There are four types of "Zi Fan" (pregnancy-related irritability): 1) Lung deficiency with fire pathogen encroaching, manifesting as heart irritability; 2) Stagnant phlegm and accumulated fluid, characterized by aversion to food odors and chest irritability; 3) Irritability caused solely by heat, termed "Xu Fan" (deficiency irritability); 4) Qi stagnation and discomfort, frequent vomiting of saliva and foam, fetal restlessness, termed "Zi Fan" (pregnancy irritability).
Deficient-heat in the Heart and Lungs. Yin blood is utilized to nourish the fetus, causing solitary Yang to flourish excessively. This results in deficient-heat in the Heart and Lungs, leading to irritability; treatment should focus on clearing Heat from the Heart and Lungs.
Frightened Heart and Timid Gallbladder. After conception, Qi and Blood become congested and stagnant, with Heat rising to attack the Heart and Lungs, causing irritability, restlessness, and timidity in both the Heart and Gallbladder. Use Zhu Ye An Tai Yin (Bamboo Leaf Fetus-Calming Decoction) or Xi Jiao San (Rhinoceros Horn Powder). If Kidney Fire is agitated, use Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill). (Refer to Formula No. 4 in Harmonizing category, Formula No. 3 in Cold/Heat-clearing category, Formula No. 2 in Tonifying category.)
Stagnant Phlegm and Accumulated Fluid. Qi stagnation and discomfort lead to vomiting of saliva and foam; if severe, it causes fetal restlessness. Use Zhu Ye Tang (Bamboo Leaf Decoction). (Refer to Formula No. 4 in Cold/Heat-clearing category.) Mai Dong Yin (Ophiopogon Drink). (Refer to Formula No. 5 in Cold/Heat-clearing category.)
Dryness of Body Fluids. Deficiency of Zang-Fu Qi and disharmony of Ying (Nutritive) and Wei (Defensive) Qi lead to Yin-Yang imbalance and excess Heat invading the Heart and Spleen. Use Zhi Mu Wan (Anemarrhena Pill). (Refer to Formula No. 6 in Cold/Heat-clearing category.) Zhi Mu Yin (Anemarrhena Drink). (Refer to Formula No. 7 in Cold/Heat-clearing category.)
Qi Deficiency of Spleen and Kidney with Stagnant Fire. Use Xiao Yao San (Free and Easy Wanderer Powder) added with Huang Qin (Scutellariae Radix) and Sha Ren (Amomi Fructus Rotundus). (Refer to Formula No. 5 in Harmonizing category.)
Fetal Qi Pressing Upward. Rebellious Qi causing distension and stuffiness. Use Qing Gan Yin (Liver-Clearing Drink). (Refer to Formula No. 2 in Dispersing category.)
Qi Stagnation in the Upper Jiao. Stagnation transforms into Fire, leading to disharmony of Middle Qi. Use Fen Qi Yin (Qi-Separating Drink). (Refer to Formula No. 6 in Harmonizing category.)
Sudden Unrelenting Thirst. Thirst due to Blood attempting to coagulate is not a disease; drinking juice of Wu Wei Zi (Schisandrae Fructus) will lead to self-healing.
Ministerial Fire Encroaching on the Lung. In midsummer, Ministerial Fire prevails and attacks Lung Metal, causing irritability and fetal restlessness. Treatment should clear Heart Fire.
Disharmony of Fetal Qi, rushing upward to the Heart and Abdomen, causing distension, fullness, stuffiness, pain, and restlessness.
Turbid Qi rebelling upward causing Heart and Abdominal Pain. Treatment should regulate Qi and relieve stagnation; use Zi Su Yin (Perilla Drink). (Refer to Formula No. 3 in Dispersing category.)
Fetal Heat and Rebellious Qi. During the third month of pregnancy, Ministerial Fire and Triple Burner Fire nourish the fetus, causing Fetal Heat to rush to the Heart resulting in pain. Use Zi Su Yin (Perilla Drink). (Refer to Formula No. 3 in Dispersing category.) If Fetal Qi attacks the Heart causing loss of consciousness, use Tiao Zhong He Qi Yin (Regulate Middle and Harmonize Qi Drink). (Refer to Formula No. 8 in Cold/Heat-clearing category.)
Cold Fluid Contending with Qi. Cold pathogen in the abdomen, sensation of cold in the lower abdomen like a fan blowing, aversion to cold and preference for warmth, indicating Cold Uterus. Fu Zi Tang (Aconite Decoction) may be used cautiously. (Refer to usage in Formula No. 1 in Heat/Warming category.) However, this is only appropriate if the pulse is indeed slow and fine.
