Nausea, dizziness, food aversion, and selective eating constitute this condition.
During the second month of pregnancy, medicinal treatment should be discontinued. In the first month of gestation, the Foot-Jueyin Liver Meridian nourishes the fetus; in the second month, the Foot-Shaoyang Gallbladder Meridian nourishes it. This often leads to nausea, dizziness, and a preference for sour foods. Commonly referred to as "sickness of the child," it is not a disease requiring medical intervention. Treatment may exacerbate symptoms; it is best to wait for spontaneous resolution.
In the first month of pregnancy, fetal stabilization measures are unnecessary. As the fetus has not yet fully formed, vomiting at this stage is a maternal disorder. If one focuses excessively on stabilizing the fetus by using herbs such as Amomum fruit (Sharen) and Scutellaria root (Huangqin), it may反而 damage the Spleen and Stomach. Therefore, one should treat the maternal illness directly, after which vomiting will cease naturally. Only when a prescription matches the disease but is unsuitable for the fetus should those specific components be removed.
Impaired movement of Middle Qi. During the second or third month of pregnancy, stagnation of Middle Qi often prevents proper transformation and transportation, leading to loss of appetite and nausea upon smelling food. This obstruction is suitable for regulating the Spleen and Stomach with Xiangsha Liujunzi Tang (Aromatic Sand-Cardamom Six Gentlemen Decoction). Adding Mume fruit (Wumei) is also effective.
Constitutional weakness. Insufficiency of Qi and Blood leads to internal stagnation. Stagnant Qi rushing upward to the Stomach causes vomiting; Blood deficiency failing to nourish the Heart results in restlessness and oppression. The primary approach should be to harmonize Blood and dissipate stagnation using Shenju Yin (Ginseng and Tangerine Peel Drink). In cases of extreme weakness, or where alternating chills and fever occur alongside profound fatigue, this indicates insufficiency of the Liver and Kidney. It is necessary to tonify both the Foot-Jueyin Liver Meridian and the Foot-Shaoyin Kidney Meridian.
Phlegm-fluid retention in the Stomach. When Blood becomes stagnant, fluids follow Qi upward and stagnate in the Liver Meridian, causing a craving for sour substances. Since Metal controls Wood (Sour corresponds to Wood, Pungent to Metal), it is appropriate to use Pungent flavors to overcome this, making Amomum fruit (Sharen) and Agastache herb (Huoxiang) indispensable. Although Pinellia tuber (Banxia) may potentially harm the fetus, it can be used temporarily without hesitation, provided it is not overused. Banxia Fuling Tang (Pinellia and Polyporus Decoction) may be considered.
Deficiency-Cold of Stomach Qi. It is appropriate to warm and tonify the Spleen and Stomach to direct rebellious Qi downward, using Liujunzi Tang (Six Gentlemen Decoction).
Liver Fire attacking the Stomach. A sensation of Qi rushing upward from the lower abdomen during the Yin and Mao hours (03:00–07:00) indicates Liver Fire carrying Chong Mai (Penetrating Vessel) fire upward. This causes mental restlessness, heat, and oppression, along with persistent vomiting. Zhuru Maidong Tang (Bamboo Shave and Ophiopogon Decoction) is indicated.
Qi Stagnation injuring the Liver. Following severe anger, Liver Qi carries fetal Qi upward in rebellion, causing chest fullness, dizziness, and vomiting. The treatment should focus on soothing the Liver rather than tonifying Qi. Yiqing Wan (Suppress Blue Pill) is applicable.
External invasion of Wind-Cold. Vomiting accompanied by dizziness may also result from concurrent external Wind-Cold invasion. Within the framework of regulating Qi, Perilla leaf (Zisu) should be added to disperse the pathogen. For deficient constitutions, a combination of Ginseng (Renshen) and Perilla leaf (Zisu) may be used.
Excess above and Deficiency below. If nausea and vomiting are accompanied by lower back pain, there is a risk of miscarriage. It is appropriate to combine Liujunzi Tang with Siwu Tang, removing Ligusticum rhizome (Chuanxiong), and adding Dipsacus root (Chuanduan) and Eucommia bark (Duzhong) to stabilize the fetus. Elevating agents must strictly be avoided; further elevation increases upper excess and lower deficiency, thereby precipitating miscarriage.
Severe vomiting preventing medication intake. Dryness and depletion of Body Fluids, with Deficient Fire floating upward from the Shaoyin Kidney, Depressed Fire reaching up from the Jueyin Liver, and Hidden Fire surging from the Shaoyang Gallbladder, result in severe vomiting that cannot tolerate oral medication. Urgent symptomatic treatment is required using Ziyin Qingzao Tang (Nourish Yin and Clear Dryness Decoction).
