Breast diseases are primarily associated with the Liver, Stomach, Heart, and Spleen. The nipple belongs to the Liver Meridian (Liver Channel), while the breast tissue belongs to the Stomach Meridian (Stomach Channel). Depression and stagnation of the Heart and Spleen often manifest as symptoms such as *Ru He* (Breast Nodule) and *Ru Yan* (Breast Rock/Cancer). *Ru Yong* (Acute Mastitis) presents with swelling and redness, belonging to Yang patterns, typically caused by Heat-Toxin. It is common in women; once pus discharges, it heals easily. *Ru Yan* (Breast Cancer) presents as a white-colored nodule, belonging to Yin patterns, typically caused by Phlegm coagulation. It occurs in both men and women but is more frequent among widows and isolated individuals. Once ulcerated, it is difficult to treat. Furthermore, if an infant suckles the affected breast, healing is easier; without suckling, recovery is difficult. Conditions such as *Ru Li* (Breast Fistula) may, over time, transform into cysts that perforate, exposing internal organs, causing extreme damage that is irreversible. *Ru Yan* (Breast Cancer) is particularly characterized by a hard root that is difficult to excise. Some patients experience pain only after several years, or ulceration after ten-plus years. By the time pain begins, treatment is delayed; once ulcerated, it is incurable. Long-term administration of *Guipi Tang* (Restore the Spleen Decoction) and *Yangrong Tang* (Nourish the Nutrient Decoction) is required. One must strictly avoid harsh attacking or detoxifying herbs that damage Yuan Qi (Original Qi) and accelerate death.
Milk is transformed from Qi and Blood, originating from the Stomach, and represents the essence of food and water. Since the Chong Mai (Penetrating Vessel) is subordinate to the Stomach, ascending Qi transforms into milk, while descending Qi becomes menstruation. Three days postpartum, the onset of chills and fever is termed "Zheng Ru" (Steaming Milk). Treatment should use *Xiaoyao San* (Free and Easy Wanderer Powder) with Bai Zhu (Atractylodis Rhizoma) removed. For young women giving birth for the first time, if breasts are distended and milk flow is obstructed, clear heat and promote drainage using *Lianqiao Jinbei Jian* (Forsythia and Fritillaria Decoction). If multiparity results in low milk supply due to Qi and Blood deficiency, tonify using *Yigong San* (Extraordinary Aid Powder) modified with Dang Gui (Angelicae Sinensis Radix), Bai Shao (Paeoniae Radix Alba), Gou Qi Zi (Lycii Fructus), Shu Di Huang (Rehmanniae Radix Praeparata), and Gua Lou Ren (Trichosanthis Semen). Administer with soup broth. Spontaneous lactation postpartum indicates Stomach Qi deficiency; secure and tonify using *Qifu Yin* (Seven Blessings Decoction) modified with Huang Qi (Astragali Radix) and Wu Wei Zi (Schisandrae Fructus) to astringe. Excessive milk with distension and pain can be dispersed by applying warm cloth compresses. Infant suckling combined with wind exposure causing breast congestion, nodularity, and pain is called "Wai Chui" (External Blowing); if not treated urgently, it often develops into *Ru Yong* (Acute Mastitis).
Internally administer *Gualou San* (Trichosanthes Powder), externally apply Tian Nan Xing (Arisaematis Rhizoma) powder. Severe cases use *Lianqiao Jinbei Jian*. In pregnant women, fetal heat causing chills, fever, and breast swelling is called "Nei Chui" (Internal Blowing), treated with *Juye San* (Citrus Aurantium Leaf Powder). If a newborn cannot suckle, retained milk causes breast distension, fever, thirst, hardness, and pain, termed "Du Ru" (Jealous Milk). Squeeze out retained milk or encourage drainage via suckling. Use Bei Mu (Fritillariae Bulbus), Gualou (Trichosanthis Fructus), Gan Cao Jie (Glycyrrhizae Radix et Rhizoma with knot), and Mu Tong (Akebiae Caulis) decoction. If the infant dies, and the mother experiences steaming milk with chills, fever, distension, and pain, use weaning methods: decoct 1 liang of Chao Mai Ya (Fried Hordei Fructus Germinatum) to dissipate milk. For those with strong Qi and Blood but delayed lactation, use *Tongcao Zhuti Tang* (Akebia and Pig Trotter Decoction), *Tongcao San*, or the secret formula *Yongquan San*. If Phlegm-Qi blocks the meridians preventing lactation (common in obese individuals), use *Shenxiao Gualou San* to diffuse and descend. Alternatively, burn Si Gua Luo Lian Zi (Luffae Fructus Retinervus cum Semine) until charred on the outside, take 3 qian with wine, cover up to induce sweating, which will unblock flow. For Qi and Blood deficiency causing no lactation, use *Yulu San* or *Bazhen Tang* (Eight Treasures Decoction) modified with Huang Qi and Mai Dong (Ophiopogonis Radix) to tonify. For Lung-Stomach Deficiency-Cold causing no lactation, use *Qianjin Zhongru Tang* (Thousand Gold Calcite Decoction) to warm and nourish.
Heat-Toxin in the Gallbladder and Stomach meridians obstructing Qi and Blood causes breast swelling and pain, known as *Ru Yong* (Acute Mastitis). Initial stage with chills, fever, swelling, pain, and red skin requires cooling blood and dispersing pathogens. Use *Siwu Tang* (Four Substances Decoction) modified with Chai Hu (Bupleuri Radix), Shan Zhi (Gardeniae Fructus), Dan Pi (Moutan Cortex), Bei Mu, Gualou, and Gan Cao. Breast nodules with swelling, pain, and redness require soothing the Liver and clearing the Stomach. Internally use *Niubangzi Tang* (Arctii Fructus Decoction); externally, mash live carp including head bones, mix with one ball of Xiang La Cao (Incense Wax Residue), grind evenly, and apply to dissipate. For Qi-Blood stasis with persistent nodules, use *Lianqiao Yinzi* (Forsythia Infusion). For Liver failure to maintain smooth flow causing *Ru Yong* nodules with chills, fever, swelling, and ulceration, use *Qinggan Jieyu Tang* (Clear Liver and Resolve Depression Decoction). For Heart-Spleen depression causing *Ru Yong* fever, nodules, and ulceration, use *Guipi Tang* with raw Huang Qi, Bai Zhu, and Gan Cao. For breast swelling and pain, use Da Bei Mu (Fritillariae Thunbergii Bulbus), Bai Zhi (Angelicae Dahuricae Radix), Ru Xiang (Olibanum), Mo Yao (Myrrha), and Dang Gui Shen (Dang Gui Root Tip), 4 qian per dose taken with white wine. For breast ulceration, use Liang Tou Jian (Phragmitis Rhizoma) and male rat feces, Tu Lian Zi (Toosendan Fructus - best after frost), and Lu Feng Fang (Vespa Nidus), each 3 qian, burned to preserve nature, ground to powder, divided into three doses taken with wine, every two days. When pain stops and pus consolidates, if pus forms but does not discharge, or pus is thin and clear, use *Tuoli Xiaodu San* (Support Interior and Eliminate Toxin Powder). For chronic non-healing ulcers, use Sang Gen Mu Zhi (Mulberry Root Bark Polypore) or fungi, burned to ash, mixed with Bing Pian (Borneolum Syntheticum) powder and applied.
A small bean-sized nodule in the breast, non-red, non-painful, accompanied by internal heat, fatigue, and irregular menstruation, is called *Ru Yan* (Breast Rock/Cancer). Treat early. If it grows large over years, becomes stone-hard, with intermittent pulling pain, and eventually ulcerates like a deep rock cavity with bloody discharge, it is incurable. Post-ulceration, if it resembles an inverted bowl, painless but extremely itchy, it indicates maggot infestation internally. This condition arises from worry, thought, and depression damaging Liver and Spleen Qi and Blood. For initial small nodules, roast dozens of raw crab claw shells in a clay pot, grind to powder, take with wine. Then decoct Dang Gui, Chen Pi (Citri Reticulatae Pericarpium), Zhi Shi (Aurantii Fructus Immaturus), Bei Mu, Qiao (Lianqiao), Jiang (Zingiberis Rhizoma), Bai Zhi, and Gan Cao Jie, taking dozens of doses continuously without interruption to eliminate. Alternate crab claw ash with the decoction; this has been verified effective. If not eliminated, internally take *Yiqi Yangrong Tang* (Tonify Qi and Nourish Nutrient Decoction), externally apply *Muxiang Bing* (Costus Cake) compress. For Yin deficiency with afternoon fever, use *Jiawei Xiaoyao San* (Modified Free and Easy Wanderer Powder) removing burnt Bai Zhu, adding Shu Di Huang. For chills, fever, and pulling pain, use *Guipi Tang*. For weakened Yuan Qi, large doses of Ren Shen (Ginseng Radix et Rhizoma) decoction can eliminate it. Using harsh attacking/detoxifying methods will inevitably lead to fatal ulceration. After ulceration, strictly avoid herbs like Ru Xiang and Mo Yao.
Postpartum elongation of both breasts, thin as chicken intestines, hanging below the lower abdomen with unbearable pain, is called *Ru Xuan* (Hanging Breast), a rare anomaly. Urgently use 1 jin each of Xiong (Chuanxiong Rhizoma) and Dang Gui, sliced. Take only 4 liang, decoct in water, and drink. Have the puerpera lie prone on a table with a fire stove underneath. Place remaining Xiong and Dang Gui slices on the stove to smolder slowly. Inhale the smoke through mouth, nose, and breasts to draw them upward. If medicine finishes and breasts do not retract, repeat decoction and fumigation. Otherwise, grind 3 castor beans (Bi Ma Zi), apply to the top of the head (Baihui area). Remove after a short while, and they will retract.
