In cases of stroke (Feifeng) manifesting with deviation of the mouth and eyes, closed eyes, relaxed hands, incontinence of urine and feces, foaming at the mouth, fixed gaze, snoring-like respiration, deep coma unresponsive to stimuli, extreme emaciation of muscles with severe tendon pain, straightened hair, head shaking with upward deviation of the eyeballs, facial redness resembling makeup, or heavy head with cyanosis/blackening of the face, nose, and Shangen area, sweat droplets like pearls, and gurgling phlegm sounds in the throat, all are considered incurable.
First, regarding the pulse in stroke: a slow and moderate pulse indicates potential recovery, whereas a rapid, hurried, wiry, or large pulse indicates death.
(The above two items discuss the treatment of wind diseases, hence they are listed here.)
Hua Yuanhua stated: Wind Jue (sudden collapse due to wind pathogen) is caused by contracting the Qi of the four seasons that does not conform to normal patterns, thereby causing disease. Some present as Urticaria (Yinzhen), some as Hemiplegia (Piangu), some as Aphonia (Shiyin), some as Joint Pain (Lijie), some as Epileptic Collapse (Dianjue), some as Pain, some as Deafness and Blindness (Longgu), some as Skin Ulcers and Leprosy (Chuanglai), some as Distension and Fullness, some as Asthma and Fatigue, some as Dysentery (Chi Bai), some as Cyanosis and Blackness, some as Itching, some as Mania, all originating from Wind. A floating and deficient pulse arises from intrinsic deficiency; a solid and large pulse arises from intrinsic excess; a wiry and tight pulse arises from sweating followed by wind contraction; asthma and fatigue arise from excessive alcohol consumption; epileptic collapse arises from overexertion; inability to move limbs and slurred speech arise from sexual indiscretion; urticaria arises from inherent dampness; joint pain arises from engaging in sexual activity while intoxicated; deafness, blindness, ulcers, and leprosy arise from improper diet of the five flavors and violating taboos. Symptoms are myriad but never exist apart from the Zang and Fu organs.
Xue Lizhai stated: For the aforementioned conditions, if caused by Kidney Yin deficiency leading to deficient fire and Liver dryness, Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) should be used to generate Kidney Water and nourish Liver Blood. If caused by anger stirring up Liver Fire leading to blood depletion, Siwu Tang (Four Substance Decoction) added with Chaihu (Bupleuri Radix), Zhizi (Gardeniae Fructus), Danpi (Moutan Cortex), and Fuling (Poria) should be used to clear Liver Fire and generate Liver Blood. If caused by Spleen meridian stagnation leading to blood depletion, Guipi Tang (Restore the Spleen Decoction) and Siwu Tang should be used together to tonify Spleen Qi and generate Liver Blood. If Spleen Qi is deficient with phlegm retention, Erchen Tang (Two-Cured Decoction) added with Baizhu (Atractylodis Macrocephalae Rhizoma) and Chaihu should be used to strengthen the Spleen and resolve phlegm. If Spleen deficiency with dampness leads to obstruction by wind-phlegm, Erchen Tang added with Nanting (Arisaematis Rhizoma Preparatum), Cangzhu (Atractylodis Rhizoma), and Fangfeng (Saposhnikoviae Radix) should be used to overcome dampness and resolve phlegm. If there is stagnation in the Spleen meridian, Guipi Tang added with Chaihu and Banxia (Pinelliae Rhizoma) should be used. If敗液 (defective fluids) of the Kidney meridian condense into phlegm, Liuwei Wan (Six-Ingredient Pill) should be used.
