齿痛实症，阳明风火上升也，葛根白虎汤主之。
葛根白虎汤（自制）
葛根（二钱） 石膏（五钱） 花粉（三钱） 石斛（三钱） 连翘（一钱五分）
薄荷（一钱） 防风（一钱） 桔梗（一钱） 淡竹叶（二十张） 白茅根（五钱）
本方兼采白虎、竹叶石膏意，而加葛根、防风，使在腑之火由经而外达。石斛、花粉所以生津
清热，薄荷、连翘、桔梗所以散郁火，茅根、竹叶清心即所以清胃也。祖怡注。
齿痛虚症，肾亏而夹有胃火也。齿为后天所生之骨，亦属于肾。况肾为胃关，水不制火，故浮
阳作痛也。清热胃关煎主之。
清热胃关煎（自制）
生地（六钱） 龟版（八钱） 花粉（三钱） 石斛（三钱） 薄荷（一钱）
葛根（二钱） 连翘（一钱五分） 桔梗（一钱） 甘蔗（三两同煎）
本方以生地、龟版为主药，所以滋肾。以甘蔗、石斛、花粉为辅佐，所以清胃。薄荷、连翘散郁
火。葛根、桔梗使已发之火，从肺胃表份而解。祖怡注。
舌卷而肿，塞口作痛，难于语言，此心阳炽盛也。先用生蒲黄三钱，泡汤频漱，再服黄连清心
饮。
黄连清心饮（自制）
黄连（五分） 蒲黄（一钱五分） 犀角（五分） 元参（一钱五分） 丹参（二钱）
连翘（一钱五分） 蒌皮（三钱） 茯苓（二钱） 薄荷（一钱） 竹叶（二十张）
灯芯（三尺）
本方以黄连、犀角为主药。以蒲黄、竹叶、连翘、灯芯佐黄连，以丹参、元参佐犀角。以薄荷
合连翘散郁火。
以茯苓合灯芯，导之从小肠膀胱而去。蒌皮利大肠，又所以佐茯苓、灯芯也。祖怡注。
舌色绛红，边尖破碎，舌有血痕而痛者，乃阴液大亏，心火上炽也，大泽汤主之。
大泽汤（自制）
天冬（二钱） 生地（六钱） 人参（一钱五分） 龟版（八钱） 麦冬（一钱五分）
茯神（二钱） 柏仁（二钱） 蛤粉（四钱） 丹参（二钱） 石斛（二钱） 灯
芯（三尺） 藕（五大片）
本方以三才养阴，以龟蛤潜阳，以丹参、柏仁、茯神、藕、灯芯养血凉心，以麦冬、石斛佐地
、冬生津养胃，阴血充而虚火自熄矣。祖怡注。
齿痛的实证，是由于阳明经的风火上升所致，治疗以葛根白虎汤为主方。

齿痛的虚证，是肾气亏损而夹杂有胃火所致。牙齿是由后天生成的骨骼，也归属于肾。况且肾是胃的关隘，肾水不能制约心火，所以浮阳上扰导致疼痛。治疗以清热胃关煎为主方。
清热胃关煎（自制方）
生地黄（六钱） 龟板（八钱） 天花粉（三钱） 石斛（三钱） 薄荷（一钱）
葛根（二钱） 连翘（一钱五分） 桔梗（一钱） 甘蔗（三两，同煎）
本方以生地黄、龟板为主药，用来滋补肾阴。以甘蔗、石斛、天花粉为辅助药，用来清泻胃火。薄荷、连翘疏散郁结之火。葛根、桔梗使已经发作的火邪，从肺胃的表分得以解散。祖怡注。
舌头卷缩且肿胀，塞满口腔引起疼痛，难以言语，这是心阳炽盛所致。先用生蒲黄三钱，泡汤频繁漱口，然后再服用黄连清心饮。
黄连清心饮（自制方）
黄连（五分） 蒲黄（一钱五分） 犀角（五分） 玄参（一钱五分） 丹参（二钱）
连翘（一钱五分） 瓜蒌皮（三钱） 茯苓（二钱） 薄荷（一钱） 竹叶（二十张）
灯芯草（三尺）
本方以黄连、犀角为主药。以蒲黄、竹叶、连翘、灯芯草辅助黄连，以丹参、玄参辅助犀角。以薄荷配合连翘疏散郁结之火。
以茯苓配合灯芯草，引导热邪从小肠和膀胱排出。瓜蒌皮通利大肠，这也是为了辅助茯苓、灯芯草的作用。祖怡注。
舌色深红（绛红），边缘和尖端破碎，舌面有血痕且疼痛，这是阴液严重亏损，心火上炎所致，治疗以大泽汤为主方。
大泽汤（自制方）
天冬（二钱） 生地黄（六钱） 人参（一钱五分） 龟板（八钱） 麦冬（一钱五分）
茯神（二钱） 柏子仁（二钱） 蛤壳粉（四钱） 丹参（二钱） 石斛（二钱） 灯芯草（三尺） 藕（五大片）
本方以三才汤滋养阴液，以龟板、蛤粉潜降阳气，以丹参、柏子仁、茯神、藕、灯芯草养血凉心，以麦冬、石斛辅助生地、麦冬生津养胃，阴血充足而虚火自然熄灭。祖怡注。

This formula incorporates the principles of Bai Hu Tang (White Tiger Decoction) and Zhu Ye Shi Gao Tang (Bamboo Leaf and Gypsum Decoction), adding Puerariae Lobatae Radix and Saposhnikoviae Radix to allow the fire residing in the fu-organs to reach outward through the channels. Dendrobii Caulis and Trichosanthis Radix generate fluids and clear heat; Menthae Haplocalycis Herba, Forsythiae Fructus, and Platycodonis Radix disperse stagnant fire; Imperatae Rhizoma and Lophatheri Herba clear Heart fire, which thereby clears Stomach fire. Deficient-pattern toothache results from Kidney deficiency complicated by Stomach fire. Teeth are bones formed postnatally and also belong to the Kidney. Moreover, the Kidney serves as the gatekeeper for the Stomach; when Kidney water fails to control fire, floating yang causes pain. Tongue rolling with swelling, obstructing the mouth causing pain and difficulty in speech, indicates blazing Heart Yang. First, use Sheng Pu Huang (Raw Cattail Pollen, 3 qian) soaked in water to rinse frequently, then administer Huang Lian Qing Xin Yin (Coptis Decoction to Clear the Heart).

Typhae Pollen, Phragmitis Rhizomatis Node, Forsythiae Fructus, and Thyrsostachys Oliveri Stem assist Coptidis Rhizoma; Salviae Miltiorrhizae Radix and Scrophulariae Radix assist Rhinocerotis Cornu. Menthae Haplocalycis Herba combined with Forsythiae Fructus disperses stagnant fire. Poria combined with Thyrsostachys Oliveri Stem guides the pathogen out via the Small Intestine and Bladder. Salviae Miltiorrhizae Radix, Ziziphi Spinosae Semen, Poria cum Pinellia, Nelumbinis Rhizoma, and Thyrsostachys Oliveri Stem nourish blood and cool the Heart. Ophiopogonis Radix and Dendrobii Caulis assist Rehmanniae Radix Recens and Ophiopogonis Radix in generating fluids and nourishing the Stomach.
