Leukorrhea (vaginal discharge) and leukorrhea with blood both result from internal injury caused by the Seven Emotions (joy, anger, worry, thought, sadness, fear, fright), or from deficiency-cold of the Lower Yuan (Kidney and Mingmen). The causes are not singular. Generally, white leukorrhea is more common, while bloody leukorrhea occurs occasionally. For both, it is appropriate to take Shunqi San (Smooth Qi Powder) and swallow Zhenling Dan (Shock Spirit Pill), assisted by Aifu Wan (Mugwort and Cinnamon Twig Pill). Alternatively, one may take Sand Shen Mo (Glehnia Root Powder) mixed with rice water. If the leukorrhea does not cease and leads to emaciation (Cheng Lei), Si Wu Tang (Four Substances Decoction) with Ban Qian (half a qian) of Mu Li Fen (Oyster Shell Powder) should be taken, along with Gu Yang Wan (Solidify Yang Pill). Taking it frequently will yield results.
In some cases where leukorrhea has healed but recurs after one or two months, or occurs once every half year, bleeding precedes the discharge, flowing uncontrollably. After a brief pause, it flows again profusely. This is named "Lou Dai" (Leaking Leukorrhea) and is the most difficult to treat. The blood from the lower section is governed by the Lower Abdomen. Some cases arise because blood deficiency allows deficient heat to sink into the Small Intestine, causing painful and difficult urination. The urine may be turbid like rice water or form sand-like particles. These conditions must not be treated as Lin Syndrome (Dysuria) using cold-natured agents. It is appropriate to decoct Si Wu Tang (Four Substances Decoction) and Wu Ling Yin (Five-Ingredient Powder with Poria) together, each taking half a dose.
During pregnancy, E Zu (Morning Sickness/Obstruction) manifests as vomiting upon seeing food, craving for sour foods, lying down often and rising rarely. Colloquially called "Bing Gui" (Sickness Ghost), this is generally due to pre-existing Phlegm-Fluids in the body. When Blood is obstructed and fails to circulate, the Fluids ascend with Qi and stagnate in the Liver Meridian. Since the flavor associated with the Liver is Sour, the patient inevitably craves sour foods. According to the Five Elements theory, Metal overcomes Wood; therefore, Pungent flavors are used to overcome it. Xiao Ban Xia Fu Ling Tang (Minor Pinellia and Poria Decoction) or Er Chen Tang (Two-Aged Decoction) may be used. In severe cases, Er Chen Tang is preferred. If accompanied by vomiting, inability to eat, heart emptiness, irritability, and oppression, Ju Su Yin (Tangerine and Perilla Drink) with Zhu Ru (Bamboo Shavings) the size of a finger should be used. If taking hot medicines causes oppression and heat in the Diaphragm leading to illness, Pu Huang San (Polygonum Discoloratum Powder) or He Ye San (Lotus Leaf Powder) is suitable.
The concept of Zhuan Bao (Displaced Bladder/Uterus) is discussed in various theories. As the fetus gradually grows and presses downwards against the Bao (Bladder/Uterus), the Bao is compressed and tilted, causing frequent urination; hence the name Zhuan Bao. The Bao refers to the Bladder. However, Zi Lin (Pregnancy Dysuria) resembles Zhuan Bao. Zi Lin is characterized by frequent, dribbling urination with pain, whereas Zhuan Bao involves frequent, scanty urination without pain, though sometimes with slight discomfort, which ultimately distinguishes it from Lin Syndrome. Both conditions are suitable for treatment with Sheng Liao Wu Ling San (Raw Material Five-Ingredient Powder with Poria) plus Yi Jiao (Donkey-hide Gelatin) 1 qian. One may also seek remedies among formulas for prenatal conditions. Wu Wei Zi Ba Wei Wan (Eight-Ingredient Pill with Schisandra) added with Dang Gui (Angelica Sinensis) can also treat Zhuan Bao, or additionally add Che Qian Zi (Plantago Seed). Fa Hui Tang (Ash of Hair Decoction) is particularly effective.
For pregnant women with Zhuan Bao, Nasal Insufflation Medicines (Chu Bi Yao) can be used to induce sneezing. Alternatively, striking the sole of the foot with a fist until pain is felt can cause the patient to jerk their whole body. This movement of the viscera allows the Bao to return to its normal position spontaneously.
Postpartum diseases may present with alternating chills and fever, or solely with fever. There are three types of sole fever: If Evil Blood (Lochia) has not been discharged, there is abdominal pain with fever. If an external pathogen is contracted, there is headache, aversion to wind, and fever. Only when there is Blood Deficiency is there merely fever without other symptoms, named Ru Lao (Postpartum Consumption/Labor Fever). Medication should be sought based on previous Blood Deficiency patterns. For postpartum Blood Deficiency with irritability, Huáng Huáng (Polygonum Multiflorum/Polygoni Multiflori Radix Praeparata - *Note: Context suggests Huang Qin or similar, but text says Pu Huang. Text says Pu Huang*) fried through paper and taken with Dong Liu Shui (East-flowing Water) is indicated. For postpartum abdominal pain with unceasing lochia where other medicines fail, Xiong Gui Tang (Ligusticum and Angelica Decoction) added with Wu Ling Zhi (Trogopterus Dung) and Xuan Hu Suo (Corydalis Rhizome) should be decocted.
To determine if a fetus is alive or dead during pregnancy, and to ensure safety, Chuan Xiong (Ligusticum) and Dang Gui (Angelica) should be decocted together with water and wine. If the fetus is alive, it will be secured; if dead, it will be expelled. Dai Fu An stated that repeated use of this method has never failed.
Swelling and redness of the feet before delivery, colloquially known as Zhou Jiao (Wrinkled Feet), is treated with Xiang Su San (Tiger Balm Powder/Tangerine and Perilla Powder) plus Mu Xiang (Aucklandia Root) 1 qian.
Before delivery, one should promote Qi circulation and slim the fetus to facilitate easy labor. Shou Tai Yin (Slim Fetus Drink) with a little Su Sha (Amomum Fruit) is suitable.
If the placenta does not descend (resembling a chicken egg shape), mix half a zhan of Sesame Oil with Black Vinegar, Egg White, and water equal to the amount of oil. Beat evenly and take cold once. For retained placenta, equal parts of Chi Xiao Dou (Adzuki Bean) and Xiao Mai (Wheat) should be decocted and taken. Additionally, grind seven seeds of Bi Ma Ren (Castor Bean) after removing the shell and apply to the soles of both feet. Wash off immediately after the placenta is delivered.
If postpartum blood enters the Lungs, causing dark complexion and panting, take 1 liang of Ren Shen (Ginseng) powdered separately. Decoct 2 liang of Su Mu (Sappan Wood) in two bowls of water until reduced to 0.7 bowl, then mix with the Ginseng powder. This is named Shen Su Yin (Ginseng and Sappan Drink).
Postpartum blood loss leading to dry and constipated bowels is treated with Ju Xing Wan (Tangerine and Apricot Pill) or Ma Ren Wan (Hemp Seed Pill). These are applicable to any constipation resulting from blood loss.
Postpartum fever with confusion, colloquially called Re Xue (Hot Blood), is treated by frying Jing Jie (Schizonepeta) on a new tile. Take an unspecified amount, half fried and half raw, powder it, and take 1 qian mixed with warm water. This is named Du Xing San (Walking Alone Powder). Some may doubt that Dou Lin Jiu (Bean Dripping Wine) is too hot; using Tong Bian (Child's Urine) for mixing is especially suitable. Decocting the powder directly is also acceptable. Jing Jie is a crucial herb for postpartum conditions. For Ganglion Rigidity (Opisthotonos), mixing with Dou Jiu (Bean Wine) is excellent. For confusion without fever caused by extreme anger or joy, mixing with urine is invariably effective.
Touching the abdomen postpartum causing pain is colloquially called Jia Mu Tong (Mother-in-Law Pain). Mix Bai Zhi (Angelica Dahurica) powder with child's urine. To break blood stasis, use Gui Xu (Angelica Sinensis Tails); to tonify blood, use Gui Shen (Body) and Gui Tou (Head). This illustrates the principle that Rou Gui (Cinnamon Bark) consolidates sweat while Zhi (Branches) induces sweating.
Si Wu Tang (Four Substances Decoction) is a essential medicine for women. Adding Ban Qian (half a qian) or 1 qian of Wu Zhu Yu (Evodia Fruit) to it treats all Blood-Qi disorders.
Some women are deficient throughout life despite never having had abdominal surgery. How can they be deficient? Generally, women are governed by Blood. If Blood is weak and Qi is excessive, conception is impossible. Precisely because of Blood Deficiency, infertility occurs. Women who take too many fetal-stabilizing medicines during pregnancy, or undergo normal delivery or miscarriage, may find that although the fetus is expelled, the lochia does not clear immediately. Or, after two or three months, lochia still drips. This is not comparable to stagnant blood (Bai Xue). It is precisely because stabilizing menstruation medicines were taken previously, causing blood stagnation and failure to transform. Medications should promote smooth blood flow and Qi circulation, not accumulate blood and close off Qi.
At full term (Man Du), people often indulge in food, leading to food retention fever resembling Shang Han (Cold Damage) or Shi Fu (Relapse due to diet). Initially, use Hong Wan Zi (Red Pills) one or two doses, then proceed to Xiao Chai Hu Tang (Minor Bupleurum Decoction). This argument likely has a basis.
Medicines for women are generally sweet and disadvantageous to the Spleen. Even Xiong Gui (Ligusticum and Angelica) can be cloying, let alone Di Huang (Rehmannia). For those with Excess Spleen-Stomach, it shows merit. However, for those with Phlegm-Fluids, and when adding sweet herbs to blood-tonifying formulas, Ban Xia (Pinellia) and Chen Pi (Tangerine Peel) are indispensable. Ju Bao Dan (Treasure Gathering Pill) treats Qi pain and Blood pain in women, whether menstrual flow is scanty and painful, excessive and unceasing with pain, dark and clotted, mixed with yellow discharge, or rotting blood accompanied by diarrhea with abdominal distress. All these are treated with Chuan Xiong (Ligusticum), Dang Gui (Angelica), Pao Jiang (Fried Ginger), and Rou Gui (Cinnamon Bark), 1 qian each, decocted in plain water. It has proven effective repeatedly.
Ganglion Rigidity (Opisthotonos) is an emergency condition in women and the most severe of all diseases. Only one or two out of ten survive. Powder slightly fried Jing Jie (Schizonepeta) on a new tile and take 2 qian mixed with Dou Lin Jiu (Bean Dripping Wine). Alternatively, use only Du Huo (Angelica Pubescens) powdered and taken with Dou Lin Jiu.
