Translation:
It is discussed that harmonious Qi (vital energy) is the cause of smooth ascending and descending, leading to peace and well-being; conversely, rebellious Qi leads to stagnation and accumulation, which are the origins of disease. The condition known as "Pan Chang" (Coiled Intestine) in infants does not occur suddenly; originally, it arises from long-standing Qi stagnation that fails to disperse, causing Ying (Nutritive) Qi to become congested and Wei (Defensive) Qi to knot. Consequently, none of the Zang-Fu organs can function smoothly. When pathogenic factors attack due to deficiency, Qi surges up and down, striking between the intestines and stomach, producing continuous sounds resembling a cat's meow. This sight is unbearable, and the suffering is immense. At this time, the Four Elements (Earth, Water, Fire, Wind) within the body lose their master. Another condition, "Diao Chang" (Hanging Intestine), involves cold pathogen stagnation internally without expansion. Although it does not drive Qi to stir the Zang-Fu organs, it causes pulling sensations above the chest and umbilicus, resulting in a curved, hunched posture enduring pain. When these two conditions manifest, they startle physicians and shock onlookers. However, although both are caused by Qi disorders, their onset differs. For "Pan Chang," Yunqi San (Uniform Qi Powder) with Chenxiang (Aquilaria Wood) decocted should be administered, ensuring recovery. For "Diao Chang," Zhenzhu Wan (Pearl Pill) with Chenxiang (Aquilaria Wood) and Xurong (Olibanum) decocted, delivered via Diaoteng Tang (Uncaria Decoction), should be used until slight bowel movement occurs, after which formulas regulating Qi and supporting the root are preferable for optimal results.
It is discussed that cough belongs to the Lung Meridian. The Lung governs Qi and corresponds externally to the skin and hair follicles (Cou Li). Infants require constant care day and night to prevent wind from blowing upon the fontanelle of the head or the Cou Li of the back, which would cause the Lung to suffer from Cold pathogens, leading to persistent coughing, heat generation, and phlegm formation. If contracted by wind, it is termed "Exogenous Contraction." Pathogens transmit sequentially to the deficient areas of the Five Zang organs, entering at will. Since newborns have weak Yuan Qi, transmission occurs easily due to the disturbance of the Five Zang organs, damaging Harmonious Qi, causing disharmony among the Five Zang and obstruction of the San Jiao (Triple Burner). Thus, transmission occurs. Specifically, I am born from Kidney Water, where the Kidney governs deficiency pathogens; I am born from Spleen Earth, where the Spleen governs food retention, vomiting, deficiency-phlegm, limbs, lips, and mouth; I am born from Liver Wood, where the Liver governs Wind, epilepsy, and eyes; the element that controls me is Heart Fire, where the Heart governs fright, palpitations, stubborn phlegm, blood vessels, and facial complexion. Cough transmission may present as vomiting, phlegm-saliva, cold extremities, or fear/palpitations, eventually leading to dark purple discoloration around the eyes, resembling trauma-induced severe epilepsy. Ancients stated, "Long-term cough becomes epilepsy," meaning medicinal power is insufficient once the pattern has transmitted, making regulation difficult. This should be communicated to families in advance.
It is discussed that if a postpartum woman suffers from "Blood Failure," the blood should descend; if Nutritive Blood transforms into milk, incomplete descent of blood causes maternal illness, leading to irregular milk quality, which invites disease in the infant. When an infant suckles, if Qi stagnates in the narrow passages due to failure reaching the chest, whether from cold or sourness, problems arise. One might ask if the infant is unharmed; the answer is no. Prolonged such states cause mental fatigue, weakness, timidity, and emaciation. If the Spleen becomes deficient, it gradually evolves into epilepsy, specifically Spleen Wind. Newborns and one-year-olds have immature Stomach Qi and have not yet consumed solid food, relying entirely on breast milk. If natural and unharmed, the Zang-Fu organs fill, meridians flow smoothly, the body strengthens, and Spirit remains clear. Infant loss of milk is like adult starvation; food is the foundation sustaining life, how can it be cut off? Nursing infants are inherently weak, with slight Qi and Blood, shallow Zang-Fu, and fragile foundations; how can they lose milk? Loss of milk brings many deviations; cessation of milk threatens life. If the mother’s temperament is adverse, Qi and Blood become chaotic; chaotic Qi and Blood lead to inharmonious milk; inharmonious milk causes infant Qi rebellion. Immediate regulation is necessary. Treating this as trivial will inevitably cause harm or death. Xiao Chenxiang Jian (Minor Aquilaria Decoction) and Yanchi Wan (Salted Soybean Pill) should be administered concurrently, proving effective. The nursing mother should take Huoxiang Zhengqi San (Agastache Powder to Rectify the Qi) with Zhizhu Wan (Immature Bitter Orange and Atractylodes Macrocephala Pill) for excellent results.
