The stagnation of the Five Movements (Wu Yun) in heaven and earth corresponds to the reactions of the Five Zang Organs in the human body. When Qi mechanism gathers and fails to circulate, when it should ascend but does not ascend, when it should descend but does not descend, and when it should transform but does not transform, then diseases of stagnation arise. Therefore, stagnation may occur in Qi, or in Blood, or at the Exterior, or in the Interior; one may become ill due to stagnation, or develop stagnation due to illness. For excessive stagnation, it is appropriate to restrain and suppress it; for deficient stagnation, it is appropriate to tonify and assist it. Most diseases involve concurrent stagnation; thus, treatment methods vary.
Climatic Phenomena: Rocks and valleys shake with thunderous sounds, and loud thunder roars at the intersection of Qi (occurring between the Third and Fourth Qi phases). Dust rises, darkening the sky yellow-black, transforming into white misty air. Rivers overflow and spread widely, while fields and pastures resemble scenes after floods where foals graze freely. Clouds rush toward the Rain Mansion (where Taiyin Dampness gathers), and morning mists embrace the rising sun. Mountains and marshes are obscured by dust and haze; this is the manifestation of Earth Qi stagnation. When Earth Qi is stagnant, its transformations are delayed. However, since Earth Stagnation occurs during the Third and Fourth Qi periods, growth and transformation still proceed without missing their proper timing.
Public Health Manifestations: Diseases caused by Damp-Earth pathogenic factors. When Dampness resides in the Middle Jiao (Middle Burner), epigastric and abdominal distension occur. When Dampness resides in the Lower Jiao (Lower Burner), frequent diarrhea ensues. If the Heart is invaded by Dampness, pain results. If the Liver is oppressed by Dampness, hypochondriac distension occurs. Vomiting, cholera-like syndromes (Huo Luan), profuse watery diarrhea accompanied by edema and heaviness of the body, are all symptoms of Earth Qi activation and Dampness pathogenicity.
Treatment Principles: For Earth Stagnation, employ the "Du" (Plunder/Attack) method. "Du" refers to direct removal. Diseases of Earth Stagnation belong to the category of Dampness obstruction. The corresponding organs are the Spleen and Stomach; the primary domain involves muscles and four limbs; the site of injury is the chest and abdomen. Earth fears obstruction. If stagnation is located superiorly, remove it from above, using emesis is appropriate. If stagnation is located centrally, remove it from the middle, using purgation/attack is appropriate. If stagnation is located inferiorly, remove it from below, using catharsis/laxative therapy is appropriate. All these constitute "Du," which is not limited solely to the lower part.
Climatic Phenomena: The sky is clear, the earth bright, the wind is crisp and sharp, great coolness arises, grass and trees are shrouded in floating smoke, Dryness Qi begins to move, fog frequently rises, killing/somber Qi arrives, vegetation becomes pale and dry, Metal produces sound, mountains and marshes scorch and wither, soil congeals into frost and brine. This is the manifestation of Metal Qi stagnation. Metal Qi is prosperous during the Fifth Qi phase, governing the period after the Autumnal Equinox (mid-August), lasting slightly more than sixty days.
Public Health Manifestations: Coughing, counterflow Qi, and dry throat indicate Lung disease combined with Dryness. Fullness in the heart and hypochondrium, radiating to the lower abdomen with sudden severe pain, preventing turning over on either side, indicates Metal Qi excess injuring the Liver. Due to the肃杀 (Sousha - austere and killing/nature of autumn) quality of Metal Qi, the complexion appears dull and old, causing aversion.
Treatment Principles: For Metal Stagnation, employ the "Xie" (Drain/Unblock) method. "Xie" refers to dredging and promoting free flow. Diseases of Metal Stagnation belong to categories of constriction, closure, dryness, and blockage. The corresponding organs are the Lung and Large Intestine; the primary domain involves skin/hair and respiration; the site of injury is the Qi level. Treatment may involve releasing the Exterior, breaking Qi stagnation, or unblocking bowels. Whether the pathology is in the Exterior, Inferior, or Superior, the "Xie" method is applied.
