The Stomach (Wei) is responsible for receiving and accepting food, while the Spleen (Pi) is responsible for transportation and transformation. If Spleen Qi is deficient, it cannot adequately transform grain Qi, leading to further depletion of what the Spleen receives. The Spleen governs the movement of the four limbs; since the Qi received is deficient, this results in fatigue of the four limbs and a tendency to sleep excessively. Some individuals feel body fatigue and drowsiness after eating, which is due to Spleen deficiency failing to transport and transform properly. Treatment should generally tonify the Spleen and regulate the Stomach, combined with promoting digestion and removing stagnation, so that comfort will naturally return. A pulse that is deep and forceless, with a fine and soggy quality at the Right Guan position, indicates fatigue.

The Classics state: All rebellious Qi rushing upward belongs to Fire. All vomiting of sour fluid, sudden diarrhea with tenesmus, belong to Heat. It also states: When Cold Qi lodges in the intestines and stomach, causing Jue Ni (rebellious counterflow) to rush upward, pain and vomiting occur. These symptoms generally belong to Disharmony of Stomach Qi, but there are differences among Cold, Heat, Phlegm, Food Stagnation, Wind Pathogen, Qi Rebelliousness, and chronic illness leading to Stomach Deficiency, which must be carefully distinguished. In Stomach Cold, there is aversion to cold and food; morning intake leads to evening vomiting of clear water and saliva; preference for hot substances; pulse is deep and slow. In Stomach Heat, food is vomited immediately upon entry; restlessness, oppression, irritability, and thirst occur; vomitus tastes sour and bitter; desire for cold substances; pulse is surging and large. Vomiting due to Phlegm-Fluid involves spitting up phlegm and saliva; vomiting due to Food Stagnation involves abdominal distension and belching with sour/rotten odors; vomiting due to Wind Pathogen is sudden and severe; vomiting due to Qi Rebelliousness involves chest and diaphragmatic oppression; vomiting due to Stomach Deficiency involves inability to accept food. Each case must be weighed by pulse diagnosis.
