凡病眼不痛不痒。端然渐渐昏暗。遂至失明。眼状虽如寻常。瞳仁中潜生障翳。作青白
色。渐不辨人物。微见三光。名曰内障也。多从一眼先患。久后相牵。俱成此状。若不预服
汤丸。
黑水凝结而作障也。若忽然而暗。经三五十日翳成者。则是脑脂流下所致。若三五十日之内。
渐渐
之见功。唯金针拨之乃效。至内障眼凝滞数重。异象多般。有沉有浮。或滑或涩。或形如皓
雪。或状似清水。或散若梨花。或分如片月。风撮明窍。热攻翳开。缺角无垣。斜 有异。
或
异同。穷论莫尽。其中亦有不可治者。初患之时。脑痛眼疼。又有虽不疼痛。画之不动者。
名曰死翳。其翳作黄赤色。不可治也。又有翳状。或作破散。或深或浅。中心垂布浓浓者。
亦不可治也。凡学针药。必须审细辨其证候。明其浅深。脱或不晓病原。未达机要。妄行针
药
圆翳内障。此眼初患之时。眼前多见蝇飞花发。初如薄烟轻雾。渐渐加重。不痛不痒。
端然渐渐失明。始与不患眼相似。不辨人物。惟睹三光。患者不觉。先从一眼先患。向后相
牵俱损。此脑脂流下。肝热冲上。致翳青白。瞳仁端正。阳看则小。阴看则大。其眼须针。
然后服药补治。用防风散羚羊角饮子。
歌曰。翳中最好是团圆。一点犹如水上盘。阳里看时应自小。阴中见则又还宽。金针一
拨蝇飞去。朗日舒光五月天。不是医人夸巧妙。谁提远地密相传。
冰翳内障。此眼初患之时。头旋额角偏痛。连眼睑骨及鼻颊骨痛。眼内赤涩有花。或黑
或白或红。皆因肝脏积热。肺受风劳。或心烦或吐。大肠秘结。夜见灯光如蜂。初患之时。
宜令针治诸穴脉。忌督脉处出血过多。恐加昏重。宜令服食还睛丸。
歌曰。冰翳犹如水冻坚。阴中阳里一般般。傍观瞳仁透表白。针下分明是谎言。来往用
针三五拨。志心服药必能痊。若遇庸医强拨下。瞳仁清净不能观。
滑翳内障。此眼初患之时。不痒不痛。还从一眼先患。后乃相牵俱损。端然渐渐失明。
皆因脑脂流下。肝风冲上。瞳仁内有翳。如水银珠子。不辨人物。遂使然也。宜令金针拨之。

散翳内障。此眼初患之时。不痒不痛。渐渐失明。还从一眼先患。惟瞳仁里有障翳。作
青作白。不辨人物。犹见三光。此眼宜令金针拨之。然后宜服还睛散、补肝汤治之即瘥。
歌曰。散翳薄薄又何为。状形同酥点容仪。一随针了和涎散。未得分明自觉知。封里安
存须善巧。莫令患者致狐疑。殷勤遣服还睛散。再睹三光百日期。忌慎一如僧戒行。不须恣
意纵贪痴。深言向说何为切。记取冥冥黑暗时。
浮翳内障。此眼初患之时。都无痒痛。还从一眼先患。后乃相牵俱损。此状皆因热风冲
入脑中。脑脂流下。凝结作翳。如银钉色。虽不辨人物。犹见三光。宜用金针拨之。然后宜
服决明散、坠翳丸立效。歌曰。浮翳正观如透外。乍看白色似银钉。阴宽阳小随开合。此则
深知是本形。辨认既能无
沉翳内障眼。此眼初患之时。肝脏劳热。还从一眼先患。或见黑花。后即相牵俱损。脑
中热气流下。要辨三光。宜令金针拨之。然后服羚羊角饮子、空青丸瘥。
歌曰。一般内翳又浮沉。隐隐藏形黑水深。向日细看方得见。自古相传不是今。此障拨
时须远穴。劝君莫用短头针。坠翳强过五十息。只图牢固没他心。
横翳内障。此眼初患之时。还从一眼先患。皆是五脏虚劳。毒风冲上。脑脂下流。令眼
失明。犹辨三光。宜用金针拨之。宜服还睛丸、七宝散瘥。
歌曰。虽然稀有横关翳。学人韬钤要得知。细观横心如剑脊。上头下畔薄微微。开时先
向中心拨。随后还应若雾披。改往修来何所作。一生龙树愿归根据。
偃月翳内障。此眼初患之时。微有头旋。额角痛。亦因肝肾热生翳。脑风积热。致使生
翳。如偃月之状。还用金针拨之。然后宜服通明散、坠翳丸立效。
歌曰。眼中一种脑脂凝。何得偏称偃月名。一畔浓而一畔薄。医工不晓即疑生。欲知巧
妙行医法。浓处先宜拨便明。丸散还睛应遣服。坚牢百岁得安宁。
枣花翳内障。此眼初患之时。微有头旋眼涩。渐渐昏暗。眼中时时痒痛。脑热有花。黄
黑不定。此状宜令针治诸穴。然后宜服还睛散、坠翳丸立效。
歌曰。翳中何名是枣花。周回锯齿没诸他。拨时从上轻轻拨。状似流星落远霞。细意便
看瞳子内。莫留断却作烂遮。根据前不断还睛药。百岁光明睹物华。
白翳黄心内障。此眼初患之时。肝脏劳热。皆从一眼先患。后乃相牵俱损。初觉急治即
瘥。先须凭服汤药丸散将息。即宜针刺诸穴脉后。更宜金针轻拨之。然后服坠翳散即效。
歌曰。可怜白翳更黄心。患者商量惧用针。来往用针三五拨。不随针落药能沉。还睛方
术须通秘。百日如风卷雾阴。期若丁宁须向说。窥看神效值千金。
黑水凝翳内障。此眼初患之时。不痒不痛。微有头旋。眼涩见花。黄黑不定。瞳仁微大。

歌曰。黑翳水结凝青色。可怜内障没真容。阴阳开处难开聚。如觉风 在胆中。还用金
针三五拨。药凭芦荟作神功。期程百日丁宁说。玉兔中秋照远空。
惊振内障。此眼初患之时。忽因五脏虚劳受疾。亦由肝气不足。热毒冲于脑中。即或因
打筑着脑。恶血流下。渐入眼内。后经三五年间。变成白翳。一如内障形状。不宜针拨先患
之眼。更一只牵损之眼。却待翳成。根据法针之立效。然后服镇肝丸、还睛散即瘥。
歌曰。忽然撞振不全伤。疼痛微微目不长。便即脑脂为白色。一如内障睹三光。不须错
误将针拨。却恐为灾难可当。在后若牵非损者。医元如法始开张。
混睛外障。此眼初患之时。先痛后痒。碜涩泪出。怕日羞明。白睛先赤。发歇作时无定
准。
使然也。初患时。宜令镰洗钩割翳膜。熨烙除去根本。然后宜服凉肝散。点七宝膏。服退翳
丸。立效。
歌曰。白睛先赤作根基。痛痒风吹泪出眵。碜涩难开旬日内。发无定准有瘳时。年深渐
变睛为碧。满目凝脂始觉之。赤脉如丝横与竖。混睛外障莫狐疑。毒风赤血成其晕。如此谁
言易可医。冷涩药中须得妙。点摩翳膜尽为期。频镰双睑铜篦烙。风热平时即住之。汤药稍
和年岁服。要除根本莫嫌迟。
内障眼根源。歌曰。不痒不痛渐昏蒙。薄雾轻烟渐渐浓。或见蝇飞花乱出。或如悬喜在
虚
昏暗。次第相牵与一同。苦口何须陈逆耳。只缘肝气不相通。此时服药期销定。将息多乖即
没功。日久既应全暗黑。特名内障障双瞳。名字随形分十六。龙师圣旨会推穷。灵药这时难
得效。金针一拨日当空。强将修息依言说。莫遣根据前病复同。
凡内障之眼。形候甚多。好恶非一。有冰。有涩。有滑。有散。冰者拨之不下。滑者闲
之不
轻重粗细。则量宜浅宜深。宜迟宜疾。患人或冷或热。或实或虚。若热多者。宜先服凉药。
令热毒稍除。然后开之。不尔恐气开吐逆。若有风虚者。先宜服祛风镇心之药。候四体平和。
方
老。性冷者。其翳难老。老障者可用小针。嫩薄者可用大针。障浮者。去乌珠近下针之。障
沉者。须远下针。翳若沉。下针近拨之。则其翳不牢。翳若浮。下针远拨之。则其翳多破。
若妇人有妊娠。及新产后有斯疾者。未宜下针。直候体力安平。方可开之。不尔则必有所损。
凡开内障及诸翳膜息肉。必须候天气晴明无风。仍于静处断除喧乱。安心定意。方可行针
随眼左右。宜向小 头下针。隔鼻开眼者。鼻碍于手。下针不妙。令患人正面坐。手捉医人
腰带。勿令放手。先将锐针拄穴令定便得。眼睛勿令转动。定呼吸气五十息。徐徐进针。勿
令过重。亦不可全轻。初且须轻轻抹入。即须稍重。针头若偏或有伤损。血则随针出。即不
可止。亦不得重手按之。恐血更多。可轻轻 之。又须缓气徐徐用力逼之。血即自止。若血
坚急者。则是入膜。若放平犹滑。及未得全入。若已入了。其眼觉痛。若痛且住歇少时。渐
渐进之。临欲过膜。痛即更甚。方便用意针过。待痛稍定。即可倒针。向瞳仁齐平。拨之
向下。不得用重手也。离瞳仁微近。开眼便见物。既见物。须合眼。缓缓抽针出了。停五十
息。久开得明朗见物。即以绵封之。根据法将息。勿令失度。稍失其宜。即翳晕却上。准前更
开亦得。若拨后有动静。随状止之。若有痛处。以手随处掐之即定。若大痛不定。即以火熨
之。凡欲下针。预向人说勿恐。下手疾人惊恶呕吐。亦须药止痛。以大黄木香等为末。以醋
浆水和如泥。作饼子搦之即定。或吐不定。含白梅咽津。仍预先含之。吐逆盛即难止。凡诸
药须预备拟。不可临时阙也。痛久不可忍。即见损也。开眼后绵封七日。吃豉粥。仰卧。不
得转动侧卧。常须人看。不可离人。勿高声叫唤。大小便缓缓扶起。勿令患人用力。及不得
洗面迎风。将息七日后开封。若见物犹白色。或如霜雪。盖是眼嫩故也。亦未可全除封。看
物即可时时一开。若看物甚即睛疼。必有所损。二七日后方可除封。有物状如衣带。飞虫悬
针之动。水轮未定。吃药渐渐自已。三七日外。眼或痒无虑也。凡开时。患人不得太饱。亦
无令饥也。既开见物。或有痛处。随左右针之。及掐捻左右督脉。颞 风府等穴。若针痕痛。
二三日即自定也。一月内不用洗面。恐水入针孔。即有所损也。宜以绵渍盐汤。微微拭
。七日内不得吃饭。恐入水动牙关。应着水轮。故须吃粥。及软烂之物。夫治眼不论障翳及
诸杂状候之眼。皆不得当风看日。及喜怒房事。五辛酒面炙爆毒物。并宜断之。惟须宽缓
性情。慎护调摄。即无不瘥也。若纵恣乖违。触犯禁忌。则自贻其咎矣。
龙木论云。针内障眼法。歌曰。内障由来十六般。医者还须仔细看。分明一一知形状。
下针方可得安然。若将针法同圆翳。误损神光取瘥难。冷热先用虚与实。调和四体待平安。
不然气闷违将息。呕逆劳神翳却翻。咳嗽震头皆未得。多惊先服镇心丸。若求凉药银膏等。
用意临时体后看。老翳细针粗拨嫩。针形不可一般般。病虚新产还娠月。下手应知将息难。
不雨不风兼吉日。清斋三日在针前。安心定意行医道。念佛亲姻莫杂喧。患者向明盘膝坐。

忽然惊振翳重下。服药三旬见朗然。七日解封须见物。花生水动莫他言。还睛丸散坚心服。
百日分明复茜根。
针内障眼后法。歌曰。内障金针针了时。医师言语要深知。绵包黑豆如球子。眼上安排
吊系之。头安豆枕须安稳。仰卧三朝莫厌迟。卧后忽然微有痛。脑风牵动莫他疑。或针或烙
根据经法。痛极仍将火熨之。备拟白梅含咽汁。吐来仰卧却从伊芳。起则恐因遭努损。虽然希有
也须知。七朝豉粥温温服。震着牙关事不宜。大小便时须缓缓。无令自起与扶持。高声叫唤
多惊动。清嫩睛轮见雪飞。如此忌须三十日。渐行出外认亲知。狂心莫忆阴阳事。夫妇分床
百日期。一月不须临洗面。针痕须防湿微微。五辛酒面周年断。服药平除病本基。
绿风内障。此眼初患之时。头旋。额角偏痛。连眼睑骨。及鼻颊骨痛。眼内痛涩见花。
或因恶心痛甚欲吐。或因吐逆后。便令一眼先患。后相牵俱损。目前花生。或红或黑。为肝
肺受劳。致令然也。切宜服羚羊角饮子、还睛丸。兼针治诸穴。眉骨血脉。令住疾势也。
歌曰。初患头旋偏痛疼。额角相牵是绿风。眼眶连鼻时时痛。闷涩生花黑白红。肝脏只因先
患左。肺家右眼作先锋。续后相牵俱总患。缘他脉带气相通。风劳入肺肝家壅。客热潜流

须当服。还睛丸散成其功。频针眉骨兼诸穴。能令病本住行方。忌针督脉多出血。恐因此后
转昏朦。瞳子开张三耀绝。妙药名医更谩逢。
乌风内障。此眼初患之时。不疼不痛。渐渐昏沉。如不患眼人相似。先从一眼起。后乃
相牵俱损。瞳子端然不开。不大惟小。不睹三光。此是脏气不和。光明到退。眼带障闭。经
三五年内。昏气结成翳。如白青色。不辨人物。以后相牵俱损。瞳仁微小。针之无效。惟宜
服药补治五脏。令住病势。宜服决明丸、补肝汤立效。
歌曰。都无痛痒不头痛。渐渐昏朦似物瞒。没翳恰如昏不患。乌风根本更何言。有花脏
腑虚劳是。无即肝家气拥填。两种既知虚与实。分明用药补和先。和时宜服凉药饮。空腹宜
吞磁石丸。食后补肝须早治。瞳仁未小即能痊。阳衰年老还相似。医者搜索仔细看。若绝三

凡是眼睛不痛不痒，端正地逐渐变得昏暗，最终导致失明。眼部外观虽然如同平常，但瞳孔中暗中生出障翳，呈现青白色，逐渐无法辨认人物，只能隐约看到三光（日、月、星），这称为内障病。这种病大多先从一只眼睛患病，时间久了牵连另一只眼，两只眼都变成这种状况。如果不预先服用汤药或丸药，黑水凝结就会形成障翳。如果是突然变暗，经过三五十天障翳形成的，这是脑脂流下所致。如果经过三五十天，渐渐显现功效的，只有用金针拨法才有效。至于内障眼中凝滞数重，形态各异，有的沉有的浮，有的滑有的涩，有的形状像白雪，有的状似清水，有的散开像梨花，有的分开像半月，风撮动明窍，热攻使翳张开，缺角无垣，斜视有异象，种种不同，穷尽论述也说不完。其中也有不可治疗的。初患时，脑痛眼疼；又有虽然不疼痛，但画线不动的，称为死翳。其翳呈黄赤色，不可治疗。又有翳的形状，有的破碎散乱，有的深有的浅，中心垂布浓密的，也不可治疗。凡学习针药，必须仔细辨别证候，明了深浅。如果不晓病因，未达机要，妄行针药，就会出错。
圆翳内障：此眼初患时，眼前多见蝇飞花发，起初如薄烟轻雾，渐渐加重，不痛不痒，端正地逐渐失明，开始与未患病的眼睛相似，无法辨认人物，只能看见三光。患者不觉察，先从一只眼睛患病，后来牵连另一只眼都受损。这是脑脂流下，肝热冲上，导致翳呈青白色，瞳仁端正，阳看则小，阴看则大。此眼须先针刺，然后服药补治。用防风散羚羊角饮子。

凡开内障及诸翳膜息肉，必须候天气晴明无风，仍于静处断除喧乱，安心定意，方可行针。随眼左右，宜向小...头下针。隔鼻开眼者，鼻碍于手，下针不妙。令患人正面坐，手捉医人腰带，勿令放手。先将锐针拄穴令定便得。眼睛勿令转动，定呼吸气五十息，徐徐进针，勿令过重，亦不可全轻。初且须轻轻抹入，即须稍重。针头若偏或有伤损，血则随针出，即不可止，亦不得重手按之，恐血更多，可轻轻...之。又须缓气徐徐用力逼之，血即自止。若血坚急者，则是入膜。若放平犹滑，及未得全入。若已入了，其眼觉痛，若痛且住歇少时，渐渐进之。临欲过膜，痛即更甚，方便用意针过。待痛稍定，即可倒针，向瞳仁齐平，拨之向下，不得用重手也。离瞳仁微近，开眼便见物。既见物，须合眼，缓缓抽针出了，停五十息，久开得明朗见物，即以绵封之。根据法将息，勿令失度。稍失其宜，即翳晕却上，准前更开亦得。若拨后有动静，随状止之。若有痛处，以手随处掐之即定。若大痛不定，即以火熨之。
凡欲下针，预向人说勿恐，下手疾人惊恶呕吐，亦须药止痛。以大黄木香等为末，以醋浆水和如泥，作饼子搦之即定。或吐不定，含白梅咽津，仍预先含之，吐逆盛即难止。凡诸药须预备拟，不可临时阙也。痛久不可忍，即见损也。
开眼后绵封七日，吃豉粥，仰卧，不得转动侧卧，常须人看，不可离人，勿高声叫唤，大小便缓缓扶起，勿令患人用力，及不得洗面迎风。将息七日后开封，若见物犹白色，或如霜雪，盖是眼嫩故也。亦未可全除封，看物即可时时一开。若看物甚即睛疼，必有所损。二七日后方可除封。有物状如衣带，飞虫悬针之动，水轮未定，吃药渐渐自已。三七日外，眼或痒无虑也。
凡开时，患人不得太饱，亦无令饥也。既开见物，或有痛处，随左右针之，及掐捻左右督脉、颞...、风府等穴。若针痕痛，二三日即自定也。一月内不用洗面，恐水入针孔，即有所损也。宜以绵渍盐汤，微微拭...。七日内不得吃饭，恐入水动牙关，应着水轮，故须吃粥，及软烂之物。夫治眼不论障翳及诸杂状候之眼，皆不得当风看日，及喜怒房事，五辛酒面炙爆毒物，并宜断之。惟须宽缓性情，慎护调摄，即无不瘥也。若纵恣乖违，触犯禁忌，则自贻其咎矣。
龙木论云：针内障眼法。歌曰：内障由来十六般，医者还须仔细看。分明一一知形状，下针方可得安然。若将针法同圆翳，误损神光取瘥难。冷热先用虚与实，调和四体待平安。不然气闷违将息，呕逆劳神翳却翻。咳嗽震头皆未得，多惊先服镇心丸。若求凉药银膏等，用意临时体后看。老翳细针粗拨嫩，针形不可一般般。病虚新产还娠月，下手应知将息难。不雨不风兼吉日，清斋三日在针前。安心定意行医道，念佛亲姻莫杂喧。患者向明盘膝坐，提师腰...忽然惊振翳重下，服药三旬见朗然。七日解封须见物，花生水动莫他言。还睛丸散坚心服，百日分明复茜根。
针内障眼后法。歌曰：内障金针针了时，医师言语要深知。绵包黑豆如球子，眼上安排吊系之。头安豆枕须安稳，仰卧三朝莫厌迟。卧后忽然微有痛，脑风牵动莫他疑。或针或烙根据经法，痛极仍将火熨之。备拟白梅含咽汁，吐来仰卧却从伊芳。起则恐因遭努损，虽然希有也须知。七朝豉粥温温服，震着牙关事不宜。大小便时须缓缓，无令自起与扶持。高声叫唤多惊动，清嫩睛轮见雪飞。如此忌须三十日，渐行出外认亲知。狂心莫忆阴阳事，夫妇分床百日期。一月不须临洗面，针痕须防湿微微。五辛酒面周年断，服药平除病本基。
绿风内障：此眼初患时，头旋，额角偏痛，连眼睑骨，及鼻颊骨痛，眼内痛涩见花，或因恶心痛甚欲吐，或因吐逆后，便令一眼先患，后相牵俱损，目前花生，或红或黑，为肝胆受劳，致令然也。切宜服羚羊角饮子、还睛丸，兼针治诸穴，眉骨血脉，令住疾势也。
歌曰：初患头旋偏痛疼，额角相牵是绿风。眼眶连鼻时时痛，闷涩生花黑白红。肝脏只因先患左，肺家右眼作先锋。续后相牵俱总患，缘他脉带气相通。风劳入肺肝家壅，客热潜流到...须当服，还睛丸散成其功。频针眉骨兼诸穴，能令病本住行方。忌针督脉多出血，恐因此后转昏朦。瞳子开张三耀绝，妙药名医更谩逢。

Generally, when the eyes are neither painful nor itchy, and gradually become dim until blindness occurs, while the external appearance of the eye remains normal, a cataract (obscuration) secretly forms within the pupil, presenting as blue-white. As it progresses, one can no longer distinguish people or objects, seeing only the "three lights" (sun, moon, stars). This is called *Nei Zhang* (Internal Cataract). It often starts in one eye and, over time, affects the other, resulting in bilateral involvement. If decoctions or pills are not taken prophylactically, the "black water" (vitreous humor/ocular fluids) coagulates to form the opacity. If darkness appears suddenly and the opacity forms after 30-50 days, it is caused by the downward flow of "brain fat" (aqueous/vitreous degeneration). If efficacy is seen gradually after 30-50 days, only the Gold Needle Extraction (*Jin Zhen Bo*) method is effective. In cases where internal opacities are layered, with various forms—some sinking, some floating; some slippery, some rough; resembling white snow, clear water, pear blossoms, or crescent moons—the wind agitates the clear apertures, heat attacks causing the opacity to open, creating irregular shapes and oblique views. These variations are endless to describe. Some cases are incurable. At onset, there may be brain pain and eye pain; if there is no pain but the pupil does not move when traced, it is called *Si Yi* (Dead Opacity), which is yellow-red and incurable. Other opacities may be fragmented, deep or shallow, with dense central accumulation, also incurable. Practitioners must carefully differentiate syndromes and determine depth. Ignoring etiology and key mechanisms and practicing needle and drug therapy recklessly leads to error.
**Yuan Yi Nei Zhang (Round Opacity Internal Cataract):** At onset, patients see flying flies and blooming flowers before their eyes, initially like thin smoke or light fog, gradually worsening without pain or itch. Blindness develops gradually, starting similar to a healthy eye, unable to distinguish figures, seeing only the three lights. Patients are unaware initially. It starts in one eye and spreads to the other. This is caused by the downward flow of brain fat and upward rushing of liver heat, causing blue-white opacity with a regular pupil. The pupil constricts in bright light and dilates in dim light. Treatment requires needle extraction first, followed by herbal supplementation using *Fang Feng San Ling Yang Jiao Yin Zi* (Saposhnikoviae Decoction with Antelope Horn).
*Verse:* Best is the round shape of the opacity, like a disc on water. Small in yang/bright view, wide in yin/dim view. One拨 with gold needle drives flies away, revealing sunlight like May sky. Not boasting skill, but secret transmission from afar.
**Bing Yi Nei Zhang (Ice Opacity Internal Cataract):** At onset, dizziness and temporal headache extend to eyelid bones and nasal-cheek bones. Eyes are red, dry, with visual spots (black, white, or red). Caused by accumulated liver heat, lung affected by wind-labor, irritability, vomiting, constipation, and seeing lights like bees at night. **Hua Yi Nei Zhang (Slippery Opacity Internal Cataract):** At onset, no pain or itch, starting in one eye and spreading. Gradual blindness due to downward brain fat flow and upward liver wind. Opacity in pupil resembles quicksilver beads, obscuring vision. Treat with Gold Needle Extraction.
*Verse:* Focus mind when viewing slippery opacity, slight yellow amidst white. Opens fast then closes, spinning like quicksilver beads. Needle拨 follows hand, spirit returns to source after withdrawal. Penetrating clouds, golden crow shines in May sky.
**Se Yi Nei Zhang (Rough Opacity Internal Cataract):** At onset, vision hazy like thin smoke/fog, gradual blindness, starting in one eye and spreading.
*Verse:* Rough opacity gathers slowly, slight changes in yin/yang size. Peripheral pupil looks like congealed fat, trust ancient words. Follow standard method for撥, three to five times withdraw. Seal firmly for seven days for round wound healing, rest properly without deception.
**San Yi Nei Zhang (Scattered Opacity Internal Cataract):** At onset, no pain or itch, gradual blindness, starting in one eye. Opacity in pupil is blue-white, obscuring figures, seeing only three lights. **Heng Yi Nei Zhang (Horizontal Opacity Internal Cataract):** At onset, starting in one eye. Due to deficiency-labor of five zang organs, toxic wind rushing up, brain fat flowing down, causing blindness while retaining three-light vision. **Hei Shui Ning Yi Nei Zhang (Black Water Coagulated Opacity Internal Cataract):** At onset, no pain/itch, slight dizziness, eye dryness with spots, yellow/black uncertain, pupil slightly large, opacity...
*Verse:* Black opacity water coagulates green-blue, pitiful internal cataract hides true face. Hard to gather in yin/yang opening, feel wind in gallbladder. **Jing Zhen Nei Zhang (Shock/Vibration Internal Cataract):** At onset, sudden disease from five zang deficiency-labor, insufficient liver qi, heat-toxin rushing to brain, or head trauma causing bad blood to flow down into eyes. *Verse:* Sudden shock injury incomplete, slight pain eyes not long. Brain fat becomes white, like internal cataract seeing three lights. Do not mistakenly needle, fear disaster. Later if connected but not damaged, doctor starts correctly.
**Hun Jing Wai Zhang (Confused Cornea External Obstruction):** At onset, pain then itch, gritty tearing, photophobia, white sclera red first, intermittent unpredictable. Initial treatment: scythe wash, hook cut opacity membrane, cauterize to remove root, then *Liang Gan San* (Cool Liver Powder), apply *Qi Bao Gao* (Seven Treasures Ointment), take *Tui Yi Wan* (Remove Opacity Pill) for immediate effect.
*Verse:* White sclera red as foundation, pain/itch wind tears discharge. Gritty hard to open within ten days, unpredictable remission. Years later eye turns green, full of congealed fat noticed. Red veins like silk horizontal/vertical, confused cornea external obstruction no doubt. Toxic wind red blood forms halo, who says easy to cure? Cold/astringent medicines must be excellent, point/massage opacity membrane until end. Frequent scythe double eyelids copper comb cauterize, stop when wind-heat subsides. **Roots of Internal Cataract:** Verse: No pain/itch gradual dimness, thin fog light smoke gradually thick. See flying flies chaotic flowers, or hanging joy in void... dimness, sequentially connected same. Bitter mouth no need contrary ears, only because liver qi not communicating. Medication at this time hopes dissolution, improper care fails. Days long fully dark black, specially named internal cataract blocking double pupils. Names divided into sixteen by shape, Master Long's holy decree explores deeply. Divine medicine hard to effect now, gold needle撥 sun in sky. Strongly follow advice, don't let previous disease repeat.
Generally, internal cataracts have many forms, good/bad varied: Bing, Se, Hua, San. Bing cannot be撥 down, Hua... Light/heavy coarse/fine, measure shallow/deep, slow/fast. Patient cold/hot, excess/deficiency. If heat excess, take cooling medicine first to remove heat-toxin slightly, then open, otherwise fear qi opening vomiting. If wind-deficiency, take wind-dispelling heart-settling medicine first, wait four limbs peaceful, then... Old. Cold nature, opacity hard to age, old opacity use small needle, tender/thin use large needle. Floating opacity, needle near lower edge of black pearl (cornea/pupil); sunken opacity, needle far down. If opacity sunken, needle close and撥, opacity not firm; if floating, needle far and撥, opacity breaks much. Pregnant women or postpartum with this disease, do not needle yet, wait for physical strength peace, then open, otherwise damage.
Opening internal cataract and various membrane polyps, must wait sunny windless weather, quiet place remove noise, calm mind intent, then perform needle. Follow eye left/right, needle towards small... head below. Opening across nose, nose hinders hand, needle not good. Let patient sit frontally, hold doctor's belt, do not release. First steady sharp needle at acupoint. Do not let eye move, stabilize breathing for 50 breaths, slowly advance needle, not too heavy, not too light. Initially gently wipe in, then slightly heavier. If needle head deviates or damages, blood comes out with needle, do not stop, do not press heavily fearing more blood, gently... it. Slow breath slowly force, blood stops itself. If blood firm/tense, entered membrane. If flat still slippery, not fully entered. If entered, eye feels pain, if pain rest briefly, gradually advance. Near passing membrane, pain worse, conveniently intend needle pass. Wait pain settle, invert needle, level with pupil,撥 downwards, do not use heavy hand. Close to pupil, open eye see object. Once see, close eye, slowly withdraw needle, stop 50 breaths, long open clear see object, seal with cotton. Rest according to method, do not lose degree. Slight loss appropriateness, opacity haze rises again, reopen as before. If movement after撥, stop according to state. If pain, pinch anywhere to settle. If severe pain unsettled, cauterize. Before needle, tell person not to fear, swift hand startle vomit, also need medicine stop pain. Rhubarb, Costus root etc. powder, vinegar paste mud, make cake press settles. Or vomit unsettled, swallow white plum saliva, pre-swallow, severe vomiting hard to stop. All medicines prepare, do not lack temporarily. Pain long unbearable, see damage.
After opening eye seal cotton 7 days, eat bean porridge, supine, do not turn side sleep, always watch, do not leave person, do not shout loud, urination/defecation slowly help up, do not let patient exert force, do not wash face face wind. Rest 7 days open seal, if see object still white or like frost/snow, because eye tender. Do not fully remove seal, see object can open occasionally. If see object much eye pain, must damage. Two 7-days after remove seal. Object like belt, flying insect suspending needle move, water wheel not settled, medicine gradually self-cure. After 3 7-days, eye maybe itch no worry.
At opening, patient not too full, not hungry. After opening see object, if pain, needle left/right, pinch knead left/right Du Meridian, Temporal, Fengfu etc. acupoints. If needle mark pain, 2-3 days self-settle. One month no wash face, fear water enter needle hole damage. Use cotton soaked salt water, gently wipe... Seven days no eat rice, fear water move jaw joint, affect water wheel, so eat porridge, soft food. Treating eyes regardless of opacity/miscellaneous states, do not face wind look sun, anger/joy sexual intercourse, five pungents wine/noodles roasted/explosive toxic foods, all cut off. Only relax temperament, careful protect regulate, no not cure. If indulge violate, touch taboo, self bring blame.
Long Mu Lun says: Needle internal cataract method. Verse: Internal cataract originally sixteen kinds, doctor must look carefully. Clearly know shapes one by one, needle then safe. If needle method same round opacity, mistake damage divine light cure hard. Cold/heat first use deficiency/excess, harmonize four limbs wait peace. Otherwise qi stuffy violate rest, vomiting labor spirit opacity flip. Cough shake head all not get, much fright first take heart-settling pill. If seek cooling medicine silver ointment etc., intend temporary body later look. Old opacity fine needle coarse撥 tender, needle shape not same. Disease deficiency new birth pregnant month, hand startle know rest hard. No rain no wind plus auspicious day, pure fast three days before needle. Calm mind intent practice medicine way, chant Buddha relatives no mix noise. Patient face bright cross-legged sit, lift master waist... Suddenly shock opacity heavy down, medicine 30 days see bright. Seven days unseal must see object, flower spawn water move don't speak. Restore Vision pills/powders firm heart take, 100 days clearly restore茜 root.

Eye pain/dryness seeing flowers, or nausea severe pain wanting vomit, or after vomiting, one eye starts then spreads. Front flowers red/black, due to liver/lung labor. *Verse:* Onset dizziness temporal pain, temple connection is Green Wind. Eye socket connect nose often pain, stuffy dry spawn flowers black/red. Liver organ only first affected left, Lung family right eye vanguard. Subsequently connected all total affected, because pulse belt qi communicate. This is zang qi not harmonious, brightness retreat, eye band blocked, after 3-5 years dim qi forms opacity, blue-white, cannot distinguish figures, spreads, pupil tiny, needle ineffective, only medicine tonify five zang stop disease trend. *Verse:* All no pain/itch no headache, gradually dim like object hide. Covered opacity just like dim non-diseased, Black Wind root what say.
