[Phlegm-Resolving] Erchen Tang (Two-Aged Decoction) [See Volume 1, Stroke].
[Fire-Purging] Longhui Wan (Dragon's Nest Pill) [See Volume 1, Fire Syndromes].
Case of Mr. Chu: In his advanced years, he suddenly developed dizziness in early winter due to anger. Initially, he experienced palpitations followed by vertigo and collapse, profuse sweating as if washed, and insomnia at night. The pulse at the left Cun and Guan positions was floating, large, and extreme. This was caused by the depression and exuberance of Gallbladder Qi, which fanned the Heart Fire, causing deficient Yang to transform into wind that rushed upward to the vertex. He was prescribed Danpi (Moutan Cortex), Zhizi (Gardeniae Fructus) (each 1.5 qian), Ganju (Chrysanthemi Flos), Baishao (Paeoniae Radix Alba) (all stir-fried, each 3 qian), Gouteng (Uncariae Ramulus cum Uncis), Fushen (Poria with Hostwood) (each 3 qian), Baiziren (Biota Semen), Zaoren (Ziziphi Spinosae Semen) (raw and ground, each 8 fen), Sangye (Mori Folium) (2 qian), Fuxiaomai (Tritici Fructus) (2 liang), and Nanzao (Jujubae Fructus) (4 pieces). After two doses, palpitations and vertigo subsided, sweating stopped, and he slept soundly. Subsequently, Shudi (Rehmanniae Radix Preparata), Lushen (Codonopsis Radix), Wuweizi (Schisandrae Fructus), Fushen, Maidong (Ophiopogonis Radix), Lianzi (Nelumbinis Semen), and Baishao were used. After several doses, he fully recovered. Generally, when Ying (Nutrient) fluids are deficient and Gallbladder fire rises to obscure the orifices, it is necessary to use Danpi, Zhizi, Gouteng, and Sangye to clear heat; stir-fried Juhua and Baishao to extinguish wind and harmonize Yang; and add Fushen, Zaoren, Baiziren, and Xiaomai to calm the spirit and cool the Heart. Once wind subsides and sweating stops, it is appropriate to employ methods to astringe and conserve Ying fluids.
Case of Mr. Feng: He suffered from vertigo, epigastric fullness, and vomiting, expelling sour, bitter, turbid foam. This indicated Liver Wood overacting on Spleen Earth, with weak Stomach Qi leading to reduced appetite. Turbid stasis failed to descend, and upon encountering deficient wind, it surged outward. Rebellious Yang rose upward, obscuring the clear orifices, thus causing intermittent vertigo. The pulse was choppy and slightly rapid, especially at both Cun positions. Treatment initially focused on descending turbidity and extinguishing wind. Gualou Shuang (Trichosanthis Pulvis), Suzi (Perillae Fructus), Banxia (Pinelliae Rhizoma), Fuling (Poria), Xingren (Armeniacae Semen Amarum), Tianma (Gastrodiae Rhizoma), Ganju Tan (Charred Chrysanthemi Flos), Gouteng, and Juapi (Citri Reticulatae Pericarpium) were used. Symptoms subsided, and appetite returned. The original formula was then modified by removing Suzi, Xingren, and Gouteng, adding Fushen, Lianzi, Chai Shihu (Dendrobii Caulis), and He Ye (Nelumbinis Folium) decocted together. After more than ten doses, he stabilized.
Wife of the Patient: Fatigue and labor damaged Yang, resulting in insomnia and tinnitus. Dizziness and nausea occurred, alleviated slightly when lying on a pillow. Deficient Yang ascended to the vertex, wind moved, and phlegm rose, causing vertigo and vomiting. Treatment should subdue Yang and extinguish wind. Muli (Ostreae Concha) (ground), Baishao, Wuweizi, Ganju Tan, Tianma (roasted), Banxia (stir-fried with green salt), Shengdi (Rehmanniae Radix) (stir-fried), Fushen, Zaoren, and Sangye were used. Recovery occurred after two doses.
Case of Xiao: Following physical labor, he had lost blood several times previously. Recently, he experienced dizziness, tinnitus, blurred vision, palpitations, and epigastrical stuffiness. The Chi pulses at both wrists were floating, large, wiry, and forceful. Ministerial Fire flared easily, and Dragon-Lei Fire lost control. Phlegm accompanied the fire to attack the clear orifices, illustrating the principle that "without phlegm, there is no vertigo or palpitation." Treatment nourished Yin and subdued Yang. Dancai (Mytilus edulis), Muli, Shudi Tan (Charred Rehmanniae Radix Preparata), Shihu (Dendrobii Caulis), Ganju, Baishao, Beimu (Fritillariae Bulbus), and Fushen were used. After several effective doses, Liuwei Wan (Six-Ingredient Pill with Rehmannia) should be taken.
Case of Mrs. Xu: In middle age, excessive menstruation led to blurred vision and dizziness. Treatment aimed to secure Yin and harmonize Yang. Shudi, Baishao, Ganju (all stir-fried, each 2 qian), Danggui (Angelicae Sinensis Radix) (vinegar-stir-fried, 8 fen), Danpi, Muli Fen (Ostreae Concha Pulvis) (each 1.5 qian), Gancao (Glycyrrhizae Radix et Rhizoma) (charred-black, 1 qian), Nen Sangye (tender Mulberry Leaf) (3 qian), and Hongzao (Red Dates) (3 pieces) were used. Cure was achieved after two doses.
Case of Wang: Post-convalescence from latent summerheat disease was poorly managed. The pulse was deficient and rapid. Symptoms included dizziness, fever, thirst, and irritability. Deficient wind ascended to the vertex. The strategy employed bitter and pungent herbs to drain heat, assisted by sweet and moistening agents. Zhizi, Ganju, Danpi, Maidong, Chai Shihu, Tianma (roasted), Dangshen (Codonopsis Radix), Huafen (Trichosanthis Radix), Gancao, and Nen Sangye were used. Recovery occurred after two doses.
Elder Cousin: Upon waking, the tongue felt spicy, and body fluids failed to ascend. Dizziness and limb numbness occurred due to ascending Yang and moving wind. The main treatment harmonized Yang and extinguished wind, assisted by sour flavors to generate fluids. Xian Shengdi (fresh Rehmanniae Radix), Yuzhu (Polygonati Odorati Rhizoma), Shihu, Baishao, Wuweizi, Huafen, Wumei (Mume Fructus), Ganju Tan, Muli Fen, Sangzhi (Mulberry Twig), and Heimaazi (Sesami Fructus Niger) were commonly taken with good effect.
Case of Jiang: In youth, physical labor damaged Yang, causing mental fatigue, dizziness, fever, bitter taste in the mouth, reduced appetite, and vomiting of turbid matter. The Cun pulses were rapid. Rebellious Qi ascended with a surging, wave-like momentum. Treatment suppressed Yang and drained turbidity. Muli, Baishao, Fushen, Juhong (Citri Reticulatae Pericarpium Rubrum), Zhi Banxia (Prepared Pinelliae Rhizoma), Wuzhuyu (Evodiae Fructus), Ganju Tan, and Gold utensils for decoction were used. After two doses, turbidity descended, vomiting stopped, but the pulse remained slightly rapid, and head/eyes were unclear. This was due to Gallbladder fire rising in spring warmth. Emulating Ye Tianshi’s method of draining Gallbladder heat: Danpi, Nen Sangye, Heyebian (Lotus Leaf Margin), Gouteng, Baishao (raw), Zhizi, and Shengdi Tan were used. After several doses, vertigo ceased and heat decreased. Sangye and Shengdi Tan were removed, and Yuzhu, Fushen, Qizi (Lycii Fructus) (baked), Shanyao (Dioscoreae Rhizoma), Shudi (all stir-fried), Lushen, Lianzi, and Zao were added. The pulse normalized.
Case of Xiao: Exposure to rain caused dampness to stagnate and transform into heat, steaming and turning yellow. Chronic ailments, exacerbated by labor, coincided with spring when deficient Yang ascended. He coughed, experienced dizziness, dry mouth, yellow eyes, palpitations, and insomnia. Treatment first cleared and drained fire and wind. Shengdi, Shihu, Zhixin (Gardeniae Cor), Fushen, Danpi, Lingyangjiao (Saigae Tataricae Cornu), Xingren, Gouteng, and Ganju (stir-fried) were used. After four doses, head and eyes cleared, palpitations ceased, appetite increased, and sleep stabilized, though mental fatigue and weakness persisted. Shengdi was removed, and Ginseng, Shaoyao, Lianzi, and Zao were added to support Spleen Yuan. After several doses, condition improved further. Later, Lingyangjiao, Xingren, Gouteng, and Ganju were removed, and Yinchen (Artemisiae Scopariae Herba) and Songluo Tea were added. Jaundice gradually receded.
