Ju Fang Wu Ji Wan (Local Formulation Wuji Pill) treats persistent diarrhea and dysentery caused by damp-heat in the Spleen and Stomach (see formula under "Dysentery" section).
Commentary: In formulas for treating diarrhea, it was traditionally mostly considered a Cold pattern. It was not until Liu Wansu and Zhang Yuansu that they pointed out there are cases belonging to Heat or Fire patterns. Therefore, among various formularies, formulas specifically treating Heat-type diarrhea are very few. However, Zhang Zhongjing used Gegen Qinlian Tang (Kudzu, Scutellaria, and Coptis Decoction) for treating dysentery with heat accompanying Shaoyang disease (Taiyang disease), but people have rarely pondered this point.
Ju Fang Laifu Dan (Local Formulation Laifu Pill) treats diarrhea resembling water due to latent summerheat (see formula under this section).
Yi Jian Fang states: Shixiao (Niter) is cold in nature; assisted by Chenpi (Tangerine Peel), its qi moves freely and smoothly. Huangqi (Sulfur) can also benefit the body. If one mistakenly uses it as a warming medicine to stop diarrhea, it is an error. This occurs because consuming raw/cold foods or being exposed to summerheat qi causes closure and stagnation in the Middle Jiao (Epigastrium), leading to sudden vomiting and diarrhea with rapid changes. This medicine unblocks the Sanjiao (Triple Burner) and regulates Yin and Yang; taking it yields very effective results.
Commentary: This comes from the precedent of using Sulfur in the Jueyin chapter.
Xiangru Yin (Mosla Decoction) treats vomiting and diarrhea due to latent summerheat (see formula under "Summerheat Agents" section).
Commentary: This is a surface-releasing formula for treating summerheat. It acts on the Lung and Spleen meridians, derived from the precedent of Houpo (Magnolia Bark) in the Taiyin chapter.
Xuanming Guiling Ganlu Yin (Revealing Clarity Cinnamon, Atractylodes Macrocephala, Poria, Alisma, Talcum, and Rock Salt Decoction) treats inability to digest ingested water, vomiting, diarrhea and dysentery, or dysentery with red and white discharge (see formula under "Summerheat Agents" section).
Commentary: This consists of Wuling San (Five-Ling Powder) and Yiyuan San (Benefit Origin Powder) plus Hanshuishi (Aragonite) to resolve damp-heat. It belongs to the Taiyang meridian agents.
Jiyao Jiangshui San (Essential Secrets Porridge Water Powder) treats acute diarrhea like flowing water, sweating over the whole body, coldness throughout the body, faint and weak pulse, shortness of breath and inability to speak, and severe cases accompanied by vomiting. This is an urgent condition.
Herbs and Dosages: Banxia (Pinelliae Rhizoma, 1 liang, washed with hot water); Fuzi (Aconiti Lateralis Radix Praeparata, 0.5 liang, processed); Guizhi (Cinnamomi Ramulus, 5 qian); Gancao (Glycyrrhizae Radix et Rhizoma Praeparata cum Melle, 5 qian, honey-fried); Liangjiang (Alpiniae Officinarum Rhizoma, 2.5 qian); Ganjiang (Zingiberis Rhizoma, 5 qian, sometimes recorded as dried fresh ginger).
Preparation and Administration: Grind the above herbs into fine powder. Take 3-5 qian per dose, add two zhan of Jiangshui (fermented porridge water), decoct until half a zhan remains, and take warm including the dregs.
Baizhu Tang (Atractylodes Macrocephala Decoction)
Herbs: Baizhu (Atractodis Macrocephalae Rhizoma); Shaoyao (Paeoniae Radix Alba); Gancao (Glycyrrhizae Radix et Rhizoma Praeparata cum Melle, 3 qian each, honey-fried); Ganjiang (Zingiberis Rhizoma, 0.5 liang, processed).
Administration: Same as above. For severe cases, remove Ganjiang and add Fuzi (Aconiti Lateralis Radix Praeparata) 3 qian, because pungent substances can disperse.
Ju Fang Lizhong Tang (Local Formulation Regulate the Middle Decoction) treats continuous diarrhea due to lingering cold in the Zang-Fu organs (see formula under "Cold Agents" section).
Commentary: This comes from the precedent of internal cold in the Jueyin chapter. The aforementioned medicines using Ginger and Aconite are all emergency rescue agents for Qi collapse caused by acute diarrhea, such as in "Wu Xu" (Five Deficiencies) syndromes. Acute diarrhea is not necessarily a Yang syndrome. However, some cases belong to Fire patterns, and their root causes must be investigated.
