There is no sound and no substance being vomited; there is an urge to vomit but nothing comes out. The Qi rises to the throat and then returns inward, causing a sense of extreme restlessness in the heart.
Main Formula: Banxia (Pinelliae Rhizoma), Fuling (Poria), Chenpi (Citri Reticulatae Pericarpium) (each 2 qian), Shengjiang (Zingiberis Rhizoma Recens) (7 slices). Decoct and take hot after adding Jiangzhi (Ginger Juice).
If there is Phlegm and deficiency, add Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), and Gancao (Glycyrrhizae Radix et Rhizoma); if it is Stomach Cold, use Dingxiang Lizhong Tang (Dianthermum Magnoliae and Gastrodia Decoction) (see "Cold Section" for formula); if it is Fire, use drugs such as Ganjiang (Zingiberis Rhizoma) and Huanglian (Coptidis Rhizoma).
(Attachment: Vomiting clear water)
The Classic says: "All kinds of vomiting and acid regurgitation belong to Heat." Some regard acid regurgitation as Cold, attributing it to the consumption of cold, raw, or hard foods. However, they fail to realize that cold and cool substances suppress the intestines and stomach, causing Yang Qi to be constrained and transform into Heat. Swallowing acid differs from vomiting acid. Vomiting acid is caused by Damp-Heat stagnating in the Liver and emerging from the Stomach, rising with the Qi and being vomited out. Swallowing acid occurs when Qi stagnation lasts for a long time without dispersing, lying latent between the Lungs and Stomach; it cannot be coughed up nor swallowed down. If the body surface is further bound by Wind-Cold external pathogens, the internal stagnation becomes more severe, and the sour taste pierces the heart. If the body surface receives warmth, the Couli (pores) open, and the symptoms slightly resolve. Or, some may temporarily recover using pungent, aromatic, dry, and hot decoctions or pills because the root is Heat while the branch is Cold, and pungent substances can disperse the exterior. If too many warm and hot medicines are taken, leading to the exhaustion of Body Fluids, it may instead result in Yi Ge (dysphagia) or Fan Wei (regurgitation) syndromes. For chronic illness, one must dissipate stagnant Fire, assisting with a small amount of pungent-warm substances. A slow pulse indicates Cold; a rapid pulse indicates Heat; floating and tight pulses indicate Heat constrained internally and Cold binding externally.
Main Formula: Chenpi (Citri Reticulatae Pericarpium), Cangzhu (Atractylodis Rhizoma), Fuling (Poria), Banxia (Pinelliae Rhizoma, processed), Zhizi (Gardeniae Fructus, stir-fried with ginger juice), Huanglian (Coptidis Rhizoma, stir-fried with ginger juice), Xiangfu (Cyperi Rhizoma, stir-fried with child's urine), Hongdoukou (Myristicae Semen), Shenqu (Massae Fermentatae Medicamentosa) (each 1 qian), Wuzhuyu (Evodiae Fructus) (8 fen), Gancao (Glycyrrhizae Radix et Rhizoma) (3 fen).
