(The so-called "similar syndromes" refer to diseases that present symptoms resembling leprosy but have different etiologies.)
Some conditions resemble tinea (fungal skin infection), causing itching upon scratching. If prolonged, it may weep pus and fluid, or produce white scales after scratching. This condition is named "Kidney Wind" (Shen Zang Feng). Treatment should employ Si Sheng San (Four Generations Powder) to dispel wind pathogens; if accompanied by yin deficiency with internal fire, Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill) may be used; alternatively, Qiang Huo Bai Zhi San (Notopterygium and Angelica Dahurica Powder) or Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder) can be selected. For patients with Qi deficiency, Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) supplemented with Gleditsiae Spina (Zao Jiao Ci) and Uncaria Ramulus Cum Uncis (Gou Teng Gou) should be administered as an adjunct. For those with Blood deficiency, Ba Wu Tang (Eight Substance Decoction) added with Bupleuri Radix (Chai Hu) and Moutan Cortex (Mu Dan Pi), or concurrently taken with Jia Wei Xiao Yao San, is indicated.
In some cases, rashes appear on the limbs, particularly the legs, arms, and wrists, migrating不定ly in location. If red, it is called "Red Roaming Wind" (Chi You Feng); if white, "White Roaming Wind" (Bai You Feng). These arise from Blood deficiency leading to internal stirring of Yin-fire. Treatment involves Xiao Yao San (Free and Easy Wanderer Powder) and Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill).
When eruptions or nodules appear all over the body or on the head and face, resembling patches of rosy clouds, or when they rupture and weep pus and fluid, accompanied by itching, pain, aversion to cold, and fever, this indicates Liver Fire combined with Blood deficiency. Treatment uses Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder). If the syndrome involves phlegm-dampness obstructing the chest and diaphragm, Xiao Chai Hu Tang (Minor Blue Green Dragon Decoction) added with Gardeniae Fructus (Shan Zhi) and Rehmanniae Radix Praeparata (Sheng Di Huang) is used. For Blood deficiency, Si Wu Tang (Four Substance Decoction) combined with Xiao Chai Hu Tang is prescribed; once the illness recedes, Xiao Yao San is employed to strengthen the Spleen and Stomach to generate Yin and Blood. This condition often arises from anger, so treatment requires careful examination.
A female patient presented with urticarial wheals and nodules on her limbs, which became ulcers upon scratching, weeping pus and fluid. She suffered from heat, thirst, dizziness, and worsening symptoms in the late afternoon. Treatment utilized Si Wu Tang (Four Substance Decoction) added with Bupleuri Radix (Chai Hu) and Gardeniae Fructus (Shan Zhi). If spontaneous sweating, night sweats, irregular menstruation, and abdominal pain occurred, Ba Zhen Tang (Eight Treasures Decoction) and Liu Wei Wan (Six-Ingredient Pill) were used. If there was poor appetite, fatigue, palpitations, night sweats, amenorrhea, and alternating chills and fever, Ba Zhen Tang (Eight Treasures Decoction) with Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder) as an adjunct was indicated. In cases of long-standing disease with weak Yuan Qi (Primary Qi), Shi Quan Da Bu Tang (Ten Great Tonics Decoction) with Gui Pi Tang (Restore the Spleen Decoction) as an adjunct was prescribed.
A girl aged thirteen to fourteen or sixteen to seventeen, whose Tian Gui (Menstrual Essence/Menses) had not yet arrived, or a woman with irregular menstruation presenting with red, itchy, and painful skin lesions, belongs to the category of Liver Fire and Blood Heat. Treatment employs Xiao Chai Hu Tang (Minor Blue Green Dragon Decoction) added with Gardeniae Fructus (Shan Zhi), Rehmanniae Radix Praeparata (Sheng Di Huang), Moutan Cortex (Mu Dan Pi), and Saposhnikoviae Radix (Fang Feng).
The presence of parasites in such lesions is due to Ministerial Fire (Xiang Huo) restraining Lung Metal, failing to control Liver Wood, resulting in transformation into worms; it is not generated by Wind pathogens. The treatment focuses on nourishing Kidney Water to generate Liver Blood, or adjunctively using moxibustion at Chengjiang (CV 24), as detailed in the relevant section.
If arsenic trioxide (Pi Shuang) is applied topically and causes pain or ulceration at the site, wet mud should be frequently applied and changed. If the toxin enters the abdomen, causing distress in the chest and epigastrium, or vomiting and diarrhea, drinking one or two cups of cold rice vinegar will stop the symptoms; excessive intake is also harmless. Consuming raw mung bean powder or sesame oil is also effective.
If application of Realgar (Xiong Huang) causes mental confusion, vomiting, or diarrhea, Stephaniae Tetrandrae Radix (Fang Ji) decocted in water serves as an antidote.
If ingestion of acrid-hot herbs leads to loss of eyebrows and hair, this indicates injury to Liver Blood with stirring of deficient fire, rather than Wind pathogen. Treatment uses Si Wu Tang (Four Substance Decoction) and Liu Wei Wan (Six-Ingredient Pill) to nourish Liver Blood and generate Kidney Water.
If ingestion of Aconiti Lateralis Radix Preparata (Chuan Wu) or Aconiti Kusnezoffii Radix (Cao Wu) causes mental confusion, salivation, coma, vomiting, bleeding, or hematemesis, any single herb among Glycine Max Seed (Da Dou), Polygalae Radix (Yuan Zhi), Saposhnikoviae Radix (Fang Feng), or Glycyrrhizae Radix et Rhizoma (Gan Cao) decocted in water can serve as an antidote. Generally, for excessive use of Wind-dispelling herbs, this method is appropriate. If ineffective, urgently administer Gan Cao (Licorice Root) and fresh ginger juice to detoxify.
If application of Crotonis Fructus (Ba Dou) causes pain and muscle ulceration at the site, Raw Coptidis Rhizoma (Huang Lian) ground into powder and mixed with water for topical application is used. If the toxin penetrates the flesh causing vomiting and diarrhea, the same preparation taken orally (one or two qian) is effective. Adzuki beans, mung beans, and Acori Tatarinowii Rhizoma juice are also suitable.
If Veratri Nigrum Rhizoma (Li Lu) is applied and the toxin enters internally, Scallion Juice decocted in water serves as an antidote.
If wind-dispelling and draining herbs cause vomiting, reduced food intake, chest and diaphragm discomfort, or general fatigue and weakness, Liu Jun Zi Tang (Six Gentlemen Decoction) is used to tonify Yang Qi. If irritability, thirst, anorexia, afternoon fever, internal heat, flushed face, and fever occur, Si Wu Tang (Four Substance Decoction) added with Ginseng Radix (Ren Shen) and Atractylodis Macrocephalae Rhizoma (Bai Zhu) is used to generate Yin and Blood. Other cases should be treated according to their respective categories.
