Postpartum emaciation and weakness, shortness of breath and fatigue, alternating chills and fever, resembling malaria, is due to insufficient Qi and Blood failing to circulate throughout the body. It is appropriate to greatly tonify Qi and Blood.
Kidney and Liver deficiency, with dry mouth and cough, indicates Yin deficiency with hyperactive Fire. Liuwei Wan (Six-Ingredient Pill with Rehmannia) added with Bei Wu Wei Zi (Schisandrae Fructus) and Mai Dong (Ophiopogonis Radix) is indicated. (Ref: Bu 2)
Spleen weakness and Blood deficiency leading to irregular menstruation are treated with Bazhen Tang (Eight-Treasure Decoction). (Ref: Bu 5)
Spleen deficiency causing Blood dryness, resulting in pruritus cutis, is treated with Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder). (Ref: Bu 21)
Incessant spontaneous sweating is treated with Mother Hen Soup. (Ref: Bu 59)
Pain in the four limbs is caused by invasion of external pathogens. Shizi Tang (Stone Decoction) is indicated. (Ref: He 42)
Abdominal pain due to deficiency-cold is treated with Yellow Female Chicken Soup. (Ref: Bu 41)
External heat with internal cold, caused by invasion of external pathogens, is treated with Liyin Jian (Yin-Nourishing Decoction) modified. (Ref: Re 4)
Yang deficiency causing internal cold is treated with Liyin Jian or Mutton Soup. (Ref: Re 4, Bu 60)
Excessive blood loss leads to dryness of the Large Intestine, or may be accompanied by fire-dryness. One must never use harsh purgatives to avoid damaging Yin and worsening constipation.
Blood deficiency generating fire is treated with Siwu Tang (Four-Substance Decoction) added with Ma Ren (Cannabis Semen), or Jia Wei Liuwei Wan (Modified Six-Ingredient Pill). (Ref: Bu 7, Bu 50)
Deficiency of both Qi and Blood is treated with Shiquan Dabu Tang (Ten-Increment Great Tonifying Decoction) added with Rongzhuang (Cistanches Herba). (Ref: Bu 39)
If diet is normal but the abdomen is distended like a drum, Bazhen Tang added with Tao Ren (Persicae Semen) is indicated. (Ref: Bu 5)
If there is a sensation of fullness and an urge to defecate that cannot be expelled, with stool impacted in the rectum, pig bile enema is indicated for guidance.
Postpartum hemorrhage, or excessive sweating, combined with fatigue injuring the Spleen, prevents the Spleen from transporting fluids for the Stomach. Consequently, the transformative Qi fails to move, and the command for seepage and excretion does not function. Thus, upper Qi fails to descend, causing dry throat and thirst; lower Qi fails to move, causing closure of the Stomach and Kidney. Treatment should assist the Spleen and benefit the Lung, regulating the ascending and descending of Qi and Blood, so that Qi and Blood flow orderly, Yin descends and Yang ascends, naturally replenishing fluids and promoting urination.
If obstruction is caused by foul blood, Qumai (Dianthi Herba) and Puhuang (Typhae Pollen) are used.
If caused by Blood deficiency with heat stagnation, Liuwei Wan and Xiaoyao San are indicated. (Ref: Bu 2, He 5)
Heat pathogen lodging in the Bladder is treated with Maogen Tang (Imperatae Rhizoma Decoction). (Ref: Han 32)
Abdominal distension like a drum, with mental confusion and unconsciousness, is due to accumulation of cold qi during pregnancy. Fill the umbilicus with salt, pound Scallion White into a cake about one finger thick, place it on the salt, and moxibustion is applied through the scallion cake using mugwort. When warmth is felt entering the abdomen, bowel movement will immediately occur.
Dampness invading the Bao Luo (uterine collaterals) causes swelling of the external genitalia and inability to pass even drops of urine. This is due to Kidney deficiency and weak Bladder Qi failing to transform fluids. Ren Shen (Ginseng Radix), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Fu Ling (Poria), Rou Gui (Cinnamomi Cortex), Yi Yi Ren (Coicis Semen), Shu Di Huang (Rehmanniae Radix Praeparata), and Che Qian Zi (Plantaginis Semen) are used.
Blood exiting via the urine results from heat pathogen clashing with blood, infiltrating the Bladder, forming Xue Lin (Blood Strangury). Bai Mao Gen (Imperatae Rhizoma), Qumai (Dianthi Herba), Che Qian Zi (Plantaginis Semen), Tong Cao (Tetrapanacis Medulla), and Carp are decocted in water, with Shi Shou Yu Shi (Lithops Schlegelii, i.e., Stonefish stone/granule) powder mixed in for oral administration.
Obstruction of both large and small bowels is due to insufficiency of Yin Blood and dryness of fluids. Cow's milk or human milk is suitable. Alternatively, if lochia flows into the Small Intestine, blocking the water passages, urinary retention occurs; if it flows into the Large Intestine, stasis accumulates into masses resembling chicken liver, with dry and hard anal constriction causing fecal retention. Shenghua Tang (Life-Born Decoction) added with Tao Ren (Persicae Semen), Ma Ren (Cannabis Semen), Yuli Ren (Prunus Semen), and Qumai (Dianthi Herba) is indicated. (Ref: He 24)
