Qin Zi said: The symptoms of Huoluan (cholera-like disease with sudden vomiting and diarrhea) include severe cramping pain in the heart and abdomen, upward vomiting, downward diarrhea, restlessness, irritability, and chest oppression. In severe cases, it manifests as tendon spasms (Zhuanjin). From the perspective of meridians, Huoluan primarily involves the Yangming Stomach and Intestines. If there is only vomiting and diarrhea without abdominal pain or restlessness, it is merely ordinary vomiting and diarrhea, not Huoluan. Here, six types caused by external pathogens are listed, and four types caused by internal injuries are listed. (For cases where external and internal factors coexist, detailed treatment methods will be provided later.)
[Symptoms of Dampness-induced Huoluan] It is neither caused by dietary injury nor by emotional disturbance (Seven Emotions). Instead, it arises from the excessive dampness of the seasonal climate, causing sudden, chaotic vomiting and diarrhea of water or undigested food. This is a condition of dampness stagnation due to the external pathogen affecting the Earth element. As stated in *Neijing* (The Inner Canon): "When Taiyin (Spleen/Stomach) dominates, and Earth qi stagnates and erupts, the people suffer from Huoluan, vomiting, and profuse diarrhea." This refers to this specific condition.
[Etiology of Dampness-induced Huoluan] When Damp-Earth governs the season, the Qi of Dampness becomes excessive. The Spleen and Stomach belong to Earth; they dislike Dampness and prefer Dryness. Now, because the Qi of Damp-Earth is excessive, it causes internal stagnation.
[Pulse of Dampness-induced Huoluan] The pulse may appear Deep and Hidden (Chen Fu), Rapid with irregular pauses (Cu), Regularly intermittent (Dai), Knotted (Jie - slow with irregular pauses), or Soft and Floating (Ru Ruan).
[Treatment of Dampness-induced Huoluan] Zhang Zhongjing used Wuling San (Five-Ingredient Powder with Poria). Currently, Pingwei San (Stomach-Calming Powder) and Zhengqi San (Righteous Qi Powder) are extended, added with Qing Huoxiang (Green Patchouli). If sweating is indicated for release, Fangfeng Shengshi Tang (Saposhnikovia Decoction for Overcoming Dampness) is used.
Wuling San: Treats conditions without vomiting or diarrhea (Note: Original text context implies specific indications, though clinically often for fluid metabolism issues).
Ingredients: Atractylodis Macrocephalae Rhizoma (Baizhu), Polyporus (Zhu Ling), Alisma Rhizoma (Zexie), Cinnamomi Ramulus (Rougui), Poria (Fuling).
Pingwei San: See section on Damp Diarrhea. Treats stomach nausea/vomiting.
Buhuanjin Zhengqi San: Treats exterior pathogenic heat.
Ingredients: Atractylodis Rhizoma (Cangzhu), Magnoliae Officinalis Cortex (Houpo), Citri Reticulatae Pericarpium (Chenpi), Glycyrrhizae Radix et Rhizoma (Gancao), Costi Rhizoma (Muxiang), Pogostemonis Herba (Xian Huoxiang).
Fangfeng Shengshi Tang
Ingredients: Saposhnikoviae Radix (Fangfeng), Schizonepetae Herba (Jingjie), Atractylodis Rhizoma (Cangzhu), Angelicae Dahuricae Radix (Baizhi), Notopterygii Rhizoma et Radix (Qianghuo), Chuanxiong Rhizoma (Chuanxiong).
[Symptoms of Wind-induced Huoluan] There is no dietary injury or emotional disturbance. It is solely due to the excessive Wind pathogen of the season, presenting with headache, fever, upward vomiting, downward diarrhea, heart and abdominal pain, and in severe cases, tendon spasms. This is a condition of excessive Wood (Wind) qi. As stated in *Neijing*: "When the yearly Earth qi is insufficient, Wind qi prevails greatly, and the people suffer from Huoluan and incomplete digestion diarrhea (Sunxie)." This refers to this condition.
[Etiology of Wind-induced Huoluan] The yearly Earth qi is insufficient, while Wind-Wood qi is excessive. Wind-Wood overacts upon the Central Earth (Spleen/Stomach), leading to Wind invasion and Wood attacking Earth. Consequently, food and fluids fail to transform, resulting in Huoluan and Sunxie.
[Pulse of Wind-induced Huoluan] Floating and Tight pulse indicates Wind-Cold; Floating and Rapid pulse indicates Wind-Heat; Floating and Soft pulse indicates Wind-Dampness. A Floating pulse at the Left Guan position indicates Wind-Wood pathogen; a Floating pulse at the Right Guan position indicates... [text cuts off, implying Spleen/Stomach involvement].
[Treatment of Wind-induced Huoluan] For Wind-Cold, use Baidu San (Ten-Antidote Powder); for Wind-Heat, use Jiami Shenzhu Tang (Secret Formula Divine Atractylodes Decoction); for Wind-Dampness, use Haizang Shenzhu Tang (Sea Storehouse Divine Atractylodes Decoction). For Wind-Wood pathogen, use Chaihu Fangfeng Tang (Bupleurum and Saposhnikovia Decoction); if accompanied by food stagnation internally, use Jingfang Pingwei San (Schizonepeta and Saposhnikovia Stomach-Calming Powder).
