The *Neijing* (Classic of Internal Medicine) states: When heat is dominant, the earth becomes hot; when wind is dominant, the earth shakes; when dampness is dominant, the earth becomes muddy; when cold is dominant, the earth cracks. The qi of cold, heat, wind, and dampness all originate from the earth.
Pulse qualities are divided into three grades (referring to depth or severity), with similar internal and external symptoms but different pulse characteristics. If the pulse is floating, large, and slow, it is appropriate to administer Xuming Tang (Cure-All Decoction); two doses should lead to recovery. If wind pathogen predominates, Yuebi Tang (Yue Bi Decoction) with four liang of Baizhu (Atractylodis Rhizoma) should be prepared. If the pulse is floating, large, tight, and rapid, Zhuli Tang (Bamboo Juice Decoction) should be prepared. If the patient’s pulse is faint and weak, Fengyin Tang (Wind-Guiding Decoction) is appropriate. If the pulse is floating, large, tight, and rapid, this is the most malignant pulse among the three grades. Alternatively, if the pulse is deep, fine, and rapid, this pulse quality is identical to the floating, large, and tight type. Additionally, if there is suffering from distension and fullness, Biejia Tang (Turtle Shell Decoction) can be taken to purge. If the decoction's effect has exhausted but purgation has not occurred, pill medicine can be used to assist the decoction in inducing purgation. After purgation, Zhuli Tang (Bamboo Juice Decoction) should be taken again to subdue the pulse force and soothe the breath; thereafter, San'er Shier Wei Bafeng San (Thirty-Two-Ingredient Eight-Winds Powder) is beneficial. Upon initial onset of the disease, apply Yege Gao (Wild Hemp Paste) twice daily until stubborn numbness and leg weakness completely resolve. If taking Zhuli Tang (Bamboo Juice Decoction), the pulse force will naturally subside, returning to a state as if healthy, with normal qi circulation and sufficient physical strength, although the legs may not immediately walk; then gradually practice walking. For severe cases, it may take half a year after recovery before one can walk with support. Once the pulse normalizes and the body improves, one should diligently take Bafeng San (Eight-Winds Powder). Do not lightly add other treatments just because one foot cannot yet walk; other treatments may not be fully beneficial and could generate various adverse conditions, causing one to lose the efficacy of these primary treatments. Some doubt Yege Gao (Wild Hemp Paste); some fear that Zhuli Tang (Bamboo Juice Decoction) harms the waist and legs; for such individuals, do not administer treatment, recognizing their constitution is unsuitable for medical intervention. There are three formulas for Zhuli Tang (Bamboo Juice Decoction): mild cases take the first formula, severe cases take the second, and very severe cases take the third. This wind-toxin is easily transmitted; patients should apply Xiaojinya San (Little Gold Tooth Powder) to empty areas and apply a small amount inside the nostrils and ear gates. When observing distressed patients or mourning the newly deceased, even robust individuals should take it and apply it to the nose and ears to approach the deceased or visit the sick. Place one cun cube in a crimson pouch; men wear it on the left arm, women on the right. Start with a small dose of this powder. Patients should especially drink Chixiaodou Yin (Red Bean Drink) and, in winter, Cizi Jinya Jiu (Side-Child Gold-Tooth Wine). Xuming Tang (Cure-All Decoction) treats early-stage wind-toxin disease, resembling epidemic toxic disease, with a floating-slow pulse that does not become rapid; this indicates untreatability. Death may occur within days or weeks, or the patient may lose consciousness upon onset, develop jaundice, macules, red eyes, or ulceration in the lower body; this is the most critical situation. Administer one dose of Xuming Tang (Cure-All Decoction) immediately, followed by Genma Huang Tang (Pueraria and Ephedra Decoction) to purge. If the pulse remains un-subdued, give three more doses of Xuming Tang (Cure-All Decoction); recovery is certain.
Generally, when Hongxiangjiao (Foot Qi Disease) enters the abdomen, constipation occurs. One must unblock the bowels first, so that the internal wind-cold-damp-toxin qi can be dispersed and unblocked. Failure to follow this method causes toxins to linger and transform, rushing upward with colic, vomiting, and wailing. Only San Jiangjun Wan (Three Generals Pill) acts most swiftly. Deviating from this drug or using inappropriate alternatives leads immediately to toxicity surpassing phlegm-rebellion, suffocation, panting, profuse sweating, coma, convulsions, and teeth-grinding, leaving only death. Shanjie Tang (Arborvitae Segment Decoction) is also excellent.
For early Hongxiangjiao (Foot Qi Disease) with leg weakness, moxibustion should be performed promptly, followed by Zhuli Tang (Bamboo Juice Decoction), and then Bafeng San (Eight-Winds Powder); recovery is assured. Treatment must be urgent. Those who only undergo moxibustion but do not take powders, or take powders but do not undergo moxibustion, will be half-healed and half-dead. Even if recovered, relapse may occur within one to two years. Upon recurrence, follow this method of moxibustion and powder administration; ten out of ten cases heal. Mild cases, though asymptomatic initially, if treated improperly without removing the root cause, may eventually prove fatal; thus, careful attention is essential.
Moxibustion targets eight acupoints:
1. Fengshi (GB31): Have the patient stand upright, arms hanging straight with fingers spread, covering the thighs; mark the point at the center of the middle finger on the large tendon of the thigh. Moxa 100 cones; severe cases up to 500–600 cones per site.
2. Upper lateral branch midpoint: Moxa 100 cones (or 50).
3. Dubi (ST35): At the upper border of the patella, level with the outer bone edge, where joint space is felt upon pressure. Or below the knee, between the three outer bones near the heel; feel flexion/extension gap. Moxa 50–100 cones.
4. Xiyan (EX-LE5): In the depressions on both sides below the patella.
5. Zusanli (ST36): Below the knee joint, attached to the outside of the tibia. Or 3 cun below the knee joint, measured by the patient's hand. Moxa 100 cones.
6. Shanglian (EX-LE7): Below Zusanli, attached to the outside of the tibia. Moxa 100 cones.
7. Xialian (EX-LE8): Below Shanglian, attached to the outside of the tibia. Moxa 100 cones.
8. Yanglingquan (GB34/Jiegu): Above the lateral malleolus, one bone width away, or 4 cun above.
Moxibustion need not be done all at once; treat two sites per session. All joints and abdomen should be moxibusted. Take Bafeng San (Eight-Winds Powder); recovery is common. If disease enters the abdomen and pain is unbearable, avoid heavy moxibustion.
Ten toes ends are called Qiduan (Qi Ends); moxa 3 cones daily for great effect. Bachong points can be moxa 7 cones daily until qi rises, then stop. Patients must entrust treatment carefully; do not perform moxibustion on others without deep understanding.
