The Yellow Emperor asked: When the Foot-Yangming meridian is diseased, the patient dislikes people and fire, and is startled upon hearing the sound of wood, yet is not moved by the sound of bells and drums. Why does the sound of wood cause alarm? Qibo replied: The Yangming is the stomach meridian; the stomach belongs to Earth in the Five Elements. Therefore, the alarm caused by the sound of wood occurs because Earth fears Wood (Wood overcomes Earth). The Yellow Emperor asked: Why does he dislike fire? Qibo replied: Yangming governs the flesh, and its meridian has abundant blood and qi. When pathogenic factors invade, heat is generated; when the heat is severe, the patient dislikes fire. The Yellow Emperor asked: Why does he dislike people? Qibo replied: Reversal of Qi in Yangming leads to panting and mental depression/distress; due to this distress, the patient dislikes people.
When pathogenic heat enters the central Earth of Yangming, Earth fears Wood, hence the alarm at the sound of wood; due to severe heat, there is a dislike for fire, which corresponds to Zhang Zhongjing’s statement of "no aversion to cold but aversion to heat"; when pathogenic factors bind and Qi reverses, panting and distress occur; distress refers to inner restlessness (Ao Nao), hence the dislike for people.
The Yellow Emperor asked: Some patients die from panting, while others survive; why is this? Qibo replied: If reversal extends to the Zang organs, death ensues; if it extends only to the meridians, survival occurs.
Reversal (Jue Ni) refers to pathogenic factors binding in the Stomach Fu organ, preventing Yang Qi from reaching the four limbs, resulting in cold extremities. This is known as the false cold syndrome of "extreme Yang resembling Yin." If pathogens enter inward, they connect with the Zang organs, causing Yang Qi to sink, leading to death; if pathogens move outward, they connect with the meridians, allowing Yang Qi to extend, leading to survival.
The Yellow Emperor asked: In severe cases, patients discard their clothes and run, climb high places and sing, or go without food for several days, climbing walls and roofs—places they could never reach normally. Why can they do things contrary to their usual abilities? Qibo replied: The four limbs are the root of all Yang meridians. When Yang is excessive, the four limbs become solid and strong, enabling one to climb high. The Yellow Emperor asked: Why do they discard clothes and run? Qibo replied: Because heat is excessive in the body, they discard clothes and wish to run. The Yellow Emperor asked: Why do they speak nonsensically, curse, disregard kinship and distance, and sing? Qibo replied: Excessive Yang causes nonsensical speech, cursing, and disregard for relationships; combined with loss of appetite, this leads to aimless running.
The four limbs receive Qi from the Stomach; the Stomach is the sea of the Zang and Fu organs, and the Yangming meridian circulates Qi through the three Yang meridians; thus, the four limbs are the root of all Yang. When pathogens are excessive in the Stomach, Qi becomes solid in the four limbs, enabling climbing; when body heat is excessive, clothes are discarded; when pathogens disturb the Shen (Spirit), anger and impulse arise, causing nonsensical speech and cursing; when the Stomach contains pathogenic excess, appetite is lost; when the four limbs have strength, aimless running occurs.
