Everyone else is awake, but I alone am drunk; everyone else is awake, but I alone am asleep. Little do they know that this is neither sleep nor wakefulness; once the eyelids meet, one is forever shut in darkness. Suddenly, a guest arrives from afar, hand climbing the upper eyelid to open it towards the light. I would rather be able to open my eyes but not close them, staring intently until I kill that detestable creature.
In this condition, the interior of the eye appears normal, and the patient feels no discomfort internally. Only the upper and lower eyelids remain tightly closed day and night without opening; even when pulled open by hand, they cannot blink. The rationale for this is incomprehensible. I have seen patients who, with every movement, must use their fingers to lift their eyelids in order to see. Neither acupuncture nor herbal medicine offers a reliable basis, so this has been passed down to elders for reference. My opinion is that between the meridians and vessels of the two eyelid sacs (eyelids), evil qi strikes, causing blood and qi to fail to mutually nourish and defend, resulting in numbness and insensibility, thus producing this state. This shares the same root as wind striking the limbs. Some say there is no treatment method other than "clamping" therapy, which may be one approach. For newborn infants who do not open their eyes for ten or more days, this is due to the mother consuming excessive spicy and hot foods during pregnancy, dispersing the fetal qi, or the infant's own spleen deficiency and fatigue. If milk and feeding are sufficient, recovery occurs without medication. However, such children remain delicate and weak throughout, making it difficult for them to grow up healthy. If slight discharge (eye boogers) and tears appear on the exterior of the eyelids, this indicates spleen and lung deficiency with damp-phlegm. Apply Qingkong Gao (Clear Sky Ointment) into the eye. Additionally, decoct Ren Shen (Ginseng Radix et Rhizoma), Bei Mu (Fritillariae Bulbus), Mai Dong (Ophiopogonis Radix), Yun Hong (likely referring to a specific preparation or herb, possibly Paeoniae Radix Rubra or similar contextually), and Xia Ku Cao (Prunellae Spica). Drinking a small cupful will cause immediate opening.
Among the Six Qi (Six Excesses), wind is the most severe; even at its mildest, it damages the mouth and eyes. Paralysis facing guests brings shame and embarrassment, so one often uses a silk fan to cover the face.
In this condition, the eyeball naturally tilts, and the cheek and lips also deviate to one side. Physicians generally call this deviation of the mouth and eye (facial paralysis), or sometimes describe it as the lip and eyelid pulling each other. This occurs because wind and damp-earth qi act pathogenically; since the internal organs are inherently deficient, they invite the pathogen to strike suddenly. Upon striking, the blood vessels become dispersed, leading to deviation and motor impairment. If the deviation can correct itself, then revert to deviation, or if closure can occur then revert again, Zheng Rong Tang (Correct Contour Decoction), Jia Wei Di Huang Yin Zi (Modified Rehmannia Decoction), and Sheng Feng Tang (Extinguish Wind Decoction) can be used for treatment. However, if the condition has become fixed, regardless of duration, the disfigured appearance will persist for life. The *Ling Shu* states: regarding the Foot-Yangming Meridian, disease affects the facial tendons; cold causes tension and pulls the cheek, displacing the mouth. Heat causes flaccidity and inability to contract, resulting in deviation. "Pi" (deviation) means leaning to one side. This is also because true qi is trapped by evil qi, unable to ascend outward or descend outward, thus being pulled to one side. Cold on the left and heat on the right leads to deviation on the right; cold on the right and heat on the left leads to deviation on the left. The method should involve moxibustion at Dicang (ST4) and Chengqi (ST1). If ineffective, moxa at Renying (ST9). The Classic says, "For sunken conditions, apply moxibustion," which is precisely this principle. Another theory suggests that excess dampness causes left-sided deviation, while excess wind causes right-sided deviation; both have subtle validity and require careful consideration. Acting arbitrarily will harm the patient.
