After the eruption of macules, residual toxic pathogens remain uneliminated. Concurrent consumption of pungent, spicy, and toxic foods causes these pathogens to accumulate in the Spleen and Stomach. The Spleen governs muscles; thus, when toxins manifest between the muscles, they transform into macule-herpetic carbuncles (Zhen Ding). Symptoms include persistent high fever, irritability, thirst with desire to drink, and restlessness due to heat-toxin accumulation. Treatment should utilize Chai Hu He Hua Du Tang (Bupleurum Combined Toxin-Resolving Decoction).
Following the eruption, if dietary hygiene is neglected and wind-cold is not avoided, the Spleen and Stomach are injured, allowing the pathogen to transmit to the Lung. Since the Lung and Large Intestine are exterior-interior paired organs, dysentery results. Additionally, if residual toxins remain and lingering fire is not cleared, heat shifts to the Large Intestine, causing dysentery. Treatment must prioritize clearing Metal (Lung) and cooling the Intestines. Regulating blood promotes healing of pus, while regulating Qi eliminates tenesmus. If Qi of the Bladder fails to flow freely, strangury and retention occur; if Qi of the Large Intestine stagnates, tenesmus arises; the principle is identical. Almond (Xing Ren) and Cimicifuga Rhizome (Sheng Ma) should be added to simultaneously resolve accumulation.
Inhalation of aromatic substances irritates the Lung via the nose, inevitably causing headache and rhinorrhea. Urgently administer Poria (Fu Ling), Almond (Xing Ren), Mint (Bo He), Schizonepeta (Jing Jie), and Platycodon Root (Jie Geng). Without simultaneous intake of frost-bitten eggplant, suffering will be unbearable, leading to disaster.
When fever accompanies acute abdominal pain, suspect toxin disturbance within the interior without doubt. If stool is blocked, purgative therapy is indicated; do not wait until it is too late to regret. As stated in the Classics: "Toxin retained in the abdomen causes abdominal pain as if the intestines are touched; slowly entering the Large Intestine, it expels bloody, turbid filth." Urgently administer Raw Rhubarb (Sheng Jun) and Aurantium Fructus (Zhi Ke) for recovery.
Ministerial Fire flares upward, heat steaming like vapor, throat pain is severe. Tongue root swelling impedes speech. Increase doses of Licorice Root (Gan Cao), Platycodon Root (Jie Geng), Ophiopogon Tuber (Xuan Shen), and Coptis Rhizome (Lian). Add Rhinoceros Horn (Xi Jiao - *note: substitute with Buffalo Horn in modern practice*), Gardenia Fruit (Zhi Zi), and Forsythia Flower (Lian Qiao Lian Xin). Immediate decoction and administration yield immediate results. As stated in the Classics: "Once swelling subsides and voice returns, alarm ceases." Subsequently administer Asparagus Tuber (Dong Bai Bu), Ophiopogon Tuber (Dong Mai Dong), Anemarrhena Rhizome (Zhi Mu), Ophiopogon Tuber (Xuan Shen), Mint (Bo He), Forsythia Fruit (Lian Qiao), Schizonepeta Spike (Jing Jie Sui), Scutellaria Root (Tiao Qin), Belamcanda Rhizome (She Gan), and Hedyotis Diffusa (Dou Gen).
Toxic vapors congest internally and are closed by Wind pathogens, preventing sudden eruption, resulting in convulsions, tremors, and maniacal agitation. Treatment should utilize Su Jie San augmented with Rhubarb (Da Huang) and Aurantium Fructus (Zhi Ke). For cases with closure of both urine and stool, clearing and draining methods are most appropriate.
Fever steams upward; originally, one should consider deficiency and astringency. If it does not subside, toxic vapors are not fully resolved. Clearing heat allows evil macules to dissipate naturally. Utilize Qing Biao San Du Tang (Clear Exterior Scatter Toxin Decoction), removing Angelica Sinensis (Dang Gui), Alisma Rhizome (Ze Xie), and Polyporus (Zhu Ling). Add Cortex Moutan (Pi Mu), Scutellaria Root (Huang Qin), Ophiopogon Tuber (Mai Dong), Trichosanthes Root (Hua Fen), Millettia Speciosa Root (Da Li), Asparagus Tuber (Tian Dong), Licorice Root (Gan Cao), Bupleurum Root (Chai Hu), Plantago Seed (Che Qian), and Akebia Caulis (Mu Tong).
