The *Daquan* states: Postpartum spontaneous lactation is caused by deficiency of Stomach Qi. It should be treated with tonifying medicines to stop it. If the breasts are overly full, urgent, and painful due to excessive milk, apply warm cloth compresses (one should observe the patient's constitution—strong or weak—and the thickness of the milk—to determine excess or deficiency, as this occurs due to milk overflow). The *Chanbao* discusses this condition but provides no formula for treatment. If these symptoms occur, the Loulu San may also be used.
Spontaneous lactation before childbirth is called "Ruqi" (Breast Weeping). Children born under such conditions are often unable to survive, a topic not discussed in classical texts. (I once diagnosed a female patient whose Qi was abundant and pulse vigorous, suffering from Ruqi. Her child grew up healthy and active. Thus, the claim that such children cannot survive is not entirely accurate.)
Xue Ji stated: For the aforementioned symptoms, if both Qi and Blood are deficient, use Shiquan Dabu Tang; if there is Blood Heat in the Liver Meridian, use Jiawei Xiaoyao San; if there is Fire Anger in the Liver Meridian, use Siwu Chaihu Zhizi Huangqin Lian Decoction; if there is Depression and Anger affecting the Liver and Spleen, use Jiaweigui Pi Tang.
A postpartum woman, due to overexertion, suddenly experienced milk gushing out, became unconscious, and vomited phlegm. This was a Jue syndrome caused by Yang Qi deficiency. She revived after being administered Dushen Tang and stabilized after several doses of Shiquan Dabu Tang. If a woman’s Qi and Blood are robust, and her breasts are distended and painful due to lack of infant suckling, accompanied by aversion to cold and fever, she should take two to three liang of fried malt, decocted in water, which will dissipate immediately. Malt has such a strong effect of dispersing Qi and Blood; why is it frequently used in formulas for Spleen-Stomach weakness and indigestion?
Loulu San
See formula above.
Mianhuai Tang: For those wishing to wean, use this formula to regulate menstruation, which will stop lactation.
Danggui Wei (Tail of Angelica Sinensis), Chishao Yao (Red Peony Root), Honghua (Safflower, soaked in wine), Niuxi (Achyranthes Bidentata, soaked in wine) [each 5 qian].
Crush the herbs, decoct in water, and take orally.
Another method: For postpartum women with robust Qi and Blood, breast distension, or pain due to lack of infant suckling, who wish to eliminate swelling, taking this formula brings immediate relief. Take two liang of fried malt, decocted in water. Alternatively, grind the fried malt into powder and mix with Siwu Tang decoction to stop lactation.
The *Daquan* states: Postpartum "Chuan Nai" (Blown Milk) occurs when an infant falls asleep while nursing, and the breath blown by the infant obstructs milk flow. The milk accumulates internally, causing swelling and hardness, blocking the milk ducts, leading to injury, knotting, and pain. Some cases present with downward pain without tenderness, or hardness like stone. This is collectively called "Chuan Nai." If not treated urgently, severe swelling can lead to carbuncles and even death. Continuously administer Zaojiao San and Gualou San, apply Tiannanxing San externally, and massage with hands to dissipate the swelling.
Xue Ji stated: When treating the aforementioned condition, one must absolutely avoid damaging Qi and Blood.
Zhu Danxi stated: The breasts belong to the Yangming Meridian, and the nipples belong to the Jueyin Meridian. Nursing mothers who fail to maintain proper regulation may suffer from qi reversal due to anger, qi stagnation due to depression, or damp-heat accumulation due to rich foods. This causes Jueyin Qi to fail to circulate, resulting in blocked orifices and inability to excrete milk. Meanwhile, Yangming Heat boils vigorously, leading to severe heat and suppuration. There are also cases where the nursing child has stagnant phlegm in the chest/diaphragm and hot breath; while sleeping with the nipple in the mouth, the hot breath blows into the breast, forming nodules. At the initial stage, one must endure the pain, massage until slightly soft, and suckle to ensure milk flow, allowing natural dissipation. Otherwise, untreated, it will become a carbuncle or boil. The treatment method involves unblocking Jueyin stagnation with Qingpi, clearing Yangming heat with gypsum, moving turbid blood with finely ground gypsum, treating swelling and guiding out toxins with raw Gancao Jie (Licorice nodes), and using Gualou Zi or adding Moyao, Qingjie Ye (Green Tangerine Leaf), Zaojiao Ci (Gleditsia Spine), Jinyinhua (Honeysuckle), and Danggui Tou (Angelica Head). Whether in decoction or powder form, adjust according to circumstances, but always assist with a small amount of wine. Applying moxibustion (two to three moxa cones) on the swelling yields especially rapid results (Formulation has laws, medication has principles, which must be understood). It is pitiable that village practitioners act presumptuously, recklessly using needles and knives, aggravating the disease.
Li Shi stated: Women’s milk and men’s kidney essence are both roots of life.
If initially accompanied by irritability, thirst, and vomiting, it is Wind-Heat in the Gallbladder and Stomach. Severe cases involve toxic vapors rushing upward, obstructing the throat and diaphragm. Chills and fever indicate Liver pathogen invasion. These are exterior syndromes, best treated with Buhuanjin Zhengqi San plus Tianhuafen, which stops thirst/vomiting and regulates chills/fever. For obstruction in the throat/diaphragm, use Ganjie Tang plus fresh ginger, or Huxin San. If these four symptoms appear after ulceration, it indicates deficiency.
Rich diet, anger, and depression cause Stomach Fire to steam upward, transforming breast milk into turbid pus. Liver Qi stagnation blocks the nipple orifice, preventing nodule dissipation and causing unbearable pain. Initially, garlic-separated moxibustion is suitable; needles and knives are strictly contraindicated. Those who can drink alcohol should take Yizui Gao with Chuanxiong and Danggui (1 fen each); one dose, effective within two doses. Those who cannot drink alcohol should take Gualou San.
Nodules can also form due to Qi and Blood deficiency combined with external contraction and internal injury, leading to phlegm-blood stasis. Zhibei San is the primary formula. For Blood deficiency, combine with Siwu Tang plus Renshen, Baizhu, Chaihu, and Shengma; for Qi deficiency, combine with Sijunzi Tang plus Chuanxiong, Danggui, Chaihu, and Shengma; for Spleen injury due to worry/thought, use Guipi Tang plus Gualou Gen, Beimu, Baizhu, Lianqiao, and Gancao Jie, decocted with half water and half wine.
For severe swelling and pain due to Liver Fire nodules, use Qinggan Jieyu Tang.
"Chuan Nai" (Blown Milk) occurs when the nursing child has phlegm in the chest/diaphragm and hot breath; sleeping with the nipple allows hot breath to enter the breast, causing stagnation and pain. Initially, endure pain, massage until soft, and suckle to ensure flow for natural dissipation. If it does not dissipate, use Yiyuan San, mixed with cold ginger soup or well water, taken 30-50 times daily. For severe cases, take Dujiexiang Tang in one dose. For accompanying Qi stagnation, use Zhibei San or Danqingpi Tang. Externally, apply Loulu powder mixed with water. For non-flowing milk with breast distension/pain, use Yongquan San.
Long-standing nodules with internal distending pain and external hardness, untouchable by hand, are called "Ruyong" (Breast Carbuncle). If unulcerated, continue Gualou San or Neituo Shengma Tang, or Fuyuan Tongqi San plus Loulu. For deficiency, use Tuoli Xiaodu San. For impending ulceration (black heads between breasts, sunken black center), use Neituo Shengma Tang. For ulcerated cases with chills/fever, use Neituo Shixuan San. For poor appetite and dry mouth, use Buzhong Yiqi Tang. For afternoon/internal fever, use Bawu Tang plus Wuweizi. For Stomach deficiency with vomiting, use Liujunzi Tang plus Xiangfu and Sharen. For Stomach Cold with vomiting/diarrhea, use Liujunzi Tang plus Ganjiang and Huoxiang. For worsening pain/swelling upon exertion, use Bawu Tang with doubled Renshen, Huangqi, Danggui, and Baizhu. For worsening pain/swelling upon anger, use Bawu Tang plus Zhizi.
Wang Sun’an stated: In the year Gengwu of Longqing, I returned from the provincial exams to find my sister ill. Since July, her breast swelling and pain had not subsided. In August, fire needles were used to drain pus. The doctor prescribed Shiquan Dabu Tang and applied Tiegu San externally, but it was ineffective, and she developed wheezing and oppression. In September, she gave birth to a daughter, and the ulceration worsened, rotting the entire breast and spreading to the chest and axilla. The pus was thick and sticky, totaling nearly six to seven sheng, with no sign of healing. In November, she came to me for treatment. I switched to moderate detoxifying and harmonizing agents, applying Shengji San, Longgu, and Hanshuishi externally. Pus continued to flow profusely, causing surrounding swelling and blistering, turning purple-black on both sides, with numerous open sores. The chest and axilla were swollen and ulcerated, making movement impossible; the situation was terrifying. I reasoned that postpartum toxic vapors flared due to deficiency, and she needed large doses of Huangqi for detoxification, blood tonification, Qi supplementation, and flesh regeneration, but the doctors dared not use it. After mid-December, the condition worsened further, with many remaining sores unresponsive to various drugs. Only then did they use my prescription. The pus was foul and sticky. I boiled Zhumuye (Broussonetia leaves) and pig trotter soup to wash the area, providing immediate relief. I then prescribed Huangqi Tuoli Tang, using Huangqi (sweet and warm) as the sovereign herb to expel pus, tonify Qi, and regenerate flesh; Gancao to tonify Stomach Qi and detoxify; Danggui Shen to harmonize and generate blood as ministerial herbs; Shengma, Gegen, and Loulu as foot-Yangming meridian specific herbs; Lianqiao and Fangfeng to dissipate knots and unblock meridians; Gualou Ren and Niubangzi to detoxify and reduce swelling; Zaojiao Ci to guide medicine to the ulcer site; Baizhu entering the Yangming meridian to expel pus and grow tissue; Chuanxiong (3 fen) and Rougui fried with Baipiao as guiding herbs. Each dose was taken with one cup of wine and Baiyu Shuan Wan to drain pus and detoxify. Since the pus was thick and the disease vigorous, astringents were contraindicated; instead, Cuiqing Dingzi was applied externally. The next day, pus thinned significantly, pain ceased, and foulness resolved, indicating improvement. By the New Year of Xinwei, new flesh had grown over the affected areas. Purple-swollen areas were treated with Congxi (Scallion moxibustion), dissipating instantly. However, the Shaoyang area near the axilla had not yet healed, so I added Chaihu (1 qian), Qingpi (3 fen), and doubled Chuanxiong. As pus cleared, Sou Nong San was applied externally. She fully recovered after the Lantern Festival.
Kang Zu, magistrate of Guangde, served Zhang Wang devoutly. Later appointed to Wendi, he developed a carbuncle in the left breast, followed by a nodule in the chest/epigastrium, fist-sized and stone-hard, persisting for half a year despite all treatments. Pain radiated to the arm, axilla, and shoulder. Desperate, he prayed at the Wang Shrine and dreamed of a voice saying: "To be safe, use fresh ginger juice and processed Xiangfu." He woke, instructed his son to check the Materia Medica, found the indications matched, and consulted doctors who agreed. He used Xiangfu (dehulled, soaked overnight in ginger juice), powdered (2 qian), taken with rice drink. After a few doses, pus drained, and swelling/hardness subsided, leading to recovery.
A woman with a robust constitution and irritable temperament, suffering from long-term emotional depression, had a nodule in the left breast, slightly painful on pressure. After more than twenty doses of Lianqiao Yinzi, it receded slightly. Then, twenty doses of Bazhen Tang plus Qingpi, Xiangfu, Jiegeng, and Beimu dissolved it completely.
Xiaodu Yin: Treats Chuan Nai, Chuan Yong, and genital sores. If accompanied by aversion to cold, high fever, or headache, first take Renshen Baidu San for one to two doses before using this. If no prior symptoms, take this for two to three doses. If swelling persists, use interior-tonifying medicines.
Danggui, Baizhi, Qingpi (peeled), Beimu, Chaihu, Tianhuafen, Jiangcan (fried), Jinyinhua [each 3 qian].
Crush one dose, decoct in water, and take orally. For genital sores, add Huanglian (roasted, 1 qian), taken on an empty stomach. (Baizhi enters Yangming meridian to expel pus and relieve pain; Qingpi and Chaihu move Jueyin Qi stagnation; Danggui moves blood; Tianhuafen unblocks vessels; Jiangcan dissipates phlegm knots; Beimu opens depressed phlegm; Jinyinhua is sweet and resolves all toxins. Adding Huanglian treats genital sores by moving turbid blood in the Jueyin meridian.)
Lianqiao Yinzi: Treats Ruyong. (This specifically moves Jueyin Qi and Blood stagnation.)
Lianqiao (clears knot-heat), Gualou Ren (purges phlegm, soothes Liver), Chuanxiong (blood), Zaojiao Ci, Ju Ye (Qi), Qingpi (Qi), Gancao Jie (blood), Taoren [each 2 qian, blood].
Make one dose, decoct in water, take after meals. If ulcerated, add Renshen, Huangqi, Danggui; if unulcerated, add Chaihu, Shengma.
Another formula: Treats blood vessel stagnation and condensation forming Chuan Nai/Ruyong with unbearable pain/swelling.
Tianhuafen, Jinyinhua, Zaojiao Ci, Chuanshanjia, Danggui Wei, Baizhi Shao, Gualou Ren, Beimu (core removed), Gancao Jie.
Crush, decoct in wine, and take. (Due to stagnation, Tianhuafen, Zaojiao Ci, Chuanshanjia, and Danggui Wei are used.)
Another formula: Treats breast nodules in women.
Shengma, Lianqiao, Qingpi, Gancao Jie [each 2 qian], Gualou Ren (3 qian).
Make one dose, decoct in water, sip slowly after meals. (Shengma and Qingpi address breast nodules; Lianqiao clears knot-heat; Gancao and Gualou soothe Liver and moisten dryness.)
Neituo Shengma Tang: Treats black heads appearing between breasts, sunken black centers in ulcers, and early-stage Ruyong.
Shengma, Danggui Shen, Gegen, Lianqiao, Huangbai [each 2 qian], Huangqi (3 qian), Niubangzi, Gancao (honey-fried) [each 1 qian], Rougui (5 fen).
Make one dose, decoct with one cup water and half cup wine, take after meals. (An excellent formula for sinking lesions. Huangbai assists in lifting sinking Qi; Rougui promotes toxin expulsion.)
Qingan Jieyu Tang: Treats Liver Blood deficiency with Wind-Heat, or Liver Depression-Fire injuring Blood, causing breast nodules, or persistent swelling/ulceration. Suitable for any disharmony of Qi and Blood in the Liver/Gallbladder meridians.
Renshen (rhizome removed), Fuling, Shudihuang, Shaoyao (fried), Beimu (core removed), Zhizi (fried) [each 1 qian], Baizhu, Danggui [each 1.5 qian], Chaihu, Mudanpi, Chuanxiong, Chenpi [each 8 fen], Gancao (5 fen).
Decoct in water and take. (Wind-Heat implies Liver Heat generating Wind; Heat implies Fire within. Treatment uses Xiaoyao San plus Zhizi and Siwu Tang to nourish Liver Blood and clear Gallbladder Heat. Treating Heat is treating Wind; nourishing Blood is clearing Fire. Renshen and Baizhu should not be used lightly unless Qi is deficient.)
Another formula: Breast abscesses that rupture rarely heal without major tonification. (This formula emphasizes the "rupture" aspect.)
Renshen, Huangqi, Baizhu, Danggui, Chuanxiong, Lianqiao, Baishao, Gancao Jie.
Crush, decoct in water, and take.
Another formula contains Qingpi and Gualou, but no Baizhu. For minor rupture of Ruyan, add Chaihu. (The presence of Qingpi and Gualou without Baizhu is the key; "minor rupture" is another clue.)
Another formula: Treats hard, painful breasts.
Danggui, Gancao [each 3 qian], Moyao (1 qian).
Crush, make one dose, decoct in water with a little wine, drink hot.
Jinyinhua San: Treats blocked milk channels forming carbuncles with unbearable pain.
Jinyinhua, Danggui, Huangqi (honey-fried) [each 2.5 qian], Gancao (2.5 qian). (Using Huangqi for blocked channels requires careful consideration.)
Make one dose, decoct in water with half cup wine, take warm after meals.
Danxi Formula: Treats breast swelling and pain.
Qingpi, Shigao (calcined), Lianqiao, Zaojiao Ci (fried), Huangyaozi, Danggui Tou, Mutong [each 1 qian], Sheng Gancao (3 fen). (Huangyaozi is a key herb for clearing heat-toxin, and its combination here is appropriate, though likely not from Danxi’s own writings.)
Make one dose, decoct in water with a little good wine, take together.
Another formula: Treats Ruyong and "Nailao" swelling.
Shigao (calcined), Hua Ye (burnt), Gualou Zi, Qingpi [each ?], Gancao Jie. (Shigao and Qingpi are a pair, but the preparation differs.)
Crush, decoct in water, and take.
Another formula: Treats breast nodules.
Tiannanxing, Beimu, Gancao Jie, Gualou [each 1 liang], Lianqiao (0.5 liang).
Decoct in water, add wine, and take.
Fuyuan Tongqi San: Treats female Ruyong and all types of swelling/toxins. ("Tongqi" is excellent.)
Muxiang, Huixiang, Qingpi, Chuanshanjia (prepared with butter), Chenpi, Baizhi, Gancao, Loulu, Beimu (core removed, ginger-processed) [equal parts].
Grind into fine powder. Take 3 qian per dose, mixed with good wine.
Formula for Ruyong (Moves stagnant Qi and Blood; adding Renshen/Huangqi is another approach).
Qingpi, Gualou Zi, Ju Ye, Lianqiao, Taoren (keep tips), Zaojiao Ci, Gancao Jie.
Decoct in water and take. Add Renshen and Huangqi if ulcerated.
Shenxiao Gualou San: Extremely effective for female Ruyong and Ruyan.
Huangqi, Gualou (if many seeds, do not remove skin, bake dry and grind), Danggui (wine-washed), Sheng Gancao [each 5 qian], Ruxiang, Moyao (separately ground, 2.5 qian each). (Use of Ruxiang/Moyao originates here.)
Make one dose, simmer in three bowls of good wine in ceramic/stoneware over slow fire until reduced to half a bowl. Divide into three doses, take after meals. For Ruyan, taking this can prevent recurrence. If toxin is formed, it converts pus to yellow fluid; if not formed, it dissipates internally. For severe cases, take another dose until cured. Lixiao San is similarly effective but differs by removing Danggui and adding 1.6 liang of purple Zaojiao Ci to the Gualou San formula.
Jiuyuan Wuwu Tang: General formula for carbuncles, back sores, and Ruyong.
Gualou (fried, one piece), Zaojiao Ci (half burnt, half raw), Moyao, Ruxiang, Gancao [each 0.5 liang].
Crush, decoct in three sheng of pure wine to yield two sheng. Drink frequently. Stops unbearable pain immediately.
Gualou San: Treats Chuan Nai.
Ruxiang (1 qian, ground), Gualou (one piece; alternative: root 1 liang).
Crush, decoct in wine and take. Or grind into powder, take 2 qian with warm wine. Externally, apply Tiannanxing powder mixed with warm water.
Juxiang San: Treats Ruyong that dissipates if not knotted, or ulcerates if knotted, with extreme pain relieved immediately by the medicine. Highly effective. Also treats diseases arising from infant "blown milk."
Chenpi (white part removed, fried in dry flour until yellow, powdered, 1 liang), Shexiang (1 fen).
Grind uniformly, take 2 qian mixed with wine. Cover with blanket to induce sweating for cure.
Supplemental Formula: Treats solid, swollen, painful Chuan Nai.
Chenpi (2 liang) (Chenpi also enters Jueyin), Gancao (1 qian).
Crush, decoct in water, divide into two doses. Use Jingjie, Qianghuo, and Duhuo decoction to fumigate and wash, causing dissipation.
Another formula: Treats Ruyong.
Yeming Sha, Gualou (fried), Awei.
Grind into powder, make pills with rice paste, swallow with wine. (No specific dosages given; generally, Yeming Sha is sovereign to break blood knots, Gualou is minister to moisten phlegm knots, and Awei is assistant to soften hardness and break stagnation.)
Tonghe Tang: Treats female Ruyong with unbearable pain.
Chuanshanjia (fried yellow), Mutong [each 1 liang], Ziran Tong (0.5 liang, quenched in vinegar seven times). (Does Ziran Tong unblock stagnation?)
Grind into powder. Take 2 qian per dose, mixed with hot wine, taken away from meals.
Erhui San: Treats postpartum failure of milk ejection, stagnant knots not dissipating, and heat-toxin.
Manjingzi (burnt), Zaojiao Ci (burnt) [equal parts].
Grind into powder. Take 2 qian per dose, mixed with warm wine, anytime.
Yeyin San: Treats Chuan Nai and Ruyong.
Spider (three), Red Dates (three, pitted).
Place one spider inside each date, roast until cooked, chew and swallow with liquor. Unformed nodules dissipate immediately; formed ones ulcerate immediately.
Dusheng San: Treats early signs of female Chuan Nai: body heat, headache, chills/fever, and chest/breast hardness. Taking this promotes milk flow, unblocks vessels, and causes immediate self-dissipation.
Baidingxiang (authentic).
Grind into powder. Take 2 qian per dose, mixed with wine. Hardness/swelling dissipates immediately; severe cases resolve within three doses.
Another formula: Excellent for female Chuan Nai with hardness/swelling, fever/chills, unbearable pain, and loss of appetite.
Deer Antler (1 liang) (enters Jueyin Meridian). Calcine until charred but retain nature, grind fine. Divide into two doses. Put 5 qian of powder in pot, add one bowl of pure wine, boil briefly, pour into bowl, drink hot while covered to induce sweat for relief.
Another formula: Deer antler ground to fine powder, mixed with wine (2-3 qian), is also effective.
Another formula: Grind deer antler on rough stone to extract white juice, apply topically (grinding is better). Reapply when dry.
Shengjin Dan: Extremely effective for Chuan Nai nodules that do not dissipate, causing swelling/pain; also treats Ruyan.
Baichi Shuang (scum from hairbrush teeth).
