The upper end of the reed tube is held outside the lower teeth, tightly adjacent to the tip of the tongue; the lower end is immersed in the medicinal liquid two to three finger-widths deep, and should not be inserted suddenly too deeply. First, blow gently around the periphery for ten to twenty breaths, then blow deeper for four or five breaths, allowing the medicinal qi to ascend. When breathing, only open the upper lip, keeping the tube against the lower lip. The speed, urgency, depth, and shallowness of blowing must follow proper technique. The patient will perceive the effect after five or six blows; this subtlety cannot be fully conveyed by words.
*The Ophthalmic Secret Manual* states that from Yin hour to Wu hour, eye drops can be applied multiple times, which is very clear. I once treated a young nobleman who did not observe strict quietude. As a temporary measure, I applied eye drops while he was lying down, following the number of applications specified in *The Secret Manual*, all completed before the third watch (midnight). The next morning, a significant amount of nebula had receded. Although *The Secret Manual* advises against application after noon, I applied it while lying down and cured many patients. *The Secret Manual* states that cloud-like nebula will completely recede in twenty-one days (three sevens); however, in my practice, it took several months to completely clear the nebula through blowing. For internal cataracts, taking Bu Nao Huan Jing Wan (Brain-Nourishing Eye-Clarifying Pill) felt effective, so I continued with eye drops and intermittent medication. If vision clears and the divine light does not leak outward, then take Zi Shen Ming Mu Wan (Kidney-Nourishing Eye-Brightening Pill).
The content regarding Huan Jing Wan in *The Secret Manual* ends here. Zi Shen Ming Mu Wan (Kidney-Nourishing Eye-Brightening Pill) is the concluding formula collected by Master Wang Wan. Take Zi Shen Ming Mu Wan in the morning, and Bu Nao Huan Jing Wan (Brain-Nourishing Eye-Clarifying Pill) after breakfast. After completing both formulas, take Di Huang Ming Mu (Rehmannia Eye-Brightening) and other medicines. There is no case where these do not prove effective.
Generally speaking, if internal cataracts are not removed, taking Di Huang Ming Mu (Rehmannia Eye-Brightening) and similar tonics will instead aid the rise of fire pathogenic factors, causing the nebula to increase daily; thus, tonification alone is inadvisable. Taking cold and cool agents to clear fire damages the Qi and Blood of the Spleen and Stomach; thus, purgation alone is inadvisable. The reason for this is that the Liver and Lung meridians are the pathways to the Kidney meridian. If these pathways are blocked by internal cataracts, how can tonic medicines enter? Now, by using the blowing method to remove the obstructing membranes from the Liver and Lungs, the orifices and pathways of the Kidney meridian are opened. Any tonic medicine can then enter the Kidney meridian via the Liver and Lungs, sufficiently replenishing Kidney Water. Consequently, both eyes become bright as those of a child. The hidden key to ophthalmology lies precisely in this mechanism.
Bai Ju Hua (Chrysanthemi Flos, 3 liang), Chuan Xiong (Ligustici Chuanxiong Rhizoma, 5 qian), Bai Zhu (Atractylodis Macrocephalae Rhizoma, stir-fried with soil, 8 qian), Cao Jie Ming (Cassiae Semen, stir-fried, 5 qian), Ren Shen (Ginseng Radix et Rhizoma, 3 qian), Chen Pi (Citri Reticulatae Pericarpium, 4 qian), Zhi Ren (Gardeniae Fructus, stir-fried, 8 qian), Rou Cong Rong (Cistanches Herba, washed with wine to remove scales, 8 qian), Huang Bo (Phellodendri Cortex, stir-fried with salt water, 1 liang), Zhi Mu (Anemarrhenae Rhizoma, stir-fried with salt water, 1 liang), Mu Zei (Equiseti Herba, nodes removed, 1 liang), Chong Wei Zi (Leonuri Fructus, stir-fried, 5 qian), Gou Qi (Lycii Fructus, washed with wine and dried, 3 liang).
Make honey pills the size of tung tree seeds. Each qian of powder yields ten pills. Dry them in the sun. Take 3 qian on an empty stomach with light salt water.
The flushing/blowing method in *The Secret Manual* was originally intended for youths and middle-aged individuals with robust Qi and Blood. For frail elderly people, it should not be used lightly; even for youths and middle-aged individuals who are not robust, one must first tonify before applying this method.
For those with deficient Qi and Blood, tonify their Qi and Blood; for those with weak Spleen and Stomach, nourish their Spleen and Stomach; for those with deficiency of True Yin, nourish their Kidney Water. The method of building the foundation is absolutely indispensable. If the blowing/flushing is done halfway and the nebula has completely receded, and the person is slightly weak, there is no need to continue blowing. If a weak person forces the complete removal of internal cataracts, it may damage the Kidney meridian, leading to the risk of Qing Mang (a condition characterized by severe visual impairment or blindness). Exercise utmost caution!
