Mild cases are limited to foot pain; severe cases progress from foot pain to the genitalia, reaching the lower abdomen, passing through the costal region up to the head; even more severe cases develop into Hong Kong Foot rushing to the heart (referring to damp-toxic qi attacking the heart), and mismanagement leads to immediate death. (*Cangsheng Siming*)
The disease of Hong Kong Foot is mostly caused by water-dampness, but may also be complicated by wind-cold or summer-heat. During attacks, the affected area may swell or hurt. When damp-heat prevails, it forms blister sores, or becomes red swelling and erysipelas, or acts like hernia attacking upwards and downwards. In all these cases, one must carefully examine the pulse and verify symptoms, thoroughly distinguishing between interior/exterior and the deficiency/excess of blood and qi, then treating accordingly. (*Yi Gu*)
Hong Kong Foot is kidney fire carrying dampness, manifesting in the skin and flesh, presenting as red swelling resembling cloud marks, or faintly visible redness, which feels hot and painful upon pressure. The red swelling starts from the feet and gradually moves up to the thighs; its momentum inevitably rises. If it reaches the heart, it is incurable. Urgent treatment is required to prevent the pathogen from rising, which is the best approach. (*Dingbu Mingyi Zhizhang*)
The syndrome of damp-type Hong Kong Foot begins very subtly. People often mistake it for internal or external sores. Due to improper treatment, the condition gradually worsens. Some present with a depressed center and protruding edges, while others show purple-dark, cracked surroundings. At this stage, the toxin has penetrated outward. Without using herbs that disperse and ascend (such as Ephedrae Herba) or promote circulation (such as Feibu Wan), the sores will only become more ulcerated and open. It is advisable to wash with decoctions of Lotus Leaf Heart, Ligusticum Wallichii Rhizome (Gaoben), or Nardostachys Chinensis Radix et Rhizoma (Gansong) to absorb dampness and draw out toxins, while continuing to administer Xuanma Feibu Wan, allowing the sores to heal naturally. One should not use astringent drugs to hastily close the sores; if closed too quickly, the toxin may attack upwards, causing rapid and severe harm. Although the sores are repulsive, there is no danger of the toxin entering the abdomen to attack the stomach or rushing to the heart, which is a cause for relief. (*Xu Yijian Houji*) [Note: Sores arising from Hong Kong Foot are recorded in *Waitai Miyao* citing Xiao Liang, and in *Shenghui Fang*; verification is recommended.]
Hong Kong Foot may present as a single pore at the heel, about half cun deep, with exceptional pain every afternoon. This is foot qi transforming into a fistula. Burn Renzhongbai (Human Tooth Fossil) over fire, collect the exuded water, and drip it into the sore. (*Yaojue*)
Daoist Wang Yu said: Some suddenly suffer from foot sole pain as if pierced by an arrow, occurring intermittently without fixed timing. This is wind-toxin in the kidney; purging the kidney meridian will cure it. (*Yishuo*, citing *Bozhai Bian*)
Death Signs: *Ge Shi Fang* states: Weakness and fullness with bi-syndrome in the legs. If the pathogen reaches the lower abdomen accompanied by dysuria, and qi rebels upward, death ensues. (*Yixin Fang*)
Whenever any abnormality in symptoms is perceived, one must feel extreme alarm and resolve to treat urgently. Delaying treatment allows qi to rebel into the abdomen, causing swelling or non-swelling, chest and hypochondriac fullness and rebellion, and shortness of breath with qi rushing to the shoulders. Acute cases die instantly; milder cases die within days. Urgent treatment is imperative. Look for signs such as urgency below the heart, continuous panting, spontaneous sweating, alternating chills and fever, a hurried, short, and rapid pulse, and incessant vomiting; all indicate death. (*Qianjin Fang*) [Note: *Shenghui Fang* adds that rapid pulse with inability to lie down indicates death.]
When Hong Kong Foot transmits to the heart and kidneys, ten out of ten cases are fatal. Transmission to the heart manifests as mental confusion, forgetfulness, errors in speech/action, vomiting, inability to eat, restless sleep, and twitching of the mouth and eyes. The pulse at the left Cun position fluctuates between large/small and present/absent. Transmission to the kidneys manifests as swelling of both waist and feet, urinary retention, continuous groaning, black discoloration of the eyes and forehead, and wheezing due to qi rushing to the chest and abdomen. The pulse at the left Chi position is absent. Careful examination is essential. (*Zhongzang Jing*)
Treatment: Avoid drastic tonification or purgation. All deaths from Hong Kong Foot result from excess qi; no one dies from deficiency caused by medication. Therefore, patients with Hong Kong Foot should not undergo drastic tonification nor drastic purgation. Do not fear deficiency to the point of prematurely stopping diuretic/decoction treatments; such patients will die and cannot be saved. (*Qianjin Fang*)
Treatment Must Follow the Four Seasons: Treating Hong Kong Foot requires following the seasons. In spring and autumn, combine tonification and purgation. In summer, when the disease is prevalent, focus on sweating and urination. After the tenth month, use tonic medicines. Although minor adjustments are made, the principle remains unchanged. (*Waitai Miyao*, citing Su Jing)
Patients with Hong Kong Foot, despite suffering from weakness and emaciation, should not take excessive tonics. Tonics cause abdominal distension; without purgation, the disease will not improve, and emaciation will worsen. Tonics are only suitable in winter. However, if qi rebellion persists in winter, tonics should still be avoided; judgment is required. Generally, qi rebellion requires mild purgation, but excessive purgation leading to deficiency must be avoided. Most chronic foot conditions stabilize in winter because kidney qi is prosperous in winter, prospering qi resists pathogens, thus allowing the waist and feet to remain clear. (Ibid.)
After entering winter, assess the patient's strength and add mild nourishment; otherwise, qi and blood will decline daily, inevitably causing recurrence during hot, humid periods, which is natural law. (*Jisheng Fang*)
Contraindication: Vomiting. Cases where Hong Kong Foot qi rushes to the heart should not be treated with emetics. (*Yixin Fang*, citing *Yimen Fang*)
Contraindication: Washing/Bathing. Taking tonics and using herbal baths are major taboos for physicians. (*Huren Shu*)
Most taboo: Steeping/bathing with hot medicines. This risks forcing pathogens into the meridians, making them difficult to treat. (*Jicheng*)
Most taboo: Tonic preparations, herbal baths, and cold herbal compresses. Violating these taboos allows toxic vapors to enter the heart, causing stubborn numbness in the lower abdomen, or wheezing and vomiting. (*Lingnan Weisheng Fang · Appendix*)
*Huren Shu* states: Taking tonics and using herbal baths for Hong Kong Foot are major physician taboos. *Bencao Faming* argues this applies to southern external dampness invading deficiency causing swelling/pain, not northern internal dampness sinking downward causing swelling/pain. Since dampness cannot reach the exterior, washing and guiding to vent the pathogen is appropriate. (*Yuji Wei Yi*)
Treatment of Secondary Diseases: For diseases arising secondary to Hong Kong Foot, treat according to the specific symptoms. For dysuria, use Polyporus (Zuling), Poria (Fuling), and other diuretics. For severe constipation, use Wuruomaren Wan. For generalized edema becoming water disease, use formulas from water-draining categories. Others follow this pattern, without rigid restrictions. (*Qianjin Fang*)

Midnight, sudden blockage, mass in hypochondrium stone-sized (*Jifeng Fang* says dry foot qi, midnight blockage, left hypochondrium mass soybean-sized). Near death, unconscious for three days due to severe cold, family weeping. Zheng Xunmei of Xingyang passed down Shanmu Tang. After half meal time, severe purgation, three purgations allowed qi to pass and mass to disperse. Pinus Massoniana Nodes (Shanjiejie) 1 da sheng, Cut Citrus Reticulata Leaves (Juye) 1 da sheng (North lacks leaves, use bark substitute), Large Arecae Seeds (Dafu Binglang) 7 pieces, crushed with shell, Boy's Urine (Tongzi Xiaobian) 3 da sheng. Boil together to obtain 1.5 sheng. Divide into two doses. If first dose causes rapid purgation, stop second dose. (*Zhenglei Bencao*) *Xu Yijian Houji* states: For severe toxicity, phlegm rebellion/oppression, wheezing/sweating, coma/spasms/grinding teeth/upward gaze, near death, only Shanmu Jie Tang is supreme, 100% effective, reviving the dying.
