Qibo said: Walking at night causes panting, the root of which lies in the Kidney; excessive pathogenic qi damages the Lungs. Panting resulting from falling and fear originates in the Liver, with excessive pathogenic qi harming the Spleen. Panting resulting from fright...
Upper body panting and shortness of breath, or obstruction in the throat, is caused by extreme deficiency of the original Qi, compounded by internal and external cold. If the Yin-cold dissipates internally and Yang-Qi recovers, recovery will occur. It is appropriate to use Fan Yin Dan (Return to Yin Pill) as the primary treatment. Generally, for panting caused by mutual overacting among various Zang organs, Xing Zi San (Apricot Kernel Powder) should be used. For panting due to cold invasion and latent heat, Hua Gai San (Flower Canopy Powder) and Jiu Bao Tang (Nine Treasures Decoction) are indicated. For sudden cold exposure where excessive clothing and bedding lead to heat, causing excess Lung Qi and panting, the presentation resembles a heat syndrome; Xi Xin San (Wash Heart Powder) is appropriate, administered with cold water. For panting and shortness of breath due to disharmony of Qi and Blood with abundant phlegm, Si Qi Tang (Four Seven Decoction) is suitable. If the official bureau's Jie Geng Tang (Platycodon Decoction) is unavailable, decoct Jie Geng (Platycodon Root) with ginger juice. For panting accompanied by excessive saliva, Qian Min Tang (Thousand Strings Decoction) and Ju Pi Ban Xia Tang (Tangerine Peel and Pinellia Decoction) are appropriate. For inability to lie flat, where panting occurs immediately upon lying down, this is due to rebellious Water Qi rising to the chest and diaphragm, knotting in the right hypochondrium, large as a cup, causing chills and fever, panting, and potentially carbuncles and sores; Zao Gao Wan (Date Paste Pill) is indicated. If Water Qi invades the Lungs, the Lungs become floating and open upon contact with water, preventing Qi from flowing freely through the six meridians; Shen Mi Tang (Mysterious Decoction) is highly appropriate. For lung accumulation named "Upper Body Panting," Shen Shou Tang (Divine Bestowal Decoction) is indicated. For panting and shortness of breath characterized by excess above and deficiency below, Su Zi Jiang Qi Tang (Perilla Seed Descending Qi Decoction) is suitable, taken with swallowing Hei Xi Dan (Black Tin Pill). For elderly individuals who naturally have a robust constitution and cannot tolerate hot medicines, Dan agents (mineral pills) should not be lightly administered; instead, various hot medicines such as Mi Chuan Jiang Qi Tang (Secretly Transmitted Descending Qi Decoction), Fen Xin Tang (Separate Heart Decoction), and Qi Yin (Qi Drink) may be discussed, the general principles of which do not extend beyond this scope. For those with pre-existing Hong Jiao Jiao (Beriberi/Foot Qi) who develop panting, it should be treated as Hong Jiao Jiao. There are also specific methods for treating Gu Zheng Lao Chuan (Bone-steaming Labor Panting).
The term "Cu" (rapid/shortened pulse or breathing) refers to foot diseases. The Lungs govern this; the five Zang and six Fu organs all receive Qi from the human body. Excessive anger or joy without restraint leads to Lung Qi excess, causing upper panting, and Lung Qi deficiency, causing shortness of breath. However, in pregnant women, menstrual blood ceases; it nourishes the breasts upwards and the fetus downwards. If there is pre-existing retained fluid that binds with blood, leading to disharmony of cold and heat, disturbing the blood and allowing fluid to rise upward, chest and diaphragm oppression and coughing result. Postpartum panting occurs because Ying Blood (Nutritive Blood) is suddenly depleted, and Wei Qi (Defensive Qi) loses its master, attacking the Lungs, thus causing panting and shortness of breath. The fault lies in excessive purgation during childbirth; this is termed a condition of Yang extinction, which cannot be discussed merely as panting; Liu He Tang (Six Harmonies Decoction) is appropriate. For panting triggered by the expulsion of lochia, Hu Po San (Amber Powder) should be given first, followed by anti-panting medicines.
