The forehead (Liangshang) scabbing first with a ruddy and vibrant color is an auspicious sign; scabbing first on the ears indicates death. To understand the rulership of the Eight Trigrams, one must recognize the distribution of smallpox lesions on facial areas to predict fortune and misfortune like a true immortal.

The Heart corresponds to Bing Fire in the South; red lesion color is auspicious. The Liver corresponds to Jia Wood in the East; green lesion color is auspicious. The Spleen corresponds to Wu Earth in the Center; yellow lesion color is favorable. The Lungs correspond to Geng Metal in the West; white lesion color indicates excess; round and bright appearance is excellent. (If initially tender white, Su Ge Tang should be used.) The Kidneys correspond to Ren Water in the North; pale white like pearls indicates a ominous sign. (Si Wu Tang and Si Jun Zi Tang modified are indicated, using Southern Dates as a guide herb.)

(At the time of seeing the marks, one glance at severity predicts fortune or misfortune.)
The song states: Ear-posterior vein patterns faintly red, sparse and distinct ensure no disaster; if purple, black, cyan, or white, even Bian Que and Hua Tuo would waste their efforts.
Heart Meridian: A single red vein behind the ear, without branches extending upward, indicates Heart Meridian smallpox; sparse lesions on the head and face are certainly good.
Liver Meridian: Ear veins purple-black resembling fish bones, moving inward, indicate Liver Meridian smallpox; widespread red spots leading to darkness by the eighth day.
Spleen Meridian: Ear veins waxy-cyan indicating sparse smallpox, large heads and pointed tails unknown to people, indicate Spleen Meridian smallpox; arranged outwardly in a unique pattern.
Lung Meridian: Ear veins pale white chaotic like hemp, crawling outward, indicate Lung Meridian smallpox; shaped like silkworm eggs, sighing by the seventh day.
Kidney Meridian: Ear veins chaotic purple-black cyan, black smallpox carbuncles certain on waist and back, indicate Kidney Meridian smallpox; overflowing ears, collapsing on the ninth day.

Earlobe belongs to Stomach, ear leaf to Spleen, inner ear wheel to Heart, ear skin/flesh to Lungs, ear canal to Kidneys, and ear back Wanglou area to Liver.

The upper ear belongs to Heart Fire, favoring fresh redness and strong heat, not suitable for black/white coldness. If plum-blossom patterns appear, or string-like patterns emerge from ear skin, these are adverse signs.
The lower ear belongs to Kidney Water, favoring red-purple with coldness, not suitable for pale yellow strong heat. If plum-blossom patterns appear, it is actually a favorable condition; if resembling sesame or flea bites, these are ominous signs. Seeing such lesions implies black smallpox carbuncles at Shenshu (BL23).
The inner ear belongs to Lung Metal, favoring pale white with warmth, not suitable for fresh red/cyan strong heat. If resembling dogwood shapes or fire-brand burn marks, these are death omens. Such lesions必然 cause dry nose/mouth, phlegm-wheezing choking throat; lesions should be sparse, large, pale white-peach red for best outcome.
The outer ear belongs to Liver Wood, favoring green-cyan warmth, not suitable for pale white coldness. Dense lesions indicate misfortune, sparse few grains indicate good fortune.
The center belongs to Spleen Earth, favoring gray-yellow mildness, not suitable for green/strong heat. Sparse yellow-wax color is auspicious; dense like ants with cyan color is definitely ominous. (Song: Upper ear Metal/Wood/Water/Fire, doctors ignorant suffer greatly; knowing this method spreads worldwide, serving nobles and officials.)

Generally, three ear-posterior veins with many branches, faint red color, are auspicious, indicating Heart Meridian smallpox will be good, head/face lesions sparse.
Generally, ear-posterior veins purple-red indicate Liver Meridian smallpox will be ominous; rapid eruption is unfavorable; if not receding, tidal fever follows, leading to five poisons and nine grinds.
Generally, ear-posterior veins gray-yellow, or vein head large root small, indicate Spleen Meridian smallpox, head/face/chest/abdomen lesions sparse.
Generally, ear-posterior veins pale white, branches chaotic, indicate Lung Meridian smallpox, erupting together like silkworm eggs, causing itching collapse, extremely ominous untreatable sign, death by scratching within 3-5 days.
Generally, ear-posterior veins black with many branches, indicate Kidney Meridian smallpox, black depression certain, black smallpox on waist/back, scorched seedlings depressed, carbuncle poison causes death within 9-10 days.

Any fever with ear veins appearing purple/black/red/white is ominous; cool upper ear is auspicious, cool lower ear is ominous. I now only prefer faint red, this method is usable. Listening to voice knows syndrome: speaking hotly boiling, lesion colors mostly red-purple, stool not closed hard, voice trembling cold certain, stool diarrhea several times, clear voice evidence, soon self-peace, hoarse voice throat surely rots, fast voice poison gas boils, food choking throat out, instantly down nine springs. Generally, erupting smallpox must transmit from Kidney to Heart, to Lungs, to Liver, to Spleen to heal.

Cool hands, cool lower ears indicate severe condition. Warm/level hands/ears indicate 20-30% smallpox. White face indicates Lung Meridian wind-cold phlegm-wheezing, 50% smallpox. Cyan face indicates diarrhea, 40-50% smallpox. (If diarrhea at onset, urgently use Si Wu Tang plus Doukou Wan, mixed with brown sugar rice drink.) Purple-red face indicates 80-90% smallpox. (Or causing itching collapse/diarrhea, or vomiting, surely dies from itching collapse.) Jiache swelling indicates toxins in chest/abdomen/back. Lesions inside mouth corners: male Yang orifice has smallpox carbuncle, female Yin orifice has smallpox carbuncle, indicates death on eighth day. Vein patterns at both Taiyang temples indicate eye smallpox. (Urgently remove eye toxin, explode visible vein patterns with lamp cautery, then grind away eye smallpox.) Purple blisters/red patterns under Yongquan (KI1) indicate Heart smallpox carbuncle. Blue veins rising behind ears indicate convulsions. Black veins crossing hairline behind both ears indicate umbilical pain, Kidney Qi pain. Gray ears black nose indicate urinary obstruction. Dry nostrils coarse breath indicate [missing term]. Cyan nose indicates milk vomiting. Black Yintang indicates night crying or fetal cold exposure. Cyan Renzhong indicates anorexia, difficult stool. Red nasal tip indicates Heart heat, carbuncle/furuncle toxin or night crying. Dry nose indicates death. (If red nose, there is still hope for life.)
