To treat conditions such as wind-heat with phlegm turbidity obstructing the interior, mumps (Zhang Sanlei's term for parotitis), erysipelas of the neck/face, epidemic toxic heat, phlegma nodules (subcutaneous nodules), scrofula (tuberculous lymphadenitis of the neck), swelling and pain with ulceration of the throat, lung abscess (pulmonary suppurative infection), stomach abscess (gastric suppurative infection) with expectoration of pus and blood, and various other syndromes.
Xiangshan Fumu (Fritillariae Thunbergii Bulbus, 6 liang) Guanxi Niuhuang (Bovis Calculus, 5 qian) Touming Yaohuang (Realgar from Yaozhou, 2 liang)
Grind each of the above into an extremely fine powder. Dissolve Chendanxing (Arisaema cum Bile, prepared with bovine bile) in good yellow wine, pound and mix to form pills the size of soybeans, coated with Chen sha (Cinnabar). Store tightly sealed to prevent air penetration; do not store together with Duanshi (calcined minerals). Take 3, 5, or 7 pills per dose, chew thoroughly, and swallow slowly.
[Formula Analysis] The various syndromes treated by this pill are all caused by wind-heat binding with phlegm, condensing and failing to dissipate. The formula employs methods to clear heat and detoxify, and to open up phlegm obstruction. It heavily uses wind-dispelling herbs, which might seem to stir up phlegm-heat and assist its upward movement, inevitably leading to adverse effects. This is the profound intent of the formula formulation, not a defect. The astringent property of Wu beizi (Galla Chinensis) is used to prevent the pathogenic factors from scattering and wandering when the fire-heat is at its peak. This is also the key difference between pills and decoctions. If Wu beizi were used in a decoction, it would be a major error.
For treating severe sores with great toxicity where fire-heat is raging, and constipation is present, use this formula. Once the bowel movements are unblocked, the pathological momentum will naturally subside.
Ming Xionghuang (Realgar, 1 liang) Sheng Jinwen (Rhei Radix et Rhizoma, raw, 2 liang) Badou Shuang (Crotonis Fructus Frost, extracted white flesh, oil pressed out, 4 liang)
Grind the above three ingredients into fine powders separately. Add a small amount of Feimian (refined wheat flour), 5-6 qian, and pound together with rice vinegar to form pills the size of Fengxianzi (Pharbitidis Semen). Take 5 to 7 pills per dose. For the most severe cases, do not exceed 12 pills; do not use more. Swallow with warm boiled water. After one or two episodes of diarrhea, prepare several mouthfuls of cold Menvdou Qingtang (mung bean clear soup) to stop the diarrhea. Contraindicated for persons with deficiency and pregnant women. For children with excess syndrome of phlegm and food retention, presenting with fever and constipation, use 2-3 pills per dose, grind finely and feed. One episode of diarrhea leads to cure.
[Formula Analysis] Ding du (carbuncle toxins) all belong to excess-heat syndromes. Obstruction of the intestinal pathway causes fire-heat to rush upward, making the toxicity even more intense. This formula corresponds directly to Beiji Wan (Emergency Rescue Pill) by Zhang Zhongjing, addressing the condition head-on. Children can also use this formula because their condition involves extreme excess and obstruction, allowing for purgative therapy to drain the pathogen. Moreover, the dosage is small, so the harshness of the medicine is not a concern. Baochi San (Infant Protection Powder), popular in Kyoto, consists of Badou Shuang (Croton frost) and Zhusha (Cinnabar). People know its efficacy but do not recognize its ingredients. If the formula were explicitly revealed, people would likely be shocked and consider it strange. Note that taking only one li (an extremely small unit) per dose causes no harm; the caution is against frequent use which could damage the Spleen and Stomach. In recent years, a certain pediatrician in Shanghai also self-prepared a generic pediatric pill named [Redacted] Dan. I have seen his medicine; it is slightly red in color and should be the same as the product from Kyoto. This gentleman is well-known in pediatrics and has rich experience, yet he also uses this medicine. This indicates that the medicine does not cause disaster.
