Foul odor emanating from the mouth makes it difficult for others to approach. If it were not for toxic fire scorching within, how could the halitosis be so severe?
Tongue coating can distinguish between exterior and interior patterns. A coating white as frost indicates extreme cold. In *Shanghan* (Cold Damage), this should be treated with warming methods to release the exterior; however, in epidemic diseases, a frost-like coating presents with a thick and enlarged tongue, which is a manifestation of extreme heat transforming into water-like signs. Misusing warm exterior-releasing herbs will cause the tongue color to turn black shortly. The *Ling Shu* states: "Black tongue in heat syndromes reflects the color of the Kidney." The Heart opens into the tongue; if Water and Fire clash (failure of mutual interaction), death is inevitable. I have treated many such cases without any deaths, showing that ancient physicians also had limitations in understanding, simply lacking this specific method!
The throat receives Qi and connects with Heaven; the pharynx receives food and connects with Earth. Together, they are the pathways for food and fluids and the channels for the ascending and descending of Qi. Swelling and pain indicate blockage at both ends. Fear of using cooling agents causes profound harm.
The lips are the flourishing aspect of the Spleen, serving as the gateway for food intake and excretion, and closely related to respiration. Swelling and inability to move freely indicate Heat in the Spleen.
Blisters resemble burns, varying in size, appearing red, white, or intermixed with purple-black. They are unbearably painful, weeping clear fluid upon rupture, or sometimes bloody fluid. Treatment is the same as for Da Tou Wen (Great Head Plague). (Empirical experience)
The head is the summit of all Yang meridians; its swelling is abnormal. This is due to toxic fire following Yang energy attacking upward, causing the head to swell.
The cheeks belong to the Liver and Kidney. Swelling may start on the left, then the right, or spread from right to left or left to right. If not cleared and resolved promptly, it will develop into Da Tou Wen.
The neck belongs to the Foot-Taiyang Bladder Meridian. The Shaoyin Kidney Meridian and the Bladder Meridian are exterior-interior pairs. When heat-toxin enters the Taiyang, neck swelling occurs.
The area behind the ears belongs to the Kidney Meridian. Toxic eruption here indicates a more malignant disease, requiring immediate clearing and dispersing. Hemorrhage from the ear is incurable.
The tongue is the sprout of the Heart. When the Heart is tranquil, the tongue is still; when the Heart is disturbed, the tongue moves. In the Bagua, the Heart corresponds to Li (Fire). It communicates downward with the Kidney, receiving assistance from Kan (Water) to form Ministerial Fire (Jun Huo). Without external stimulation, it remains naturally tranquil. However, under the assault of toxic fire, scorching the Shaoyin, Fire meets Fire, and the two Fires combine. The Heart cannot remain tranquil; thus, symptoms like protruding the tongue (Da She) are unavoidable?
