Shaoyang disease is characterized by bitter taste in the mouth, dryness of the throat, and dizziness (dizzy vision).
(This is a pathology of the Foot-Shaoyang Gallbladder Meridian. The *Neijing* states: A disease manifesting with bitter taste in the mouth is named Dan-dan (Gallbladder Heat). The *Jiayi Jing* states: The Gallbladder is the Middle-Clear Viscus; the decisions of the Five Zang Organs depend on the Gallbladder.)
When Shaoyang is struck by Wind-pathogen, symptoms include bilateral deafness, red eyes, fullness in the chest accompanied by vexation. In such cases, emetic or purgative therapies must not be used; if mistakenly applied, palpitations and fright will ensue.
(The Shaoyang meridian originates at the outer canthus and runs into the ear. Its branch descends through the chest, piercing the diaphragm. Wind damages Qi, and Wind tends to generate Heat. Shaoyang Wind-strike represents a deficiency pattern.)
In Cold Damage diseases, if the pulse is wiry and thin, accompanied by headache and fever, it belongs to Shaoyang. Shaoyang disease prohibits sweating therapy; if sweating is mistakenly induced, delirium occurs, indicating involvement of the Stomach. If the Stomach harmonizes, the disease resolves; if not, it worsens.
(The Classics state: When pulses in all three positions are wiry, Shaoyang is affected. A thin pulse indicates the pathogen gradually transmitting inward. Although headache and fever persist, indicating the exterior is not yet resolved, because the pathogen lodges in Shaoyang...)
Originally suffering from Taiyang disease that did not resolve, transferring into Shaoyang manifests as hardness and fullness below the costal margin, dry retching, inability to eat, and alternating chills and fever. If emetic or purgative therapies have not yet been used, and the pulse is deep and tight...
(Taiyang transferring to Shaoyang means the exterior pathogen enters the interior-half-exterior region. Hardness and fullness below the costal margin, inability to eat, and alternating chills and fever indicate the pathogen is between the half-exterior and half-interior. If...)
If emetic, purgative, sweating, or warm-needle therapies have already been administered, resulting in delirium, and the Chaihu Tang syndrome has ceased, this is termed a "ruined disease" (Huai Bing). One must determine what error was committed and treat according to the specific condition.
(The Shaoyang pathogen resides between the exterior and interior. If emetic, purgative, sweating, or warm-needle therapies are used recklessly, body fluids are depleted, leading to dryness in the Stomach. Wood-pathogen invades the Stomach, inevitably... causing delirium.)
Combined disease of the Three Yangs presents with a floating and large pulse, prominent above the Guan position, somnolence, and sweating upon closing the eyes.
(The Guan pulse is used to observe Shaoyang Qi. Taiyang pulse is floating, Yangming pulse is large. A floating pulse extending to the Guan position indicates combined disease of the Three Yangs. Gallbladder heat causes somnolence... Gallbladder has...)
After six or seven days of Cold Damage, with no significant external heat but restlessness and vexation in the patient, this is due to Yang receding and Yin receiving the pathogen.
(The exterior is Yang, the interior is Yin. When the pathogen is in the exterior, there is external heat. Six or seven days is the time for the pathogen to enter the interior. External absence of significant heat with internal restlessness indicates the exterior...)
On the third day of Cold Damage, when the Three Yang meridians' pathogens are exhausted, the Three Yin meridians should receive the pathogen. However, if the patient can eat and does not vomit, this indicates the Three Yin meridians have not received the pathogen.
(On the fourth day of Cold Damage, exterior pathogens transmit inward. If the interior is disharmonious, eating is impossible and vomiting occurs. Now, being able to eat without vomiting indicates the pathogen does not transmit to Yin, remaining only in Yang...)
On the third day of Cold Damage, if the Shaoyang pulse becomes small, the disease is about to resolve.
(The *Neijing* states: Large pulse indicates pathogen arrival; small pulse indicates peace. On the third day of Cold Damage, when the pathogen transmits to Shaoyang, the pulse should be wiry and tight. A small pulse now indicates the pathogen is weak and the disease is about to cease...)
The time when Shaoyang disease tends to resolve is from Yin hour to Chen hour.
(The *Neijing* states: Shaoyang among Yang corresponds to Spring Qi. Yin, Mao, and Chen hours are the times when Shaoyang Wood Qi generates.)
