The true Qi vibrates internally, and the deficiency wind can be extinguished. Therefore, a large dose of Yupingfeng San (Jade Windscreen Powder) was administered, added with Beimu (Fritillariae Bulbus), Xingren (Armeniacae Semen), Suzi (Perillae Fructus), and Jiegeng (Platycodonis Radix). After one dose, the pulse and symptoms gradually harmonized. Another three qian of Renshen (Ginseng Radix et Rhizoma) was added. After two doses, all symptoms gradually subsided. Only the cough persisted, so Liuyin Tang (Six-Ingredient Rehmannia Decoction) with Renshen was used for conditioning. After one month of treatment, full recovery was achieved.
Madam Gu Zhiyan suffered from wheezing and coughing for half a year, unable to rest her head on a pillow, with a dry tongue lacking saliva. Multiple treatments were ineffective. Upon diagnosis, the Right Guan and Right Chi pulses were deficient, choppy, and lacked spirit. This indicates that the branch manifestation is in the Lung, while the root cause lies in the Kidney. The Lung serves as the pathway for exhalation, and the Kidney is the residence for grasping Qi. Now, due to Kidney Qi deficiency, inhalation fails to return to its root. The Qi of the Triple Burner exits more than it enters, causing Qi to accumulate in the upper body, resulting in wheezing, coughing, dry mouth, and inability to lie down comfortably. The *Zhongzang Jing* states: "In Yin diseases, when inhalation is impaired, this is the case." The treatment method should clear the Qi in the upper body and grasp the Qi in the lower body, allowing the Lung to achieve clarity and descent, and the Kidney to restore its storing function. Thus, Qi will naturally be grasped, and wheezing and coughing will subside. Su Zi Jiang Qi Tang (Perilla Fruit Descending Qi Decoction) with five qian of Renshen and one qian of Rougui (Cinnamomi Cortex) was used. After three consecutive doses, symptoms gradually subsided. It was then changed to Shenqi Tang (Kidney Qi Decoction) from *Jinkui Yaolue*, with five qian of Renshen. After more than twenty doses, she could sleep peacefully. Later, due to improper care, the previous symptoms recurred. She entrusted herself to mediocre physicians who exclusively used Qi-breaking and downward-purging agents, severely criticizing the use of Renshen. The patient felt her vitality failing and requested a small amount of Renshen, which was denied. Consequently, her Qi collapsed, and she died. How tragic!
Qiu You suffered from Phlegm-Wheezing with fever, thirst, chest fullness, body pain as if beaten by a stick, and frequent aversion to cold. Treatments with wind-dispelling and phlegm-resolving herbs were ineffective. Diagnosis revealed a Wiry-Rapid and Choppy pulse. This is caused by emotional stagnation leading to Lung Dryness. Current physicians only know how to dispel wind, clear heat, and resolve phlegm. Wind-dispelling herbs are Acrid, Heat-clearing herbs are Bitter, and Phlegm-resolving herbs are Drying. Acrid herbs consume fluids, Bitter herbs damage Qi, and Drying herbs stir up Fire; thus, these treatments merely exacerbate the condition. To treat Phlegm-Wheezing now, one must first clear and purify the Lung Qi. Clearing Lung Qi requires moistening Dryness first, enabling Qi to descend clearly and purely. In that case, what need would there be for Phlegm-Wheezing? (Xu Lingtai's Note: The theory is correct, but the prescription is flawed. Using Louren (Trichosanthis Semen), Banxia Qu (Pinelliae Fermentatum), Zhike (Aurantii Fructus), Qinjiao (Gentianae Macrophyllae Radix), Xingren, and Suzi, with small amounts of Chaihu (Bupleuri Radix) and Guizhi (Cinnamomi Ramulus), resulted in halving the symptoms after two doses. Adding Sheng Shouwu (Polygoni Multiflori Radix) to nourish Yin and moisten Dryness, along with unblocking the bowels, led to complete recovery.)
Miao Zhongchun treated Zang Yibu Jinghan, who suffered from wheezing, spontaneous sweating, insomnia, and loss of appetite day and night. Other physicians treated it as an External Contraction. Miao diagnosed: "This is Kidney Deficiency causing Qi to fail to return to its origin, leading to Floating Fire. Wheezing and sweating occur alternately. Spleen Deficiency causes lack of appetite." He urgently prescribed Maidong (Ophiopogonis Radix), Wuweizi (Schisandrae Fructus), and Gouqizi (Lycii Fructus) to nourish Yin and astringe the Lung; Suzi and Juhong (Tangerini Pericarpium) to descend Qi and resolve phlegm; Baishao (Paeoniae Radix Alba), Suanzaoren (Ziziphi Spinosae Semen), and Fuling (Poria) to tonify the Spleen and arrest sweating. Within a few doses, he recovered.
Du Ye suffered from Phlegm-Wheezing. Other physicians treated it as Stomach Deficiency without success. Later, Daoshui Wan (Water-Guiding Pill) was used to promote urination and defecation five or six times, after which he stabilized. (From *Pingzhi Huicui* and *Xueshi Shu*.)
Xue Zhongfang stated: Madam Zhou Zhongjie, over sixty years old, had suffered from Phlegm-Wheezing for five or six years without effect from medicine. Master Shicai diagnosed: "The Right Cun pulse is Floating and Surging, indicating Excess Pathogen in the Lung. Mahuang (Ephedrae Herba), Shigao (Gypsum Fibrosum), Banxia, Fangfeng (Saposhnikoviae Radix), Xiacha (Fine Tea), and Shengjiang (Zingiberis Rhizoma Recens) must be used. Although the medication is potent, the disease can bear it; do not fear." Administration resulted in cure. However, Mahuang should only be used in doses of 2-3 fen, and wind avoidance is necessary after ingestion.
Wu Qiao treated Cheng Canjun Wen, nearing old age, suffering from chronic Phlegm-Wheezing that worsened in autumn and winter and improved in spring and summer. Having just returned from Liudu, his Phlegm-Wheezing was as before. One physician considered it a Heat syndrome and prescribed Shigao. After two doses, Phlegm-Wheezing persisted, accompanied by diarrhea. Another physician considered it Deficiency caused by excessive purgation and prescribed strong tonification with Renshen. After one dose, Phlegm-Wheezing intensified severely, throat obstruction prevented speech, and he fell into unconsciousness. Upon arrival, Wu Qiao diagnosed: The Cun pulse was Floating and Large, Wiry and Rapid, beating against the finger, but intolerant to pressure. The illness originated from anger injuring Liver Qi. The treatment method should be gradual, but the two physicians acted hastily, worsening the condition. Although the pulse indicated near-death, a chance of life remained. He prescribed Daotan Tang (Great Phlegm-Resolving Decoction) as the base, adding Huangqin (Scutellariae Radix), Huanglian (Coptidis Rhizoma), and Maidong. After one dose, consolidated phlegm was vomited, providing slight relief and allowing brief sleep. Before finishing two doses, the consolidated phlegm surged to the throat but could not be expectorated. A hairpin was inserted to extract it, pulling out long strands resembling cotton, filling two jars with material like rotten wadding. Only then did the wheezing subside slightly. He fell unconscious again, lying stiffly like a corpse, waking only after seven days. Upon waking, he groaned and breathed, eyes slightly open, accepting spoonfuls of liquid. Medication was given every other day. After a month, he stabilized. Some compared Cheng Canjun to Zhao Jianzi. (From *Taihan Ji*.)
Liu Qingjiang said: My father once dispensed medicine for wheezing, using Mahuang three liang (not removing roots and nodes, boiled in water), and Hezi (Myristicae Semen, two liang, pitted flesh only). These two were ground into fine powder. Each dose was three large spoons, boiled in two cups of water until reduced by half, then one qian of Laicha (Preserved Tea) was added, and boiled again to eight-tenths. Taken hot, it was invariably effective. Later, he received a formula from Vice Minister Peng Zishou: Xinluoshen (Korean Ginseng, one liang) ground finely, mixed with raw egg white to form pills the size of mulberry seeds, dried in shade. Each dose was one hundred pills, taken with warm preserved tea broth, stopping wheezing immediately. I once heard Ye Shouduan, Prefect of Linjiang, say his ancestor Shilin cured this disease by taking only Dahuang (Rhei Radix et Rhizoma). His father also suffered from it and was cured solely using Fuzi (Aconiti Lateralis Radix Praeparata). For someone else, Mahuang was indispensable. Thus, it depends on individual constitution. He claimed his daughter had wheezing since childhood, passing through four generations, yet the medications differed each time. (Liu Changshi, courtesy name Yu, pseudonym Qingjiang. From *Lupu Biji*. Xu Lingtai's Note: With such transmission and varying conditions, those who apply rigid formulas to living patients are blind physicians.)
Wei Yuheng said: Zhu Wuzhang, aged thirty-eight, stayed in Yaojiang. In mid-winter, he developed a Ding sore (carbuncle/furuncle) on the left forehead. After seven or eight days, mild wheezing appeared. After the sore ulcerated, severe wheezing occurred. When the sore healed, wheezing worsened, preventing him from sitting or lying down. Physicians prescribed Qi-descending and Lung-cooling herbs without effect for twenty days. Upon returning and crossing the river, upon reaching the shore, both feet became red and swollen like lanterns, making walking impossible. Carried home, he saw seven physicians in one month. Those focusing on chest expansion used heavy doses of Houpo (Magnoliae Officinalis Cortex); those draining the Lung used harsh Tingli (Descurainiae Semen seu Lepidii Semen). Some cited unresolved Exterior Pathogen, others Foot Bi attacking upward, Edema entering the abdomen, or Ding toxin entering the Lung. Mixed treatments caused swelling to spread to the scrotum. One physician attributed it to Deficiency and prescribed Bawei Wan (Eight-Ingredient Pill), which increased delirium. Diagnosis: Both Guan pulses were indistinct, Left Chi was absent, remaining pulses were slightly Rapid and Surging. Face showed red glow, breathing labored preventing sleep, urine thick and turbid, palms hot to touch, buttocks had bedsores from prolonged sitting. Tongue left side was red-purple, edges convex and concave, worse on the left. Nostrils dry. Could bend forward but not backward. Diagnosis: This is a sign of severe Liver and Kidney injury. The initial Ding sore was also due to blazing Liver Fire. If treated appropriately, it would have healed in a few doses. Houpo and Tingli were incorrect; Rougui and Fuzi were also errors. Treating the complications requires thirty doses. Prescribed Shengdi (Rehmanniae Radix), Shudi (Rehmanniae Radix Preparata), Tian Dong (Asparagi Radix), Mai Dong, Shashen (Adenophorae Radix), Gouqizi, Louren, and Yiyiren (Coicis Semen). After four doses, swelling subsided, delirium stopped. After ten doses, urination cleared, swelling receded, and he could lie down. However, supine or right-lateral positions caused restless wheezing and coughing. After another thirty doses, he could entertain guests. Sleeping on the left was still preferred. Shudi was increased to one liang, Wuweizi three fen, and Gejie (Geckos) one piece. After one dose, stability was achieved; after four doses, full recovery.
Wu Xingquan suffered from wheezing since childhood. Pediatricians prescribed Zhike, Jiegeng, Ju Hong, Banxia, Sangbaipi, Xingren, Qianhu, and Suzu, damaging his Lung Qi, leading to a chronic ailment. Each episode lasted for days or weeks. At seventeen, I diagnosed him and prescribed heavy doses of Liver and Kidney tonics, including Shashen, Louren, and Maidong. Since then, episodes became milder, occurring every few months. Using modified prescriptions, he recovered quickly within a few doses. Recently, he rarely suffers attacks throughout the year. I have treated many wheezing cases, mostly successfully with this method. Approximately ninety percent of wheezing cases are Deficiency-type, and ten percent are Excess-type.
Lady Jin, over forty, suffered from wheezing due to excessive purgation, becoming a chronic ailment by nearly seventy. Suddenly, a physician prescribed Sanzi Yangqin Tang (Three-Seed Nourishing Elderly Decoction) with Tingli. After ingestion, she experienced hypochondriac pain, Cold limbs, and impending collapse. I used a large dose of Gouqizi and Dihuang, adding one Chuanlian (Toosendan Fructus), alleviating the condition. Lanting, her fourth son, also suffered from wheezing, with a pale complexion, requiring days in bed during attacks. He was given herbs to nourish Water, tonify Liver, and support Metal. Attacks became less frequent and severe. He claimed recovery after consuming Goubao (Dog Gallstone). For this syndrome, if the face is pale white, skin tight, phlegm foul-smelling, and urination scanty, it is incurable. This is not Lung Abscess, but destruction of Lung tissue. The Lung is the source of Water; once damaged, urination must be scant.
Zhang Simen, over sixty, addicted to alcohol, suffered from wheezing, profuse sputum, exertional dyspnea, shoulder shrugging, and gasping. Others prescribed Xingren, Zhike, Suzi, and Qianhu. After more than ten doses, wheezing worsened, leaving him emaciated as a ghost. He declined further treatment. I prescribed Shengdi, Shudi, Tiandong, Maidong, Yiyiren, Louren, Shashen, Gouqizi, Zhizi (Arctii Fructus), and Nvzhenzi (Ligustri Lucidi Fructus). After eight doses, full recovery. Advised abstinence from alcohol. Initially restrained, but later indulged. Relapse occurred after a year. Left pulse felt like plucking a lute string, indicating True Zang Pulse appearance. No medication was given; he died over a month later.
Zhang Youqiao treated a wine drinker suffering from summer Phlegm-Wheezing and dyspnea, unable to sleep at night. Cooling and Qi-opening herbs were ineffective. Some suggested Maidong Tang from *Jinkui Yaolue*. Zhang diagnosed: Right Cun pulse was Rapid and Solid, indicating Lung Excess, not Deficiency. Administering Renshen would cause immediate death. Instead, Tingli five qian (baked and powdered) and Huashi (Talcum) five qian were decocted and taken, resulting in immediate cure. The following year, re-infected by external pathogens, obstructing Lung Qi, wheezing and coughing recurred. Physicians used Tingli without effect, causing irritability and sweating. Zhang diagnosed: Right Cun pulse Floating and Rapid, thirst, aversion to heat, spontaneous cold sweat, urgent wheezing, and irritability. He stated: "This is Heat Pathogen obstructing internal Lung Qi, extreme stagnation forcing sweat outward. It is not Qi Deficiency spontaneous sweating. Irritability after Tingli is due to extreme Lung Heat rejecting Bitter-Cold herbs. The Lung dislikes Qi rebellion, ideally treated with Bitter to drain. But the Lung desires dispersion, requiring Acrid to disperse rapidly." He prescribed Maxing Shigan Tang (Ephedra, Apricot Kernel, Gypsum, and Licorice Decoction). After one dose, Lung Qi flowed freely, wheezing stopped, sweat ceased, and all symptoms resolved.
Xingren Jian (Apricot Kernel Decoction): Xingren one jin, peeled and cored, pounded and cooked into a paste. Baimi (White Honey) five he, Suyou (Butter Oil) five he, prepared with cow milk. Shengjiang汁 (Fresh Ginger Juice) three he. Add three sheng of water to the Xingren paste, boil and stir until reduced by half. Add ginger juice, boil to thin syrup consistency. Add butter and honey, boil to thick syrup. Take one spoon per dose, three times daily, once at night, gradually increasing to two spoons. Alternative: Add Beimu eight he, sifted into powder. Suzi汁 (Perilla Fruit Juice) one sheng, made from grinding seven small he of Suzi, filtered with water. Alternative: Add Shengdi汁 (Rehmannia Juice) three sheng, Maidong five he. Wang Jinsan stated: This is a Moistening Agent, treating fatigue with exterior laxity, Cold invading the Lung, upper Qi dry cough, and Lung萎 (Flaccidity) with aphonia. The combination of slippery and moistening substances avoids intestinal diarrhea. Xingren enters the Lung, specializing in descending rebellious Qi and stabilizing wheezing. Honey provides lubrication, butter oil provides slipperiness. Even with ginger juice ascending, they seek similar energies. It forces stagnation in the Middle Jiao, harmonizes Spleen and Stomach, generates Lung fluids, and stops dry cough. Adding Suzi and Beimu descends Fire in the Qi level. Adding Dihuang and Maidong clears Fire in the Blood level. Selection based on syndrome differentiation is left to the practitioner's discretion.
Huang Jinfang treated Zhu Mou suffering from wheezing and coughing, restlessness day and night, alternating chills and fever, bilateral headache, constipation and urinary retention, frequent shoulder shrugging, and rushing upward Qi. Pulses: Both Chi pulses very weak, both Cun pulses very Surging. This indicates Qi rising but not descending. Used Wuweizi thirty grains, Guzhi (Psoraleae Fructus) six fen, Chenxiang (Aquilariae Lignum Resinatum) three fen, Yuanzhi Rou (Polygalae Radix Flesh) five fen to calm the Right Kidney; Longgu (Fossil Bones) one qian, Niuxi (Achyranthis Bidentatae Radix) one qian, Cheqian (Plantaginis Semen) four fen, Guiban (Testudinis Carapax et Plastrum) one qian to calm the Left Kidney. This ensured Qi from both Kidneys returned to their origin without rushing upward. After one dose, wheezing and coughing ceased, headache and chills/fever disappeared. After another dose, bowel movements and urination normalized, leading to cure.
