all aim to clear heat. If the body is yellow, pulse is deep and knotted, lower abdomen is hard, urination is normal, and the patient is manic-like, this is blood stasis in the lower jiao requiring Didang Tang (Efficacious Pill) to purge. Since jaundice represents extreme pathology, many cases are incurable. Not only is weak pulse at Cun Kou and cold breath signs of incurability, but also a smoky appearance of the body, staring eyes, and shaking head indicate heart qi exhaustion; blackening around the mouth and soft sweat with jaundice indicate spleen qi exhaustion. These are all terminal conditions requiring careful observation by physicians.
Furthermore, if jaundice patients develop fever at dusk and feel aversion to cold, it is female labor jaundice (Nü Lao Dan). Symptoms include bladder urgency, lower abdominal fullness, generalized yellowing, black forehead, and hot soles, leading to black jaundice. Abdominal distension suggests impending ascites; stools are black and loose. This is female labor jaundice, not ascites. Abdominal fullness makes it difficult to treat.
The appearance of jaundice in cold damage involves total body discoloration, either like smoked yellow or orange. Any case of Yangming wind-stroke or Taiyin dampness can cause jaundice. Generally, lack of sweating and urinary difficulty prevent the dispersion of cold-dampness, leading to heat accumulation in the spleen and stomach, closing the pores. Stagnant heat clashes with retained food. If the lower abdomen is full and urgent, eyes are sore, nasal bone hurts, shoulders and neck are stiff, and back is tight, these are the symptoms. Once urination becomes smooth, recovery gradually ensues. Avoid excessive bowel movements, as this causes chest fullness. Acute jaundice (Ji Huang) presents with severe yellowing, sudden onset, chest fullness, asthma, and imminent death. Some present with yellowing immediately upon illness onset, or only after death. These are caused by spleen-stomach stagnant heat and epidemic pathogens. Careful differentiation is needed. Fever with palpitations indicates acute jaundice.
Verse states: Normal urination with black stool confirms blood syndrome leading to mania. Complete urinary difficulty indicates Dan disease (jaundice).
For blood syndrome, use Didang Tang. For mild cases, Tao Ren Cheng Qi Tang or Xi Jiao Di Huang Tang. For Dan jaundice, use Yinchin Tang or Yinchin Wuling San.
Another verse: Taiyin fever with body pain and yellow eyes/face indicates dampness. Abdominal fullness, string-taut pulse, and hypochondriac pain indicate Yangming wind-stroke. Zhang Zhongjing said: After sweating in cold damage, if eyes and body turn yellow, it is due to unresolved cold-dampness internally. Do not purge; seek resolution within cold-dampness. Fever, body pain, and yellowing indicate Taiyin dampness. Abdominal fullness, dry nose, hypochondriac pain, tidal fever, and yellowing indicate Yangming wind-stroke.
To prevent jaundice, quickly insuffuse about one character amount of Goudi powder (Melonis Fructus Seed Powder) into the nose. Refer to the headache dampness section. Generally, for jaundice and dampness, failing to promote urination is incorrect treatment. Abdominal fullness, floating-stringy pulse, cough, tidal fever, difficult urination, hypochondriac pain, shortness of breath, dry nose, inability to sweat, and lethargy indicate Yangming wind-stroke. Da Xiao Chai Hu Jia Fuling Tang (Major and Minor Bupleurum Decoction with Poria) treats this. Add Poria if hiccups occur. See the difficult urination section. If cold-dampness remains unresolved, heat accumulates in spleen/stomach, pores close, and stagnant heat clashes with food, causing steaming and yellowing. This is common in southern summer dampness. Ancient texts equate Dan with yellow. Damp-heat interaction causes many cases of Dan, sometimes swelling before yellowing. This resembles blood stasis externally and in pulse, but urinary difficulty distinguishes jaundice from blood stasis (normal urination). Jaundice patients have spleen heat accumulation, fever, thirst, and floating-slippery-rapid pulse. Blood stasis patients are manic with normal stools. Jaundice patients have fever, head/face sweating, no body sweating up to the neck, thirst, and urinary difficulty, leading to jaundice. Use Yinchin Hao Tang or Wuling San plus Yinchin. Mix Yinchin Hao Tang (10 parts) and Wuling San (5 parts). Take 2 qian with warm water, three times daily. After medication, if urine becomes red like soap pod juice and abdomen reduces overnight, yellow exits via urine. Ancients said: Treating dampness without promoting urination is incorrect. Jaundice is stagnant heat in the interior caused by urinary difficulty. Zhizi Bopi Tang and Mahuang Lianqiao Chixiaodou Tang are options. Another method: For impending jaundice, insuffuse Goudi powder into the nose with water to extract yellow fluid, then take Yinchin Hao Tang mixed with Wuling San.
Question: Baihu Tang syndrome also has fever, thirst, and urinary difficulty, why no jaundice? Answer: Baihu Tang syndrome has full-body sweating (heat disperses). Jaundice has no sweating below the head.
Question: Taiyin syndrome with body pain, fever, and smoked-yellow appearance? Answer: Taiyin dampness. Zhang Zhongjing said: Post-sweating yellowing is due to unresolved cold-dampness; do not purge, treat cold-dampness.
Question: String-floating-large pulse, shortness of breath, abdominal fullness, hypochondriac pain, dry nose, inability to sleep, generalized yellowing, difficult urination, tidal fever, cough? Answer: Shaoyang wind-stroke. Treat with Xiaochaihu Tang (Minor Bupleurum Decoction).
