The forehead, heart region, chin, kidney area, nose, and spleen zone correspond to specific locations. The left cheek is the position of the Liver (Gan), and the right cheek is the region of the Lung (Fei). The distribution across the body primarily associates the face with the Stomach (Wei) meridian; the head and back belong to the Bladder (Pangguang) meridian's domain, while the waist belongs to the Kidney (Shen) scope. The Heart (Xin) and Lungs reside in the chest and diaphragm areas; the Liver and Gallbladder (Dan) are located in the hypochondriac regions; the abdomen and limbs belong to the Spleen (Pi) as well as the Large Intestine (Dachang) and Small Intestine (Xiaochang). The collateral vessels of the Pericardium (Baoluo) connect the Zang-Fu organs, while the Qi of the Sanjiao (Triple Burner) permeates the entire body without fixed boundaries.
[Note] When children develop smallpox or chickenpox (Douzhen), from the head and face down to the whole body, each part corresponds to specific Zang-Fu organ regions. Practitioners must carefully observe these locations to determine auspiciousness or ominous signs. For instance, if spots first appear on the forehead, the toxin originates from the Heart; if on the chin, from the Kidneys; if on the left cheek, from the Liver; if on the right cheek, from the Lungs; if on the nose, from the Spleen. The face belongs to the Stomach meridian; the head and back belong to the Bladder meridian; the waist belongs to the Kidney meridian; the chest and diaphragm belong to the Heart and Lung meridians; the hypochondriac sides belong to the Liver and Gallbladder meridians; the abdomen and limbs belong to the Spleen, Large Intestine, and Small Intestine meridians. As for the Pericardium, it consists of the collateral vessels of the subcutaneous fat membranes throughout the body, connecting the bones and Zang-Fu organs. The Sanjiao contains the essence-Qi of water and grain throughout the body, filling the shell and organs. Since all rashes erupt from here, there is no single fixed location.
Regarding the shape of the rash, when spots appear, a pointed and round form is favorable, whereas forms resembling mosquito bites, heat pruritus (Maire), cold chills, or silkworm seeds are unfavorable. During the swelling stage, gradual expansion and plumpness indicate a favorable course (Shun); a flat, non-protruding, hard, and solid texture indicates difficulty in treatment. During the pustule formation stage, tight contraction and intact appearance indicate favorability; soft, thin-skinned, itchy, and collapsed textures indicate an unfavorable course. During the scabbing stage, scabs falling off sequentially like snail shells indicate favorability; the most undesirable outcome is thin scabs like bran that ulcerate and stick together.
[Note] The formation of rash shapes is driven by "Qi" (Vital Energy). If Qi overcomes the toxin, the toxin is controlled and resolved, indicating a favorable course; if the toxin overcomes Qi, Qi is eroded and exhausted, indicating an unfavorable course. If Qi and toxin are balanced, the situation is precarious, depending on the physician's appropriate regulation to transform danger into safety. For example, initially emerging spots that are pointed at the apex and round at the base indicate Qi overcoming the toxin, which is favorable; hidden spots resembling mosquito bites, heat pruritus, cold chills, or silkworm seeds indicate toxin overcoming Qi, which is unfavorable. In the swelling stage, gradual expansion and plumpness indicate Qi overcoming the toxin (favorable); flat, non-protruding, hard, and unexpanded textures indicate toxin overcoming Qi (unfavorable). In the pustule stage, red bases with tight contraction and intact shells indicate Qi overcoming the toxin (favorable); soft shells and thin skin leading to itching and collapse indicate toxin overcoming Qi (unfavorable). In the scabbing stage, scabs like snail shells falling off in sequence indicate Qi overcoming the toxin (favorable); thin scabs like bran with ulceration and adhesion indicate toxin overcoming Qi (unfavorable). This summarizes the general principles of favorable and unfavorable rash shapes.
