To treat infants and young children suffering from eight types of deficiency dysentery accompanied by fever, or vomiting, diarrhea, fever, cholera-like syndrome (Huoluan), resulting in abnormal qi movement above and below, empty heart with vexation and oppression. When these symptoms appear, cold and cooling medicines should not be added; it is appropriate to administer this formula: Zhengqi Tiaowei San (Upright Qi Regulating Stomach Powder), combined with Shengshu Yinzi (Raw and Cooked Decoction). If the child cannot take food, Shenqi Zisu Yinzi (Ginseng and Perilla Decoction for Boosting Qi) is appropriate.
Zhengqi Tiaowei San (from *Liangfang*):
Houpo (Magnoliae Officinalis Cortex, 1 liang, processed with Shengjiang [Fresh Ginger] and skin, 2 liang, pounded and pressed in a mortar for one to two nights, turned frequently, taken out and slow-roasted until dry); Banxia (Pinelliae Rhizoma, 1 liang, washed and soaked seven times); Bian Dou (Phaseoli Semen, stir-fried); Huoxiang Ye (Agastaches Folium); Chenpi (Citri Reticulatae Pericarpium, each 1 liang); Gancao (Glycyrrhizae Radix et Rhizoma, honey-fried); Yiyiren (Coicis Semen, stir-fried, each 0.5 liang); Baifuling (Poria); Baizhu (Atractylodis Macrocephalae Rhizoma).
Grind the above herbs into fine powder. Each dose is 1 qianbi (approx. 3-5 grams), decocted with a small cup of water, adding two small slices of Shengjiang (Fresh Ginger) and one Jzao (Jujubae Fructus), boiled together.
Discussion states: Vomiting and diarrhea accompanied by fever occur due to disharmony of yin and yang, interference between pathogenic factors and righteous qi, and dysfunction of the zang-fu organs. Upper vomiting and lower diarrhea, or only vomiting with fever, or only diarrhea with fever, are all caused by damage to true qi and failure of qi to ascend and descend. When vomiting and diarrhea occur simultaneously, it is called Huoluan (cholera-like syndrome). At this time, the child's yin and yang qi are abnormal, and the zang-fu become increasingly deficient, with both vomiting and diarrhea exacerbating the condition. Otherwise, wind may arise internally, causing disease progression, transforming into Manpi Feng (Slow Spasm Wind). Because families do not understand the principle, and physicians fail to investigate the details, and ancients did not elaborate on the subtleties, later learners inevitably incur harm. Now knowing that fever from vomiting, diarrhea, gan accumulation (nutritional deficiency syndrome), and dysentery originates from internal deficiency, how can one resolve the exterior? If one attacks the interior, deficiency will generate more deficiency. Following this regulation can achieve appropriateness; whether for large or small, severe or mild cases, perfection can be attained. This is the ultimate truth, which the wise can learn from.
Discussion states: If the Spleen is disharmonious, the Stomach fails to generate Ying (Nutrient) qi; if Ying qi is insufficient, Zheng (Upright) qi is not correct. The Stomach is the root of the Spleen family; Ying is the foundation of the Wei (Defensive) chamber. If the root and foundation are solid, various deficiencies will not arise; if the interior and exterior are full, various pathogens cannot enter. Treatment methods and medication administration involve six combinations: To separate fluids and grains, administer Wuling San (Five-Ingredient Poria Pill); to retreat summerheat, administer Xiangru San (Elsholtzia Powder); to balance yin and yang, administer Jiji Dan (Ji Ji Pill); to warm the Spleen and Stomach, harmonize qi, regulate the Sanjiao (Triple Burner), and remove dampness, administer Lizhong Wan (Regulate the Middle Pill); to pacify and rectify the five zang organs, disperse scattered qi, tonify deficiency, and warm the center, administer Zhengqi Tiaowei San (Upright Qi Regulating Stomach Powder); to warm the Zhongwan point, awaken the Spleen, harmonize the Stomach, eliminate deficiency, boost qi, and open the stomach to promote appetite, administer Fenqi Zisu Yin (Qi-Separating Perilla Decoction). Medications for the above six conditions all harmonize and smooth the zang-fu. Since the zang qi is warm and harmonious, upright qi is correct and smooth, heat naturally dissipates. This illustrates the symptom presentation; if one must not directly use cold and cooling medicines to reduce fever, using hot medicines to stop vomiting and diarrhea, attacking yin and yang qi, causing errors, leading to internal wind, urgently treating wind, and then administering cold medicines in sequence, are all erroneous. Therefore, heat from vomiting and diarrhea cannot be treated with cold and cooling medicines, otherwise it may lead to counterproductive harm. One should act according to the timing.
